[Peripheral vertigo classification. Consensus document. Otoneurology committee of the Spanish otorhinolaryngology society (2003-2006)].

There are many different vertigo classifications and different denominations are frequently used for the same clinical processes. The Otoneurology Committee of the Spanish Society for Otorhinolaryngology and Head and Neck Pathology proposes an eminently practical classification of peripheral vertigo to facilitate a common terminology that can be easily used by the general ENT practitioners. The methodology used has been by consensus within our Society and especially among the most outstanding work groups in the area of otoneurology in Spain. Initially vertigo is divided into single-episode ver

Augmented chemosensitivity in black-eyed white Mitfmi-bw mice, lacking melanocytes.

Microphthalmia-associated transcription factor (Mitf) is responsible for differentiation of melanocytes, and a recessive Mitf mutant, black-eyed white (bw) mouse, is characterized by the lack of melanocytes in the skin and inner ear. To search for the hitherto unknown roles of melanocytes, we analysed the ventilatory responses of unanaesthetized bw mice by whole body plethysmography. During air breathing, bw mice showed lower breathing frequency and larger tidal volume, compared with age-matched wild-type mice, although there was no difference in the minute ventilation. Importantly, bw mice pr

Spiral ligament fibrocytes release chemokines in response to otitis media pathogens.

CONCLUSION: Spiral ligament fibrocytes (SLFs) may be involved in the innate immune response of the inner ear by producing chemoattractants for recruiting inflammatory cells such as neutrophils and monocytes. OBJECTIVE: The purpose of this study was to investigate the cellular responses of SLFs when challenged by inflammatory stimuli such as components of otitis media pathogens or proinflammatory cytokines. MATERIALS AND METHODS: To detect released inflammatory cytokines and chemokines, cells were treated for 48 h with whole lysates of nontypable Haemophilus influenzae (NTHi), Streptococcus p

[Evidence of a novel gene for the LAV-syndrome].

BACKGROUND: Both LAV- (large or enlarged vestibular aqueduct) and Pendred-syndrome are autosomal recessive diseases. In contrast to Pendred-syndrome, LAV-syndrome is characterised only by an enlarged vestibular aqueduct. Pendred-syndrome is a more complex disease. Classically it is characterised by sensorineural hearing loss and enlargement of the thyroid gland. Up to now, only mutations in SLC26A4 gene are known as being responsible for both syndromes. The gene for Pendred-syndrome (SLC26A4) has been localised by linkage analysis of chromosome 7q31. This protein is expressed in the inner ear,

von Hippel-Lindau disease.

In recent years advances have been made in the clinical and genetic aspects of von Hippel-Lindau disease (VHL). Retinal capillary hemangioma is the most common manifestation of VHL disease and, therefore, ophthalmologists are frequently involved in the care of patients with this disease. The incidence of VHL disease is approximately 1 in 40,000 live births. It is estimated that there are approximately 7000 patients with VHL disease in the USA. The inheritance of VHL disease is autosomal dominant with high penetrance. Depending on the clinical circumstances, retinal capillary hemangioma may be

Defective calmodulin-dependent rapid apical endocytosis in zebrafish sensory hair cell mutants.

Vertebrate mechanosensory hair cells contain a narrow "pericuticular" zone which is densely populated with small vesicles between the cuticular plate and cellular junctions near the apical surface. The presence of many cytoplasmic vesicles suggests that the apical surface of hair cells has a high turnover rate. The significance of intense membrane trafficking at the apical surface is not known. Using a marker of endocytosis, the styryl dye FM1-43, this report shows that rapid apical endocytosis in zebrafish lateral line sensory hair cells is calcium and calmodulin dependent and is partially bl

Neurobiology of fish under altered gravity conditions.

In vertebrates (including man), an altered gravitational environment such as weightlessness can induce malfunction of the inner ear, based on an irregular dislocation of the otoliths from the corresponding sensory epithelia. This dislocation leads to an illusionary tilt, since the otolithic inputs are not in register with other sensory organs. This results in an intersensory conflict. Vertebrates in orbit therefore face severe orientation problems. In humans, the intersensory conflict may additionally lead to a malaise, commonly referred to as space motion sickness (SMS). During the first days

[Furosemide test for diagnosis of endolymphatic hydrops: a clinical exploration].

