Transient facial palsy in two cases of benign, very rare middle ear tumors (carcinoid tumor and myxoma).

OBJECTIVE: Presentation of the clinical features of 2 very rare middle ear tumors in which the guiding symptom was facial palsy. MATERIAL AND METHODS: Illustrative case reports about a myxoma and a carcinoid tumor of the middle ear associated with peripheral facial palsy. RESULTS: The facial palsy was transient in either case, and its pathomechanism is open for discussion. In both cases, the initial symptoms were typical for an inflammatory process. Moreover, both tumor entities are typically found in organs other than the ear; if located in the middle ear, those neoplasms grow rather superf…

Consultations for middle ear disease, antibiotic prescribing and risk factors for reattendance: a case-linked cohort study.

BACKGROUND: Otitis media is the most common reason for children to receive antibiotics, but there is no evidence about the effect of prescribing on reattendance. AIM: To evaluate the changing workload of middle ear disease in general practice, and the impact on surgery reattendance of prescribing antibiotics at first attendance. DESIGN OF STUDY: A case-linked cohort analysis for antibiotic prescribing versus no prescribing at first consultation event. SETTING: Two hundred and ninety-one practices spread throughout the UK recording for the General Practice Research Database (GPRD) and incorp…

Recurrent autoimmune inner ear disease (AIED) and polyarteritis nodosa in a patient with large cell lung carcinoma.

Autoimmune inner ear disease (AIED) is a very rare disorder with distinct clinical features and can occur in patients with malignancy or autoimmune diseases. We report a 72-year-old male patient with polyarteritis nodosa treated continuously for 5 years with aggressive immunosuppressive drugs, including cyclophosphamide, who experienced three episodes of acute hearing loss during treatment. Organic lesions of the external and middle ear were excluded by repeated examinations, and if one subscribes to McCabe's (Ann Otol Rhinol Laryngol 88:585-589, 1979) definition of AIED, this condition must b…

Transforming growth factor-beta and its effect on reepithelialization of partial-thickness ear wounds in transgenic mice.

Transforming growth factor-beta (TGF-beta) is known to affect nearly every aspect of wound repair. Many of the effects have been extensively investigated; however, the primary effect of endogenously derived TGF-beta on wound reepithelialization is still not completely understood. To examine this, two types of wounds were made on a transgenic mouse over-expressing TGF-beta1. Full-thickness back wounds were made to compare the wound healing process in the presence of compensatory healing mechanisms. Superficial partial-thickness ear wounds involving only the epidermis were made to determine the …

Ear diseases and hearing in the Thai elderly population. part II. A one year follow-up study.

The ear and hearing survey of the Thai elderly in 14 urban communities around Siriraj Hospital was repeated one year after the first survey in order to detect any changes. Altogether 556 elderly people came for follow-up examination, 191 were males, 365 were females, the average age was 68.2 years (60-88 years). Ear disease was diagnosed by ENT specialists in 80 cases which implied that the prevalence of ear disease was 14.4 per cent. This was not statistically significantly different from the prevalence of ear disease in the survey conducted in the previous year and although the elderly who h…

Barttin is a Cl- channel beta-subunit crucial for renal Cl- reabsorption and inner ear K+ secretion.

Renal salt loss in Bartter's syndrome is caused by impaired transepithelial transport in the loop of Henle. Sodium chloride is taken up apic … calizes in basolateral membranes of renal tubules and of potassium-secreting epithelia of the inner ear. Disease-causing mutations in either ClC-Kb or barttin compromise currents through heteromeric channels. Currents can be stimulated further by mutating a proline-tyrosine (PY) motif on barttin. This work describes the first known beta-subunit for CL …

Control of symptoms in patients with Meniere's disease using middle ear pressure applications: two years follow-up.

The effects of middle ear pressure pulses were studied in 37 patients with a diagnosis of definite Meniere's disease and active vestibular symptoms, 31 of whom had failed to respond to medical treatment including diuretics prior to pressure treatment. The number of vertigo spells during the 6 months prior to treatment and for 6, 18 and 24 months after the start of treatment were evaluated. The functionality level scores were three or worse according to the American Academy of Otolaryngology--Head and Neck Surgery (AAO-HNS) criteria. The results of the study were as follows: 19 patients experie…

[Vestibular function after a reconstructive surgery improving hearing in patients after radical operation on the middle ear].

