[Study on bacteremia due to community-acquired infection in infants and children without underlying diseases].

Between July 1988 and June 1998, 22 pediatric patients without underlying diseases were diagnosed as bacteremia caused by community-acquired infection in Asahikawa Kosei Hospital. The age range of the patients was from 7 days to 4 years. Their diseases were meningitis in 6, urinary tract infection in 6, respiratory tract infection in 4, skin infection in 2, and unknown origin in 4. The causative organisms were Escherichia coli in 7, Haemophilus influenzae in 4, Streptococcus pneumoniae in 4, Staphylococcus aureus in 2, Streptococcus pyogenes in 1, Streptococcus agalactiae in 1, Listeria monocy

Hearing loss in diving--a study amongst Navy divers.

Despite the commonly observed condition of middle and inner ear barotrauma among divers, there is little evidence of insidious and permanent development of sensorineural hearing loss associated with diving. An audiometric survey was performed on a group of 120 divers and 166 non divers from the Royal Malaysian Naval Base in Lumut, West Malaysia between July to December 1991. The results of this survey revealed that insidious development of high frequency sensorineural hearing loss may be associated with diving. At frequencies 4000, 6000 and 8000 Hz the divers had higher mean hearing levels tha

Mitochondrial diabetes mellitus.

In the last few years, several mitochondrial DNA mutations and deletions have been described in association with various human diseases. Mitochondrial disorders, though long regarded strictly as neuromuscular diseases, may in fact involve non-neuromuscular symptoms. Diabetes mellitus has been reported in patients presenting with large mtDNA rearrangements (deletion, deletion-duplication) or in association with mtDNA point mutations, generally in tRNA genes (tRNA(Leu(UUR)). Genetic studies have shown that these disorders occur in sporadic cases or can be maternally inherited. The main clinical

Neuro-otologic findings in the lightning-injured patient.

Although lightning injuries are common, neuro-otologic sequelae are infrequently reported. The most common otologic injury encountered in the lightning strike victim is tympanic membrane rupture; the most common vestibular disturbance documented is transient vertigo. A variety of other clinical findings have been described in this population of patients. They include sensorineural hearing loss, conductive deafness, tinnitus, basilar skull fracture, avulsion of the mastoid bone, burns to the external auditory canal, and peripheral facial nerve palsy. The initial treatment of the lightning strik

The Juberg-Marsidi syndrome maps to the proximal long arm of the X chromosome (Xq12-q21).

Juberg-Marsidi syndrome (McKusick 309590) is a rare X-linked recessive condition characterized by severe mental retardation, growth failure, sensorineural deafness, and microgenitalism. Here we report on the genetic mapping of the Juberg-Marsidi gene to the proximal long arm of the X chromosome (Xq12-q21) by linkage to probe pRX214H1 at the DXS441 locus (Z = 3.24 at theta = .00). Multipoint linkage analysis placed the Juberg-Marsidi gene within the interval defined by the DXS159 and the DXYS1X loci in the Xq12-q21 region. These data provide evidence for the genetic distinction between Juberg-M

Treatment of an acoustic neuroma in an only-hearing ear: case reports and considerations for the future.

Current options regarding the treatment of acoustic neuroma in an only-hearing ear include: observation, attempted hearing preservation surgery, and stereotactically guided radiation therapy. A patient who had a left labyrinthectomy for Menière's disease presented 15 years later with a large right acoustic neuroma. Due to anticipated profound deafness, he fit the criteria for cochlear implantation. Promontory stimulation of the left ear was positive. He underwent successful left cochlear implantation with the Nucleus 22-channel device and was successfully rehabilitated. He then underwent trans

Effects of methylmercury on the brain of infant rats reared artificially.