Furosemide test (FT) is a clinical method for the diagnosis of endolymphatic hydrops (EH). Immediately preceding i.v. injection of 20 mg furosemide, a caloric test was performed with 50 micromilligrams of water at 30 degrees C. Nystagmography was employed to measure the maximum slow velocity of caloric nystagmus. The test was repeated one hour after the injection. The two sets of data were then compared. An increase beyond 22.97% was defined as positive. The first group was composed of 54 patients of EH as the experimental group, and the second group 54 patients with vertigo of non-EH as contr

Autoregulation of cochlear blood flow in guinea pigs.

Autoregulation of blood flow in the inner ear following uncontrolled changes in systemic blood pressure (BP), which was induced by the application of pharmacological agents that cause local and/or systemic vascular effects, has been reported in previous studies. In the current study, carotid BP was systematically manipulated without drugs, while the resulting cochlear blood flow (CBF) changes were measured using a laser Doppler flowmeter (LDF). Anesthesized guinea pigs were used, and the probe of a LDF was held against the ventral-posterior portion of the surgically exposed cochlea. A mechanic

Cell division in the gerbil cochlea after acoustic trauma.

The recent discovery of hair cell regeneration in the avian inner ear raises the possibility that hair cell regeneration might occur in the mammalian cochlea as well. The authors used 3H-thymidine labeling to detect mitotic activity in the cochleas of normal 3-week old gerbils exposed to acoustic trauma. Following an acoustic insult that caused progressively more severe damage in an apical to basal progression, 3H-thymidine was injected for 5 days. Control animals were not exposed to the acoustic insult. The gerbils' cochleas were sectioned and processed for autoradiography. In the control coc

Ototoxicity of subcutaneously administered aztreonam in neonatal rats.

Aztreonam (SQ 26,776) was given subcutaneously to three groups of neonatal rats at daily doses of 150, 600, and 2400 mg/kg from postnatal Days 10 through 16. Two similar groups given 400 mg of streptomycin kg-1 day-1 or 12 ml of 0.9% sodium chloride kg-1 day-1 on the same schedule served as a positive and negative control group, respectively. On postnatal Days 28 and 56, the neonates were evaluated for level of spontaneous activity and auditory and vestibular function. Half of the neonates in each group were necropsied on postnatal Day 29, and the other half on postnatal Day 57. The inner ears

[Hearing in advanced age: critical view of so-called presbycusis].

Presbycusis cannot be defined nosologically as an uniform disease. The computed threshold curves of the age dependent high-frequency hearing loss should not conceal the fact that the degree and form of the hearing loss vary greatly within the age groups. The pathological appearances also vary; degenerative changes vary in site and extent and are not limited to the cochlear structures, but are also found in all parts of the auditory system. The etiology of presbycusis can scarcely be attributed to the physiological senile degeneration of cochlear or central nervous structures alone because epid

Meniere's disease: peripheral or central origin. A neuroanatomical study.

Previous clinical and histological studies have suggested that the aetiology of Meniere's disease should be sought not only in the endolymphatic hydrops, but also in pathological changes of the brain and brain stem. Moreover, neuropsychological studies have indicated some centralization of the disease, whether it be primary or secondary. In order to elucidate this problem further we have examined the temporal bones and brains of 4 patients with Meniere's disease, who died of diseases unrelated to the inner ear or the central auditory and vestibular pathways. Endolymphatic hydrops was found in

Some current concepts of cochlear mechanics.

The development of the current concepts is reviewed in historical perspective. Helmholtz's hypothesis of basilar-membrane resonance was partially confirmed and partially defeated by Békésy's experiments on models and postmortem cochlear preparations. He discovered that sound was propagated in the cochlea in the form of traveling waves which reached a flat maximum at a frequency-dependent location. Mathematical theory explained this type of sound propagation as a special case of surface waves. Johnstone and his coworkers discovered that the maximum of cochlear vibration in living animals was mu

[Uncommon case of paraganglioma syndrome in combination with malignant paraganglioma].

