Commotio cordis.

Sudden arrhythmic death as a result of a blunt chest wall blow has been termed Commotio Cordis (CC). CC is being reported with increasing frequency with more than 180 cases now described in the United States Commotio Cordis Registry. The clinical spectrum is diverse; however young athletes tend to be most at risk, with victims commonly being struck by projectiles regarded as standard implements of the sport. Sudden death is instantaneous and victims are most often found in ventricular fibrillation (VF). Chest blows are not of sufficient magnitude to cause any significant damage to overlying th

The Brugada Syndrome.

In 1992 a syndrome was described consisting of syncopal episodes and/or (resuscitated) sudden death in patients with a structurally normal heart and a characte ristic electrocardiogram (ECG) displaying a pattern resembling a right bundle branch block with ST segment elevation in leads V1 to V3. The disease is genetically determined with an autosomal dominant pattern of transmission in 50% of the familial cases. Several different mutations have been identified affecting the structure, function and trafficking of the sodium channel. The syndrome is ubiquitous. Its incidence and prevalence are di

Cardiac assessment of veteran endurance athletes: a 12 year follow up study.

OBJECTIVES: Sustained aerobic dynamic exercise is beneficial in preventing cardiovascular disease. The effect of lifelong endurance exercise on cardiac structure and function is less well documented, however. A 12 year follow up of 20 veteran athletes was performed, as longitudinal studies in such cohorts are rare. METHODS: Routine echocardiography was repeated as was resting, exercise, and 24 hour electrocardiography. RESULTS: Nineteen returned for screening. Mean (SD) age was 67 (6.2) years (range 56-83). Two individuals had had permanent pacemakers implanted (one for symptomatic atrial fi

[Calcitonin-induced anaphylactic shock. Case report and review of the literature].

A case of anaphylactic shock determined by intramuscular administration of a dose of synthetic calcitonin in a 64-years-old man is described. The patient had not suffered significant cardiovascular events in the past; he smoked twenty cigarettes a day and he was treated with calcitonin for osteoporosis and polyarthrosis. Allergy to diclofenac was demonstrated in the past while preceding administrations of spray calcification didn't provoke side-effects in the patient. Nevertheless after the second i.m. administration of the drug he suddenly fainted. Dyspnea, severe hypotension and maculo-papul

The incidence of atrial arrhythmias during inferior wall myocardial infarction with and without right ventricular involvement.

The atrial arrhythmia profile during inferior wall acute myocardial infarction (AMI) has not been systematically examined with respect to right ventricular (RV) involvement. To this end, 62 consecutive patients with first inferior wall AMI and no other conditions known to increase susceptibility for rhythm disturbances were studied by 24-hour Holter monitoring during the first and tenth day of infarction. Based on radionuclear ventriculography performed on day 2 of infarction, patients were allocated to two groups: group A--36 patients (58%) with right ventricular ejection fraction (RVEF) less

Natural history of patients with unexplained syncope and a nondiagnostic electrophysiologic study.

The purpose of this study was to define the natural history of 99 patients with unexplained syncope who underwent an electrophysiologic test that either was entirely normal or demonstrated nonspecific abnormalities that were nondiagnostic (inducible polymorphic ventricular tachycardia or ventricular fibrillation, a mildly prolonged sinus node recovery time of less than 2 s, a His-ventricular interval of 55 to 99 ms or supraventricular tachycardia not associated with hypotension). The mean age (+/- SD) of the patients was 56 +/- 19 years; structural heart disease was present in 47 patients and

A Case of Severe Hyponatremia Unmasking Borderline Heart Failure With Preserved Ejection Fraction With Right Ventricular Dysfunction.

Hyponatremia, characterised by a low serum sodium level, is the most common electrolyte abnormality encountered in clinical practice. Its diverse etiologies and varied clinical presentations can make diagnosis and management complex, especially in the context of borderline heart failure with preserved ejection fraction (HFpEF). We report the case of a 53-year-old male who presented with slurred speech, generalised weakness, shortness of breath, and bilateral leg swelling over eight weeks. Initial investigations revealed severe hypotonic hyponatremia (sodium = 97 mmol/L), elevated urine osmolal

Autodecremental pacing--a microprocessor based modality for the termination of paroxysmal tachycardias.

Five patients aged between 27 and 48 years were referred for investigation of recurrent paroxysmal tachycardias. Electrophysiological studies revealed concealed ventriculoatrial accessory pathways in two patients, possible atrionodal pathways in two patients and dual intranodal pathways in one patient. During electrophysiological study, particular attention was paid to methods of terminating tachycardia by pacing techniques including single or double atrial and ventricular extrastimuli, atrial or ventricular underdrive, atrial overdrive pacing, and in two patients, rapid ventricular pacing. Predictors of appropriate implantable cardioverter-defibrillator therapy during long-term follow-up of patients with coronary artery disease.

Indications for implantable cardioverter defibrillators (ICDs) are expanding. Defining long-term predictors of ICD therapies might help to identify those patients who will benefit most from implantation of an ICD. The objective of this study was to examine long-term predictors of appropriate ICD therapy among patients with coronary disease at high risk of sudden cardiac death. An analysis of 245 patients with coronary disease, who had been implanted with an ICD for primary or secondary prevention of sudden cardiac death, was performed. Time to appropriate ICD therapy, defined as antitachycardi

Cardioneural and accessory atrioventricular pathway ablation for vagally mediated atrial fibrillation in PRKAG2 syndrome.

Cardioneural ablation (CNA) is an emerging strategy for vagally mediated arrhythmias, but its role in patients with underlying conduction system disease remains undefined. Here, a young patient with PRKAG2-mediated cardiomyopathy presented with refractory paroxysmal atrial fibrillation (AF) despite prior accessory atrioventricular pathway (AP) ablation and antiarrhythmic therapy. Episodes of AF were triggered consistently by cold milk ingestion, and dual-chamber pacemaker interrogation revealed a high atrial pacing burden due to sinus pauses, suggesting a vagally mediated mechanism superimpose

Gene Therapy for Inherited Cardiac Arrhythmias.

Inherited cardiac arrhythmias are genetic disorders that impact the heart's electrical circuitry. These disorders are caused by inherited mutations and markedly increase the mortality risk of affected individuals. Conventional treatments, such as pharmacological agents for rate and rhythm control or surgical interventions, are often not efficacious and cause long-term complications. Gene therapy has recently emerged as a feasible and potentially curative approach for treating inherited cardiac arrhythmia syndromes. Advancements in elucidating the molecular mechanisms, pinpointing essential can

Aortic Thrombus Formation in the Mixing Zone of a Patient With Acute Myocarditis.

A 66-year-old man presented with chills, exertional dyspnea, and palpitations; he later developed a fever. Because of his elevated cardiac enzymes and electrocardiography and coronary angiography findings, he was diagnosed with acute myocarditis. Given his unstable hemodynamics, an intra-aortic balloon pump was inserted; however, he experienced a hemodynamic collapse due to refractory ventricular fibrillation. He was then cannulated with venoarterial extracorporeal membrane oxygenation and microaxial flow pump and underwent steroid pulse therapy. His lactate levels were elevated, and computed
