Self-report sleep habits as predictors of subjective sleepiness.

Although sleepiness is pervasive in our society, there is little agreement on how to measure sleepiness or on how well sleepiness is actually related to sleep habits. To better assess how subjective sleepiness is related to sleep, the authors used self-report measures of sleep quantity, sleep quality, and napping to predict 4 different sleepiness-related measures in a group of healthy young and middle-aged-to-older adults. A forward regression analysis indicated that sleep quality was better than sleep quantity as a predictor of participants' sleepiness. The sleep measures, furthermore, predic

Morbidity and mortality following bone marrow transplantation: predictive utility of pre-BMT affective functioning, compliance, and social support stability.

This study investigated the utility of 3 pretransplant psychological variables (affective functioning, compliance, social support stability) in predicting subsequent bone marrow transplantation (BMT) health outcomes. The pre-BMT psychological evaluations of 92 BMT recipients were coded along the specified psychological dimensions and used to predict post-BMT survival status and health-related quality of life. Data analyses showed that, in addition to medical risk status (low) and quality of the marrow graft (histocompatible), higher levels of pre-BMT affective functioning and social support st

Hostility-related differences in the associations between stress-induced physiological reactivity and lipid concentrations in young healthy women.

We examined the relations of fasting lipid levels to stress-induced neuroendocrine and cardiovascular responses as a function of hostility in 36 healthy young women. Participants were women who scored above 17 (n = 23) or below 12 (n = 13) on the Cook-Medley Hostility (Ho) Inventory. Lipids were determined following an overnight fast. Individuals participated in a solvable anagram task, which evoked significant physiological responses in all participants. The Ho group by total serum cholesterol (TSC) and the Ho group by low-density lipoprotein (LDL) cholesterol interactions significantly predi

Aberrant parasympathetic and hemodynamic function distinguishes a subgroup of psychologically distressed individuals with asymptomatic type-I diabetes mellitus.

In a previous study, a subgroup of asymptomatic insulin-dependent diabetic individuals (termed IDDM-2) were identified on the basis of diminished parasympathetic cardiac input and elevated heart rate at rest. When compared to another group of asymptomatic IDDM participants (termed IDDM-1), and a nondiabetic healthy control group, the IDDM-2 group displayed elevated blood pressure, supported by elevated total peripheral resistance. Measures of psychological regulation were also taken in this study, and form the basis of this article, which examined whether these IDDM-2 patients differed from th

Comparison of clinic, home self-measured, and work self-measured blood pressures.

Self-measured and clinic blood pressure (BP) estimates of patients' true levels of BP were compared as part of a controlled study of stress management training for essential hypertension. Forty-three patients underwent 6 to 9 clinic measurements and made 48 self-measured readings of BP at home and at work before and after treatment. Analyses showed that (a) self-measured BP values at work and at home were significantly correlated with each other but did not correlate with clinic blood pressure measurements at pretreatment, and (b) test-retest correlations over 2 months and intraclass correlati

How daughters of women with breast cancer cope with the threat of the illness.

Coping strategies and attitudes toward medical treatment of 45 daughters of women who had had breast cancer were compared with strategies (sense of coherence) and attitudes on a health opinion survey of 51 women with no such family history who were referred to a breast health clinic in northern Israel. The daughters of women with breast cancer ranked lower on sense of coherence than the women with no such history. They also were more actively involved in the medical setting and requested more medical information than women whose mothers did not have breast cancer. The age of the daughter at th

Adherence to a prophylactic medication regimen in patients with symptomatic versus asymptomatic ischemic heart disease.

Although angina pectoris is the most common symptom of coronary artery disease, some patients do not experience angina during ischemic episodes. The effects of asymptomatic (silent) heart disease on patient self-management have rarely been studied. Studies of other patient populations with asymptomatic illnesses indicate that patients with silent myocardial ischemia might adhere less well to a prophylactic medication regimen than would those with symptomatic ischemia. Depression, a state associated with poor adherence to medical regimens is more common among patients with symptomatic ischemia.

Acute stress modulates symptom awareness and hormonal counterregulation during insulin-induced hypoglycemia in healthy individuals.

The purpose of this study was to determine whether the perception of hypoglycemia is reduced during acute stress. In Session I each of our 40 healthy male volunteers received a bolus injection of human insulin (0.05 IU/kg) resulting in plasma glucose nadirs of below 2.8 mmo/L. In Session 2 participants received insulin or saline, with half of each group being stressed by having to prepare and give a speech. Data collection at 5- to 25-min intervals included a symptom checklist, blood pressure, heart rate, and blood sampling for measurement of plasma glucose and counterregulatory hormones. Indi

Higher levels of social support predict greater survival following acute myocardial infarction: the Corpus Christi Heart Project.

Although low levels of social support have been related to mortality from coronary heart disease, little is known about the role of social support among Mexican Americans. The authors therefore examined the relationship between social support and long-term survival in the Corpus Christi Heart Project. They developed a social support scale that used data collected during in-hospital interviews of 292 Mexican Americans and 304 non-Hispanic Whites who survived a myocardial infarction for more than 28 days. The scale incorporated three measures: marital status; if not married, whether living alone

The psychobiology of hostility: possible endogenous opioid mechanisms.

This study examined the role of endogenous opioids in the relation between hostility and cardiovascular stress responsiveness. Forty-six men completed the Cook-Medley Hostility Scale and experienced a laboratory pain stressor once under opioid blockade and once under placebo. Hostility scores were significantly related to the magnitude of change in cardiovascular reactivity/recovery resulting from opioid blockade. Low scorers on the Cynicism subscale displayed increases in heart rate (HR) reactivity under blockade relative to placebo, with reactivity decreases noted in high scorers. Low Hostil

Differential hostility profiles accompany different hemodynamic response patterns.