BACKGROUND: Combination of paraganglioma syndrome and malignant paraganglioma is an uncommon disease worldwide. PATIENT: We report the case of a 69-year old man with a jugular-tympanal paraganglioma, who underwent surgery. Histopathological examination revealed a malignant paraganglioma. An other contralateral carotid-body and a tumor in the thyroid gland had been discovered during staging. The molecular results confirmed the existence of a paraganglioma syndrome. DISCUSSION: A malignant paraganglioma based on a hereditary paraganglioma syndrome is a rare described case in literature. On the

Description of the novel variant c.784delG;p. (Ala262Profs*68) at BSND gene and its association with Bartter Syndrome Type Iva.

Bartter syndrome (BS) is a group of diseases caused by variants in genes related to salt reabsorption in the thick ascending limb of the loop of Henle. It causes dysregulation in salt homeostasis and is characterized by hyperplasia and hypertrophy of the juxtaglomerular apparatus, hyperaldosteronism, hypokalemic alkalosis and impaired growth and development. BS type IVa is caused by variants in BSND, which encodes Barttin, a subunit for chloride channels (CLC). This specific subtype also causes sensorineural deafness due to its impact on CLC-kidney a (Ka), a channel important for the productio

The functioning of otolith organs in individuals with type-II diabetes mellitus.

AIM: Diabetes mellitus (DM), a systemic disease, is a major public health concern. It is possible that micro and macrovascular changes in the inner ear due to DM could affect both auditory and vestibular functioning. However, there is a lack of conclusive evidence for vestibular impairment in individuals with DM. The current study investigated the effects of Type 2 diabetes mellitus (T2DM) on the functioning of the vestibular system and its duration dependency. METHODS: Eighty individuals in the age range of 40-70 years (40 with T2DM and 40 age-matched healthy controls) underwent cervical ves

Continuous automated analysis of facial dynamics of brachycephalic and normocephalic dogs in different contexts.

This study develops a novel automated method for measuring continuous dynamics of dog facial behavior based on video-based tracking of 46 facial landmarks grounded in the Dog Facial Action Coding System. This method is applied for comparing the facial behavior of ( n 1 = 7 ) brachycephalic (Boston Terrier) and ( n 2 = 7 ) normocephalic (Jack Russel Terrier) dogs in four different contexts, eliciting various inner states: positive (play and called by name) and negative (separation and stranger). Having objectively quantified facial dynamics in brachycephalic and normocephalic dogs, we have

Atypical presentation of granulomatosis with polyangiitis: a case report and review of the literature.

BACKGROUND: Granulomatosis with polyangiitis (GPA) is a systemic autoimmune disease characterized by necrotizing granulomatous vasculitis of the small- and medium-sized vessels. Classical GPA affects the upper and lower airways and kidneys. It commonly affects the nose and paranasal sinuses, middle and inner ear, and subglottic region of the larynx. Therefore, otolaryngological involvement is common in GPA and can sometimes be the initial presentation. In rare cases, otologic signs and symptoms can be the initial manifestations of this disease, including recurrent otitis media, otitis media wi

Neonatal Onset Distal Renal Tubular Acidosis: Description of Two Novel Variants on the ATP6V0A4 Gene and Review of the Literature on Associated Extrarenal Manifestations.

Distal renal tubular acidosis (dRTA) is an extremely rare disease that affects the distal tubule's ability to excrete proton cations, acidify urine, and maintain the acid-base balance. The clinical presentation of dRTA typically includes normal anion gap metabolic acidosis with decreased serum bicarbonate levels, hypokalemia, hypercalcemia, nephrocalcinosis, and alkaline urine. Hereditary causes of dRTA include pathogenic variants in ATP6V1B1 , ATP6V0A4 , SLC4A1 , FOXI1 , and WDR72 genes, which encode different transmembrane proteins on the apical surface of type A intercalated cells in the di
