Conservative functional treatment of ankle fractures.

Thirty-eight patients (mean age 49 years; range 19-91 years; nine of them over 60 years; 28 women, 10 men) suffering from an isolated Weber B fracture with a dislocation of less than 1 mm underwent functional therapy using a pneumatic ankle brace and were included in a prospective study. The clinical outcome was measured according to the Olerud-Molander ankle score. Functional therapy was finished in 34 cases successfully. Twenty-one patients were scored after 17 months on average (range 8-27 months) with the Olerud-Molander ankle score. A very good result was seen in 18 patients, including 12

Contribution of individual projections alone and in combination for radiographic detection of ankle fractures.

OBJECTIVE: We wanted to determine whether the standard three-view ankle radiographic series could be replaced by a two-view combination, and if so, which two-view combination (anteroposterior with lateral or mortise with lateral) would be superior. MATERIALS AND METHODS: During a 12-month period, we retrospectively reviewed 556 consecutive ankle radiographic studies consisting of anteroposterior, mortise, and lateral views. One hundred twenty patients with at least one ankle fracture were paired with 140 healthy control subjects. Each image in the three-view examination was separated and sort

Stability of the braced ankle. A biomechanical investigation.

We measured the bare ankle and the braced angle-torque relationships in 12 uninjured volunteers under static and dynamic conditions within the full range of inversion motion. These relationships were measured with a specially designed mechanical device that allowed inversion movements with angular velocities up to 850 deg/sec. In testing the bare ankle under static conditions, the torque showed a 10-fold increase within the full range of motion (average, from 0.9 N-m at 7 degrees to about 8 N-m at 48 degrees of inversion). The slope of the angle-torque relationship increased under dynamic cond

[Therapy and results of fresh lateral instability of the upper ankle joint].

During a retrospective study we especially examined the late outcome in therapy of injuries of the lateral capsula ligament-lesions in 210 patients. In addition we scrutinized the literature on this subject within the last 60 years, concerning the results of different therapy methods were checked. 203 collective writings on this subject, 23,279 patients and our own results achieved in the BG Accident Hospital in Murnau (Germany) all added up to 23,389 patients. We did not only observe the subjective opinion regarding the outcome of the therapy but also the socio-economic factors, such as the d

Operative treatment of ankle fracture-dislocations. A follow-up study of 306/321 consecutive cases.

In a prospective study of 321 consecutive cases of dislocation ankle fractures, operatively treated according to the AO (ASIF) principles, 306 cases (95%) were followed up two to six years after surgery. The infection rate was 1.8% with no septic arthritis. The clinical results were "excellent and good" in 82%, "acceptable" in 8%, and "poor" in 10%. Posttraumatic arthritis occurred in 14% and was significantly more common among middle-aged women. There was a strong correlation between the degree of arthritis and the clinical result. A computer analysis (AID) revealed that the most decisive fac

[Post-traumatic epiphysiodesis of the distal end of the tibia in children].

PURPOSE OF THE STUDY: Thirteen distal traumatic epiphyseal closures of the lower end of the tibia are studied. A bony bridge resection was performed in nine cases. The aim of this study was to define the kind of injury that drives to epiphysiodesis in this localisation, and secondly to define the factors that may influence the results of de-epiphysiodesis. MATERIALS AND METHODS: One hundred and eighty-eight children were hospitalized between 1981 and 1995 for the treatment of a fracture of the lower end of the tibia. Six epiphysiodesis followed a Mac Farland lesion. Six epiphysiodesis followe

Japanese Spotted Fever Complicated with Cholecystitis: A Case Report.

INTRODUCTION: Japanese spotted fever (JSF) is a tick-borne infection caused by Rickettsia japonica, classically presenting with the triad of fever, rash, and eschar, and characterized by systemic vasculitis resulting from endothelial injury. Concomitant cholecystitis associated with JSF is exceedingly rare. CASE PRESENTATION: We report the case of a 77-year-old man who presented with fever, was diagnosed with calculous cholecystitis at a referring hospital, and underwent laparoscopic cholecystectomy. Postoperatively, erythema developed on the extremities and trunk, along with an eschar on the

Medial malleolar fractures: current treatment concepts.

The medial malleolus, once believed to be the primary stabilizer of the ankle, has been the topic of conflicting clinical and biomechanical data for many decades. Despite the relevant surgical anatomy being understood for almost 40 years, the optimal treatment of medial malleolar fractures remains unclear, whether the injury occurs in isolation or as part of an unstable bi- or trimalleolar fracture configuration. Traditional teaching recommends open reduction and fixation of medial malleolar fractures that are part of an unstable injury. However, there is recent evidence to suggest that nonope

MRI of the tibioastragalus anticus of Gruber muscle: a rare accessory muscle and normal anatomical variant.

We present the case of a 31-year-old man who sustained a hyperplantar flexion injury of his right ankle, and was evaluated using computed tomography and MRI to assess for osseous and ligamentous injury. The MRI and CT studies demonstrated a tibioastragalus anticus of Gruber (TAAG) muscle in the lower limb's anterior compartment. To our knowledge, the imaging of this muscle has not been previously described. The TAAG muscle arises from the lower third of the anterolateral tibia and the interosseous membrane. Its tendon passes laterally, deep to the tibialis anterior and extensor hallucis longus

Driving after orthopaedic surgery.

