T cell immunodeficiency.

T cell immunodeficiency can occur as one of a group of primary disorders or develop secondary to chronic infection, illness or drug therapy. Primary T cell disorders are rare, accounting for approximately 11% of reported primary immunodeficiencies, and generally present in infancy or early childhood. Early recognition is very important as many of these patients will require bone marrow transplantation prior to the onset of severe infection or other complications. Because of their rarity, these infants usually present to clinicians who have little or no prior experience of these conditions, and…

Utility of store-and-forward pediatric telecardiology evaluation in distinguishing normal from pathologic pediatric heart sounds.

Because pediatric cardiologists can accurately diagnose innocent murmurs by physical exam alone, the authors developed a system for remote c … ther normal/innocent or pathologic, with a sensitivity of 82% and specificity of 86%. However, when accounting for clinical diagnosis, reviewer uncertainty, and ECG abnormalities, the sensitivity and specificity improved to 91% and 88% (accuracy 89%), respectively. Degree of certainty with the telecardiology diagnosis correlated with correct interpretation ( …

Otolaryngologists and orbital pseudotumor: a case report.

Orbital pseudotumor (idiopathic orbital inflammation) is the third most common orbital disease, accounting for 5 to 6% of orbital disorders. It often presents in a manner very similar to inflammatory or neoplastic sinus disease. In addition, sinus disease is often a cause of orbital pseudotumor, although the etiology of this entity has yet to be defined. Treatment with steroids is usually successful early on, but long-term treatment-failure and recurrence rates are high. Despite otolaryngologists' intimate knowledge of sinus pathology, we have generally not been heavily involved in researching…

Rheumatic fever and rheumatic heart disease: primary prevention is the cost effective option.

OBJECTIVE: To measure the economic output/input ratios for the various options of prevention of rheumatic fever/rheumatic heart disease (RF/RHD) and check the viability of primary prevention vis-à-vis secondary and tertiary preventions. METHODS: Cost accounting of the various prevention options was calculated for each variable as available in literature. Actual data as obtainable for the financial year ending March 2006 were computed for the Pondicherry population. Both direct and indirect costs (including community/social costs) were worked out using mostly primary data and wherever necessar…

[Diagnostic evaluation of temporal paragangliomas].

Paragangliomas are rare tumours deriving from paraganglia--small bodies of neuroendocrine tissue. Approximately 3% of paragangliomas occur in the head and neck area accounting for about 0,6% of all tumours of this region. Head and neck paragangliomas are divided into two groups: neck tumours (carotid body tumor and vagal paraganglioma) and temporal paragangliomas (jugular bulb tumor and tympanic paraganglioma). Temporal paragangliomas arise from paraganglia located in the petrous part of the temporal bone. Radiological evaluation of these lesions is essential for confirming the diagnosis and s…

Test meal intake in obese binge eaters in relation to impulsivity and compulsivity.

OBJECTIVE: Studies have linked increased impulsivity and compulsivity with bulimia nervosa (BN). Less is known about this relationship in bi … significantly higher compulsivity scores than women. Impulsivity correlated significantly with TMI, accounting for 16% of the variance. CONCLUSION: There was greater impulsivity in BED and BE, compared with non-BED. Moreover, impulsivity was the best predictor of TMI, and may play a larger role in BE than previously realized.

Congenital and neonatal pneumonia.

The greatest risk of death from pneumonia in childhood is in the neonatal period. It is estimated that pneumonia contributes to between 750000-1.2 million neonatal deaths annually, accounting for 10% of global child mortality. Congenital and neonatal pneumonias are often a difficult disease to identify and treat, with clinical manifestations often being non-specific. Many of the normal lung defences are compromised in the fetus and neonate, leading to an increased susceptibility to infection. The aetiology and epidemiology of congenital and neonatal pneumonias will depend on the clinical setti…

Determinants of social functioning in bipolar disorder.

BACKGROUND: Evidence indicates that social outcomes in bipolar disorders (BP) are poorer than previously assumed. This study explores determinants of the quality and quantity of social activity. METHODS: Seventy-seven subjects with BP completed self-report and observer-rated questionnaires assessing demographic and clinical factors, personality and social functioning. RESULTS: Quality of functioning and spectrum of activity were found to be independent. The best determinants of the quality of functioning, accounting for 33% of the variance, were premorbid neuroticism and the current level of…

Rapid fertility decline in Iran: analysis of intermediate variables.

The remarkable decline in fertility in Iran, which saw the total fertility rate fall from 7 children per woman in 1986 to 2 in 2000, has received only limited analysis in the demographic literature. Using the 2000 Iran Demographic and Health Survey and Bongaarts' age-specific fertility model, this paper examines the role of the major proximate determinants of fertility in bringing about the rapid decrease in fertility in Iran. The analysis indicates that contraception had the largest effect on fertility, accounting for 61% of the reduction in fertility from its theoretical maximum. The fertili…

Sensitivity of mammography and ultrasonography on detecting abnormal findings of ductal carcinoma in situ.