Fetal Minamata Disease leads to cerebral palsy, blindness, and deafness accompanying microcephaly. Fetal Minamata Disease has been considered to be exposed to methylmercury during mid and late gestation periods. Brain development in the late gestation in human seems to be equivalent to that in the postnatal stage in rats and mice. Artificial rearing systems for infant rats exclude the maternal and nutrient factors. We administered methylmercury to the infant rats in the artificial rearing system attempting to produce an animal model for Fetal Minamata Disease. Brain weight and brain weight/bod

Mutations at the mouse microphthalmia locus are associated with defects in a gene encoding a novel basic-helix-loop-helix-zipper protein.

Mice with mutations at the microphthalmia (mi) locus have some or all of the following defects: loss of pigmentation, reduced eye size, failure of secondary bone resorption, reduced numbers of mast cells, and early onset of deafness. Using a transgenic insertional mutation at this locus, we have identified a gene whose expression is disrupted in transgenic animals. This gene encodes a novel member of the basic-helix-loop-helix-leucine zipper (bHLH-ZIP) protein family of transcription factors, is altered in mice carrying two independent mi alleles (mi and miws), and is expressed in the developi

Optic atrophy, sensorineural hearing loss and polyneuropathy--a case of sporadic Rosenberg-Chutorian syndrome.

We report on a 41-year-old woman with slowly progressive motor dominant polyneuropathy, optic atrophy and sensorineural deafness, but without any known familial feature. These neurological manifestations were in accordance with the syndrome reported by Rosenberg and Chutorian in 1967 (Rosenberg-Chutorian syndrome), a unique form of Charcot-Marie-Tooth disease. The syndrome was first reported to be autosomal dominant in trait, but subsequently recessive forms and sporadic ones have also been described. Nerve biopsy and neurophysiological studies including electromyography, nerve conduction velo

Neurological complications of sickle cell anaemia at KNH: a five year retrospective study.

In a five year retrospective study of 360 patients with homozygous (SS) sickle cell disease, eighteen (5%) were found to have neurological complications. Their ages ranged from 7 months to 21 years with a mean of 11.1 +/- 6 years. Of those with neurological complications, twelve (67%) of the patients had cerebrovascular accident, six (33.3%) convulsions, three visual disturbance; one sensorineural deafness, one cerebellar degeneration and the last one confusion and hallucinations. Four of the patients had multiple neurological complications. There was only one patient with recurrence of neurol

Out-of-home placement of children with severe handicaps: a comparison of approaches.

The relation of risk factors to out-of-home placement of children with severe and profound mental retardation was examined using both cross-sectional (n = 5,992) and 2-panel (n = 141) designs. Predictor variables included age, sex, ethnicity, adaptive behavior, maladaptive behavior, retardation level, blindness, deafness, and epilepsy. Multivariate analysis of variance indicated significant differences in the relative importance of predictor variables between sampling approaches. However, accuracy in predicting the children placed increased little in the large cross-sectional sample (model acc

Seropositivity, adenoid hypertrophy, and secretory otitis media in adults--a recognized clinical entity.

Otolaryngologic manifestations of AIDS have been described in the past. In this study, I had examined 14 adults with nasal obstruction and mouth breathing. Nine patients also reported deafness--unilateral in three of them and bilateral in six. All of them revealed a mass in the nasopharynx, either on the posterior rhinoscopy or the x-ray neck-lateral view. To exclude nasopharyngeal malignancy, all of the patients underwent examination of the nasopharynx while under general anaesthesia and biopsy. The histopathologic diagnosis in every patient was nonspecific, reactive lymphoid hyperplasia, whi

[A case of syphilitic meningitis presenting the seventh nerve and the eighth nerve palsies].

Here we described a case of syphilitic meningitis presenting the seventh nerve and the eighth nerve palsies. A 56-year-old man was admitted because of left facial palsy and hearing loss of bilateral ears. He had a penile chancre five months before admission. Cerebrospinal fluid examination showed high pressure, pleocytosis predominantly with mononuclear cells and high protein content. STS and TPHA of serum and CSF were positive. High dose penicillin G was effective, and he was discharged with only mild hearing loss in the left. Considering the penetration of penicillin G into the CSF and the m

Neurological complications of sarcoidosis.