The decision to drive after orthopaedic injury or surgery is fraught with legal and safety issues. Although driving is an important part of most patients' lives, there are no well-established guidelines for determining when it is safe to drive after injury or treatment. Typically, impairment in driving ability is measured by changes in the time needed to perform an emergency stop. Braking function returns to normal 4 weeks after knee arthroscopy, 9 weeks after surgical management of ankle fracture, and 6 weeks after the initiation of weight bearing following major lower extremity fracture. Analysis of muscle function in the lower limb after fracture of the diaphysis of the tibia in adults.

We examined the recovery of power in the muscles of the lower limb after fracture of the tibial diaphysis, using a Biodex dynamometer. Recovery in all muscle groups was rapid for 15 to 20 weeks following fracture after which it slowed. Two weeks after fracture the knee flexors and extensors have about 40% of normal power, which rises to 75% to 85% after one year. The dorsiflexors and plantar flexors of the ankle and the invertors and evertors of the subtalar joint are much weaker two weeks after injury, but at one year their mean power is more than that of the knee flexors and extensors. Our f

Single-Stage Fixation of Ipsilateral Tibial Plateau and Tibial Plafond Fractures: A Case Report.

BACKGROUND: Simultaneous ipsilateral fractures of the proximal and distal tibial articular ends are uncommon. Although this pattern has sometimes been described as a 'floating tibia', the term is not standardised. Management is dictated by fracture morphology, soft-tissue status, patient physiology and the ability to obtain stable articular reduction without additional soft-tissue injury. CASE PRESENTATION: A 45-year-old man sustained a closed Schatzker VI tibial plateau fracture, an ipsilateral partial-articular distal tibial plafond fracture documented as AO/OTA 43-B2 and a comminuted fibul

Are peroneal tendons safe after intramedullary fibular nailing? A prospective MRI-based cohort.

BACKGROUND: Intramedullary fibular nailing (IFN) offers a minimally invasive alternative to plate fixation for unstable ankle fractures, yet concerns persist regarding potential iatrogenic injury to the peroneal tendons during portal creation. No previous in vivo study has evaluated tendon integrity after IFN using advanced imaging. In this cohort, we examined peroneal tendon morphology and syndesmotic reduction following IFN using serial postoperative magnetic resonance imaging (MRI), comparing findings with a healthy control group. METHODS: A prospective cohort of 102 adults with unstable W

Neuromuscular characteristics of individuals with chronic ankle instability during unilateral landing tasks: a meta-analysis of electromyographic studies.

BACKGROUND: Chronic ankle instability (CAI) is a prevalent consequence of lateral ankle sprain. Electromyographic (EMG) investigations examining neuromuscular control during unilateral landing tasks have produced inconsistent findings, particularly during the critical post-initialcontact (post-IC) phase. This systematic review and meta-analysis aimed to synthesize current evidence on post-IC muscle activation patterns in individuals with CAI during unilateral landing tasks. METHODS: This PRISMA-compliant review (PROSPERO: CRD420261415793) searched PubMed, Scopus, and Web of Science from incep

Influence of bionic footwear on lower limb biomechanics across running experience levels: a controlled laboratory study.

INTRODUCTION: While the biomechanics of lower extremity during running and the impact of conventional running shoes on these traits have been extensively investigated, the influence of bionic shoes on runners remains largely, especially those runners with different experience levels. The aim of this study was to evaluate the biomechanical differences between experienced and novice runners when wearing two distinct types of footwear: bionic shoes and neutral shoes. METHODS: Fourteen healthy male heel-strike runners participated and completed the running test wearing two pairs of running shoes

The Fracture Ankle Implant Review (FAIR) study : a national multicentre retrospective review of practice in implant choice, fixation methods, and outcomes in fixation of the fibula in ankle fractures.

AIMS: In fixation of the fibula in ankle fractures, the AO Foundation advocates using a lag screw and one-third tubular neutralization plate for simple patterns. Where a lag screw cannot be placed, bridging fixation is required. A local pilot service evaluation previously identified variance in use of locking plates in all patterns with significant cost implications. The Fracture Ankle Implant Review (FAIR) study aimed to evaluate current practice and implant use across the UK, and review outcomes and complication rates between different fibula fixation methods. METHODS: The study was support

Usefulness of Suture-Tape Augmentation Based on Intraoperative Ankle Stress Radiographs During Anatomical Ligament Repair for Chronic Lateral Ankle Instability.

BACKGROUND: During the modified Broström procedure for chronic ankle instability, surgeons often experience cases with insufficiently restored mechanical stability even after anatomical ligament repair. In these cases, longer postoperative immobilization and delayed rehabilitation can be required, and a risk for recurrence of instability may persist. This study aimed to evaluate the clinical usefulness of suture-tape augmentation based on intraoperative stress radiographs during anatomical ligament repair for chronic ankle instability. METHODS: Twenty-four patients who underwent additional su

Computer-Controlled Cooling in Operatively Treated Ankle or Hindfoot Fractures: A Retrospective Case-Control Study.

Ankle and hindfoot fractures are often associated with a considerable amount of pain and need for systemic analgesics. Cooling devices have been developed to reduce swelling, pain, analgesics need, and complications. The primary aim was to examine the effect of cooling versus no cooling on pain levels in adult patients treated operatively for an ankle or hindfoot fracture. Secondary aims were to assess the effect of cooling on (1) analgesics use, (2) patient satisfaction, (3) hospital length of stay (HLOS), (4) the rate of complications, and (5) the rate of secondary interventions. In this sin