Cardiovascular reactivity has been proposed as a mechanism by which psychosocial variables may lead lo coronary heart disease. Although reactivity as a generalized response has been linked lo psychological factors, there are specific hemodynamic profiles that underlie blood pressure reactivity. This study characterized subsets of young adults as myocardial or vascular reactors, in response to three laboratory tasks: mental arithmetic, videogame, and anger recall interview. Vascular reactors had higher diastolic blood pressure increases to all tasks, whereas reactor groups were equivalent at re

Cross-sectional and longitudinal analyses of coping responses and health status in persons with systemic lupus erythematosus.

The authors investigated the relations of coping with psychological adjustment and functional status in 46 adults with systemic lupus erythematosus (SLE). The participants completed questionnaires that measured coping with SLE, depression, and functional impairment. A subgroup (n = 22) completed the same questionnaires an average of 7.7 months later. Correlational and regression analyses revealed that, at Time 1, passive coping strategies (eg, avoidance, wishful thinking, blaming self) were significantly related to poorer psychological adjustment and functional status. Anger expression and lipid concentrations.

We used 2 different strategies to examine the relation between anger expression and lipid concentrations in 116 middle-aged men. Using the common analytic method used in the literature, the group crossing approach, we examined whether Anger-In. Anger-Out, and their interaction were related to lipids. Regression analyses revealed that Anger-In and Anger-Out were marginally related to total cholesterol. These associations disappeared after controlling for hostility, anger, and anxiety. Using a new intraindividual difference approach, we determined individuals' relative dominance of Anger-In and

Modulatory effects of defense and coping on stress-induced changes in endocrine and immune parameters.

We examined whether habitual defense and coping affect the response of hormones (ACTH. cortisol, prolactin. endorphins, and noradrenaline) and immune parameters (numbers of T cells. B cells. natural killer [NK] cells, and proliferative responses to mitogens or antigens) to an acute laboratory stressor (i.e., solving a 3-dimensional puzzle and explaining it to a confederate) in 86 male high school teachers. Defense and coping were assessed by Kragh's tachistoscopic Defense Mechanism Test (a measure of perceptual defense) and by 4 questionnaire-based coping styles assessing instrumental mastery-

Effect of trait hostility on cardiovascular responses to harassment in young men.

Hostile individuals may experience more extreme and frequent episodes of anger than nonhostile persons and thus may have exaggerated physiological responses to their environments. Such responses may be associated with increased risk of cardiovascular disease. This study examined cardiovascular responses of 24 low- versus 21 high-hostile young men to a mental arithmetic task administered with and without provocation in 2 x 2 (groups x Tasks) repeated measure design. hostility classifications were based on weighted interview ratings of Potential for Hostility. As predicted, high-hostile men show

Differentiation between major and milder acute mental stress by means of the leukocyte adhesiveness/aggregation test.

Following previous observations that the adhesive state of white blood cells in the peripheral blood increases during stress, we examined 645 volunteers in various conditions of anticipatory anxiety. The volunteer subjects included 465 controls in whom stress was related solely to impending venipuncture, 149 persons under moderate stress (students before delivering a graded lecture, patients before dental treatment, etc), as well as 31 individuals under major stress (eg, before induction of anesthesia in the operating room). The respective values of aggregated leukocytes in the peripheral bloo

Anger expression, hostility, anxiety, and patterns of cardiac reactivity to stress.

The majority of studies investigating the relationships between psychological characteristics and cardiovascular reactivity to stress use a research strategy in which discrete traits are evaluated in isolation. The present study examined the effects of additive and/or interactive relationships among traits on cardiac reactivity to a mental arithmetic task. In addition, impedance cardiographic techniques were employed to examine potential relationships between such psychological traits and a specific measure--pre-ejection period (PEP)--of sympathic influence on the heart. Predicting women's delay in seeking medical care after discovery of a lump in the breast: the role of personality and behavior patterns.

Experts agree that the detection of breast cancer in the early stages significantly enhances the chance of recovery. Although most women are aware of this, many still delay in seeking medical care. This study examined the relationship between four personality measures--health locus of control, hopelessness, repression-sensitization, and trait anxiety--and women's delay in approaching a doctor after discovering a lump in the breast. The authors also examined the relationship between delay and behavioral manifestations of body awareness representing the extent of the women's "contact" with their

Self-perception of the ability to work in the cold.

Accumulating evidence of beneficial effects from physical exertion must be balanced against increased risk of cardiac arrest during performance. There is evidence that, by using such cues as heart rate, individuals can monitor their level of exertion perceptually. We undertook experiments to discover whether temperature and heart rate interact to affect self-perception when the effective temperature is moved downard from the comfort zone. In the first pilot study, 36 males practiced a new game, SwedeBall, for a period of 20 minutes. Twelve were randomly assigned to play at a temperature of 22

The influence on blood pressure during daily activities of a single session of aerobic exercise.

Previous research has examined the effectiveness of a single session of aerobic exercise in lowering blood pressure responsivity to the subsequent presentation of a nonexercise stressor in the laboratory. This case study was designed to explore the possibility that a single session of aerobic exercise might lower blood pressure for up to 4 hours postexercise as the subject engaged in his usual activities at home and at work. The subject was an untrained, unmedicated, borderline hypertensive who underwent ambulatory blood pressure monitoring as he maintained a normal schedule of activities over