The medical records of 285 patients at Royal Prince Alfred Hospital with a definite diagnosis of sarcoidosis over the period 1973-87 were reviewed. Fourteen (4.9%) had involvement of the nervous system, in 8 of whom the neurological manifestations were the presenting feature of the disease. The age range of neurosarcoidosis was 22 to 79 years and there was a female preponderance (10:4). Neurological complications (some patients experienced more than one complication) were facial nerve palsy (4), myopathy (3), aseptic meningitis (3), deafness/vertigo (2), hydrocephalus (2), hypopituitarism (2)

Infantile Refsum's disease: a peroxisomal storage disorder?

An 18-month-old infant presented with a history of arrest of neurological development from the age of eight months, with progressive ataxia, deafness, retinitis pigmentosa and hepatomegaly. Biochemical investigations revealed an elevated plasma phytanic acid level and deficiency of phytanic acid oxidase in skin fibroblasts. Histopathological findings in a liver biopsy were similar to those reported in infantile phytanic acid storage disease. Unexpected findings were the presence of elevated levels of plasma pipecolic acid, and elevated plasma long-chain fatty acid ratios, biochemical findings

Cockayne syndrome--an audiologic and temporal bone analysis.

Cockayne's syndrome is a triad of dwarfism, retinal atrophy, and deafness. Over thirty cases have been presented in the literature. We have examined and audiometrically tested three patients (ages 13 to 17) with confirmed Cockayne's syndrome and have analyzed the temporal bones of another who died at age 24. To our knowledge this is the first reported temporal bone analysis of a patient with Cockayne's syndrome. Audiograms revealed bilateral symmetric sensorineural hearing loss that was greatest in the high frequencies. Temporal bone examinations revealed inner and outer hair cell losses in th

A review of Haemophilus influenzae infections in Cambridge 1975-1981.

Invasive Haemophilus influenzae infections diagnosed in Cambridge between January 1975 and December 1981 are reviewed. Altogether, 81 sites in 77 patients were infected. Of these patients, 41 had meningitis, 20 epiglottitis, 8 bone or joint infections (2 with concurrent meningitis), 4 cellulitis, 5 pneumonia (2 with concurrent epiglottitis) and 3 septicaemia in the absence of documented localised infection. Most patients (88%) were children less than 5 years of age. Only 8 adults with such infections were identified. Of these, 6 had an identifiable predisposing condition. The incidence of meni

Histological considerations in implant patients.

The selection of patients for cochlear implantation requires consideration of a variety of anatomical and histological facts. Mechanical impediments to cochlear implantation, including the consequences of chronic otitis media, bony obliteration of the membranous labyrinth, or labyrinthitis ossificans, following bacterial labyrinthitis or vascular compromise of the inner ear, obliterative otosclerosis, and middle-ear dysplasias may present difficulties in access to the inner ear and placement of electrodes. Consideration of the histopathological features of known causes of profound deafness wil

The Wagner-Stickler syndrome complex.

The Wagner's vitreoretinal degeneration is a condition characterized by myopia, cataract, optically empty vitreous cavity and retinal breaks leading to retinal detachments with poor surgical prognosis. Several extraocular manifestations were reported to be associated with these eye findings and various syndromes were delineated describing sensorineural deafness, orofacial anomalies and skeletal dysplasias. There is enough evidence to suggest that this condition results from a phenotypic variability of genic origin, transmitted as an irregular autosomal dominant trait. In the absence of knowled

Hyperthyroidism, diabetes mellitus and the congenital rubella syndrome.

A male patient born to a mother who developed rubella during the tenth week of gestation presented a typical congenital rubella syndrome with mental retardation, neuro-sensory deafness, hypoplasia of the dental enamel and chorioretinitis. Hyperthyroidism occurred at the age of 3 10/12 years and was treated successfully with propylthiouracil for 4 years. The course was complicated by premature craniosynostosis and a craniectomy was performed at the age of 7 years. Overt diabetes mellitus developed at 17 years and was well controlled by insulin therapy. Histocompatilibity (HLA) antigens were A2,