The study included 62 patients who had undergone radical surgery (RS) on the middle ear. The patients were later operated to improve hearing according to the original reconstructive technique. Before the reconstructive surgery and 1-2, 3-6, 12 and 36 months after it combined vestibulometric examination, otomicroscopy, audiometry assessed the treatment efficiency. Perception of live speech was also investigated. Vestibular dysfunction occurred in 87.1% of the examinees with the history of RS. One of the main causes of the dysfunction was micropathological foci in the trepanation cavity located …

Acute renal failure in a healthy young adult after dextran 40 infusion for external-ear reattachment surgery.

Dextran 40 is used to improve the microcirculation after certain surgical procedures. We report a rare case of acute anuric renal failure in a healthy young adult after administration of dextran 40 to improve the microcirculation following ear-reattachment surgery. The renal failure was subsequently reversed by plasmapheresis and intensive care support. Although rare in the young and fit, the risk of developing acute anuric renal failure exists with administration of dextran 40 and appropriate monitoring is essential.

The etiology of idiopathic sudden sensorineural hearing loss. Experimental herpes simplex virus infection of the inner ear.

HYPOTHESIS: Experimentally induced herpes simplex virus type 1 (HSV-1) labyrinthitis provides a suitable model for idiopathic sudden sensorineural hearing loss (ISSHL). BACKGROUND: Viral labyrinthitis has been postulated to play a role in the pathophysiology of ISSHL. Circumstantial evidence is pointing at members of the herpes virus family. Experimental viral labyrinthitis elicited by various virus families leaves a virus-specific pattern of cochlear damage. Herpes viruses provide the best matching pattern in the distribution of cochlear damage when compared with ISSHL postmortem cochlear hi…

Human autoantibodies and monoclonal antibody KHRI-3 bind to a phylogenetically conserved inner-ear-supporting cell antigen.

Autoimmunity is thought to be one cause of sensorineural hearing loss (SNHL). Sera from patients with rapidly progressive hearing loss have been shown to contain antibodies to a 68-kD protein in heterologous inner-ear tissue. Using guinea pig inner-ear tissue as the antigenic substrate and either Western blot or immunofluorescence (IF) or both, we tested sera from 74 patients suspected to have autoimmune hearing loss for inner-ear antibodies. Sera from 73 patients were tested by Western blot, and sera from 36 were tested by IF. Thirty-seven of 73 (51%) had antibody to a 68-70-kD protein by Wes…

Alterations of bone conducted hearing in cases of modified middle ear mechanics. Conclusions from an electrical model.

In clinical diagnosis bone conduction thresholds can be used to assess impaired hearing caused by pathological function of the inner ear. The effects of changed mechanical properties of the middle ear on bone conduction are usually not considered in patients who simultaneously suffer from middle ear and inner ear diseases. This procedure is only partially correct. An exact determination of the effects of altered middle ear mechanics on bone conduction in patients with otosclerosis or after middle ear operations is rather difficult, but such determinations can improve diagnostic validity. There…

Presence of cytomegalovirus and herpes simplex virus in middle ear fluids from children with acute otitis media.

Twenty-seven (10%) of 271 infants and children with acute otitis media (AOM) were found to be infected with cytomegalovirus (CMV) or herpes simplex virus type 1 (HSV). CMV or HSV, alone or in combination with bacteria or other viruses, was isolated from the middle ear fluid (MEF) of 10 patients. In three cases, CMV alone was isolated from the MEF, and in one case, HSV alone was isolated. One of the CMV cases involved an acute primary or reactivation of CMV infection, with CMV-bacterial otitis and conjunctivitis as major manifestations. One patient with AOM and stomatitis had purulent otitis as…

Prenatal dinocap exposure alters swimming behavior in mice due to complete otolith agenesis in the inner ear.

Exposure to the fungicide dinocap during gestation produces behavioral abnormalities in the house mouse that are not apparent at birth but become obvious at weaning. Pregnant mice (CD-1) were exposed on Days 7 to 16 of gestation to dinocap at 0, 6, 12, or 25 mg/kg/day and the postnatal behavioral development of the offspring was assessed. Torticollis (head-tilting) appears in the treated offspring at 3 weeks of age (4.4% at 12 and 25.3% at 25 mg/kg/day) and, during a test of swimming ability, many of the mice (6.8% at 12 and 47.2% at 25 mg/kg/day) sink below the surface or are unstable and swi…
