The 'shape&roll' prosthetic foot: II. Field testing in El Salvador.

A field test was performed in El Salvador to evaluate the usefulness of the Shape&Roll prosthetic foot, a foot developed for low-income countries, involving 12 participants. Quantitative gait parameters were measured with a Direct Ultrasound Ranging System (DURS). Qualitative information was obtained from questionnaires administered before and after a three-week trial. The results indicate that the Shape&Roll foot widened the speed range of all participants. According to the questionnaires, the Shape&Roll foot eases walking, enabling participants to walk significantly longer distances. The par

Sedation with ketamine: a safe and effective anaesthetic agent for children in the developing world.

Recently there has been a resurgence in the utilization of ketamine, a unique anaesthetic, for emergency procedures requiring sedation. The purpose of the present study was to examine the safety and efficacy of ketamine for sedation in the treatment of children's fractures in the small clinic setup of rural Nepal. One hundred and fourteen children (average age, 5.3 years; range, twelve months to ten years and ten months) who underwent closed reduction of an isolated fracture or dislocation in the Orthopaedic & Trauma Clinic at Janakpurdham were prospectively evaluated. Ketamine hydrochloride w

Thromboembolism.

Venous thomboembolism (VTE) causes only about 2% of maternal deaths in the developing world but is a leading cause of direct maternal deaths in developed countries. Pregnancy increases the risk of VTE through venous stasis, changes in blood coagulability and damage to vessels. Early diagnosis of VTE depends crucially on awareness of the condition but clinical diagnosis is unreliable in pregnancy and objective testing is essential. Compression or duplex ultrasonography is used to diagnose deep venous thrombosis and a ventilation/perfusion scan for pulmonary embolism. Low molecular weight hepari

Recommendations for indicators: night blindness during pregnancy--a simple tool to assess vitamin A deficiency in a population.

Night blindness during pregnancy caused by vitamin A deficiency is associated with an increased risk of morbidity and mortality among women. Because a history of maternal night blindness is simple and reliable to use, it is recommended as a population-based indicator of vitamin A deficiency. Furthermore, a maternal night blindness prevalence of >/=5% is recommended as a cut-off at which vitamin A deficiency may be considered to be a problem of public health significance within the community. This paper provides the justification for these recommendations. Night blindness during pregnancy is st

Micronutrients as adjunct therapy of acute illness in children: impact on the episode outcome and policy implications of current findings.

Role of micronutrients namely vitamin A, zinc and folate, as adjunct therapy of illness episodes in children in developing countries have been discussed in the light of health policy. Apart from a selective review, attempts have been made to statistically combine results of several studies to address policy issues. In children, vitamin A supplementation during illness has (a) a profound effect in reducing mortality in measles, (b) possibly a significant effect in reducing persistent diarrhea episodes in children with acute diarrhea, and (c) no benefit in pneumonia. Use of large dose vitamin A …

Meeting the contraceptive and AIDS prevention needs of people living on a dollar a day.

The new millennium sees the largest cohort of young people in history entering its fertile years. Many of these people are too poor to pay t … spread rapidly. New methods must take into account the limitations of the health infrastructure in developing countries and the imperative of low cost. Given the constraints of money, skills and facilities, it is essential to set realistic priorities for future contraceptive research and development. It is suggested that the greatest needs are for a woman-controlled …

[Congenital syphilis: update and perspectives].

Congenital syphilis is responsible for a variety of clinical symptoms, from subclinical attacks to septicemic forms that may be fatal. The m … the titer of IgG decreases. Reinfection is always possible, even in a newborn treated correctly. In developing countries, pediatricians must be aware of the various clinical forms of congenital syphilis. In addition, national programs to combat sexually transmitted diseases should be supported and developed by international aid agencies. In economically advanced count …

Bacterial and viral etiology of serious infections in very young Filipino infants.

OBJECTIVE: Pneumonia, meningitis and other serious infections are leading causes of death in developing countries. As part of a multicenter study we aimed to determine the etiology of pneumonia, meningitis and other serious infections in a cohort of Filipino infants ages 90 days or younger. METHOD: During a 2-year period, 2053 infants age 90 days or younger presenting to 1 of 3 Manila community hospitals were screened; 873 had signs or symptoms suggestive of an infectious illness, and 608 were judged to have clinical features suggestive of severe infection and had laboratory workup including …

Helicobacter pylori gastritis--epidemiology.

The acquisition of Helicobacter pylori is the main cause of chronic gastritis in humans. In Europe, a small proportion (less than 1%) of gastritis cases are caused by H. Heilmannii, and somewhat more (5%) are autoimmune in origin, in which condition H. pylori may not probably play a role. Recent findings on chronic gastritis and H. pylori acquisition in developed countries can be summarized as: (1) H. pylori gastritis is acquired in childhood and adolescence (age less than 20) in more than 50% of cases; (2) the risk and rate of acquisition is highest in early childhood, after which the rate ex

Intestinal geohelminthiasis in the developing world.

Intestinal parasitic helminths such as roundworms (Ascaris lumbricoides), hookworms (Ancylostoma doudenale and Necator americanus) and whipworm (Trichuris trichiura) are common in the developing world. Strongyloides stercoralis, though not so common, also contributes to health problems. The prevalence of intestinal worm infestation in India varies from 5% to 76%, which is similar to that in other developing countries. These parasitic infestations are acquired by ingestion, inhalation or penetration of the skin by the infective forms. Ascaris causes many intestinal and respiratory symptoms and …

Documented homicides and excess deaths: new insights into the scale of killing in the USSR during the 1930s.

"Getty, Rittersporn and Zemskov recently claimed that no more than 2 million people could have perished from collectivization, famine, execution, terror, and forced labor in the USSR during the 1930s. Prior demographic confirmation of this estimate was provided by Anderson and Silver who contended that killings were unlikely to exceed a few million and could not be more than 4.8 million victims. This essay disproves both these contentions by introducing new demographic evidence proving that Stalin killed at least 5.2 million Soviet citizens 1927-1938, with a best estimate in the vicinity of 10

Prevalence of anaemia in pregnant women during the last trimester.

Anaemia is a very common condition during pregnancy. This is particularly so in developing countries where the level of intake of iron rich foods is low; malaria and other intestinal parasites are common. This study was conducted to determine the prevalence of anaemia and the type of anaemia existing in pregnant women in Morogoro municipality. The effect of anaemia on infant birth weight was also examined. Twenty randomly selected pregnant women in their last trimester of pregnancy were studied. The subjects were recruited from the three maternal and child health clinics in the municipality. T…