OBJECTIVE: To analyze the mammographic and ultrasonographic findings of ductal carcinoma in situ (DCIS) and determine the sensitivity in Tha … ses as negative findings. CONCLUSION: Microcalcifications are characteristic findings in mammogram accounting for 81% of DCIS in the present study. Ultrasonography shows abnormalities including mass and thickened duct lesions in 54% of DCIS. The combined modalities can give the detection of abnormalities in 97% of DCIS.

[Antiphospholipid syndrome and preeclampsia].

Preeclampsia is defined as gestational hypertension plus proteinuria (≥ 300 mg/day). It is a common disorder during pregnancy, accounting for about 20% of fetal mortality occurring after the 20th week of gestation. Preeclampsia is considered severe when it involves severe gestational hypertension and/or proteinuria ≥ 5 g/day, or when it is associated with pulmonary edema, oliguria, thrombocytopenia, elevation of liver enzymes, or involvement of the central nervous system. Antiphospholipid antibody syndrome (APS) is at present considered one of the predisposing conditions for the occurrence of …

Risk of osteoporosis in endocrine disorders and celiac disease.

Osteoporosis is characterized by a loss of bone mass; the bones become less dense, fragile and prone to fracturing. It is regulated by endocrine-environmental factors with the genetic component accounting for 70% of an individual's variation in bone mass density (BMD). Pathological conditions such as celiac disease (CD) exacerbate the process of bone loss and the presence of osteoporosis in celiac subjects may be the only sign of undiagnosed CD. The interleukins IL-1alpha and IL-1beta are stimulators of bone resorption; the relatives of celiac patients shown the increased IL-1beta supporting t…

Acute stridor caused by a previously asymptomatic large oropharyngeal spindle cell lipoma.

Lipomas of the larynx, oropharynx and hypopharynx are rare, accounting for approximately 1% of benign laryngeal neoplasms. Spindle cell lipoma is a rarer histological variant characterised by mature adipocytes mixed with collagen-forming spindle cells. We present an unusual case of a 62-year-old lady who acutely presented to the emergency department with a large spindle cell lipoma arising from the vallecula. This lipoma was causing intermittent stridor unless traction was applied to it. Despite the size of the lesion, she had been completely asymptomatic prior to this single episode. To our k…

Renal artery stenosis: an update on diagnosis and management.

Reno-vascular disease, along with diabetes mellitus, is the leading cause of dialysis in the elderly population, accounting for 50-66% of cases in patients above 65 years of age. Reno-vascular disease is a broad term, which includes renal artery stenosis, ischemic nephropathy, such as atherosclerotic obstruction, thrombo-embolic phenomenon, nephrosclerosis secondary to hypertension and acute occlusion of renal arteries (either bilateral or unilateral in singlekidney patients). Renal artery stenosis, defined as a 50% or greater occlusion of a renal artery (unilateral or bilateral), is an import…

The role of learned irrelevance in attentional set-shifting impairments in Parkinson's disease.

In this study, the cognitive and neurochemical factors underlying learned irrelevance, one of the mechanisms thought to be responsible for a … ing their working memory. These results confirm that learned irrelevance is a significant factor in accounting for attentional set-shifting deficits in patients with PD, although unlike other executive impairments in this group, the phenomenon appears to be unrelated to their central dopaminergic deficit.

Spastin gene mutations in Bulgarian patients with hereditary spastic paraplegia.

Hereditary spastic paraplegia (HSP) is an extremely heterogeneous group of neurodegenerative disorders affecting the longest axons in the central nervous system. The most common genetic form accounting for about 40% of the autosomal-dominant HSP (ADHSP) cases is spastin gene, SPG4. We performed mutation screening of the spastin gene on 36 unrelated HSP patients from three different ethnic groups (Bulgarian, Turks and Gypsies) and found four new mutations and one already reported. The phenotype-genotype correlations in Bulgarian SPG4 patients showed a great difference in the age at disease onse…

Immune-mediated inner ear disease.

The incidence of autoimmune inner ear disease (AIED) is difficult to determine: probably it is a rare disease, accounting for <1% of all cases of hearing impairment or dizziness. Nevertheless, the diagnosis of AIED might be overlooked because of the lack of a specific diagnostic test. The hallmark of this clinically diagnosed condition is the presence of a rapidly progressive, often fluctuating, bilateral sensorineural hearing loss (SNHL) over a period of weeks to months. The progression of hearing loss is too rapid to be diagnostic for presbycusis and too slow to conclude a diagnosis of sudde…

Multi-exon out of frame deletion of the FBN1 gene leading to a severe juvenile onset cardiovascular phenotype in Marfan syndrome.

Marfan syndrome is caused by mutations in fibrillin-1, a large gene spanning approximately 200 kb of genomic DNA on chromosome 15q21. So far, more than 600 different mutations have been identified, accounting for 60-90% of all Marfan syndrome cases, the vast majority being single nucleotide exchanges as well as small deletions and insertions. Only four major rearrangements have been described in the literature so far. We have screened 11 individuals fulfilling the diagnostic criteria of Marfan syndrome but negative for point mutations in the fibrillin-1 gene by SSCP and/or direct sequencing, f…
