Symptomatic or cryptogenic partial epilepsy of childhood onset: fourteen-year follow-up.

This study reports on the seizure and psychosocial outcome of 29 patients with electroclinically well-defined childhood-onset symptomatogenic or cryptogenic partial epilepsy with complex partial seizures who were followed prospectively over 14 years. Many were refractory at the time of enrollment. At 14-year follow-up, we acquired information on seizure type and frequency, psychiatric history, substance abuse, criminal activity, in addition to educational, vocational, and marital status through chart reviews and/or structured telephone interviews. Sixteen patients were only treated medically.

Impact of bipolar affective disorder on family and partners.

BACKGROUND: Successful management of major mental illness in the community relies significantly on an informal or non-professional network of caregivers. The needs and experiences of such caregivers have been little studied with respect to major chronic mood disorders. RESULTS: Caregivers reported significant difficulties in their relationships with the patient when s/he was unwell, with considerable impact on their own employment, finances, legal matters, co…

Reemployment of patients with surgical salvage of open, high-energy tibial fractures: an outcome study.

Between January 1, 1988, and December 31, 1990, 36 patients with 37 type III high-energy open tibial shaft fractures were treated at Lehigh Valley Hospital. Patients with primary amputations were excluded. All patients with high-energy open tibial fractures with an intact posterior tibial nerve, protective sensations of the plantar surface of the foot, and warm ischemia time of less than 6 hours were considered salvageable. A retrospective review of the charts was completed. Twenty-eight patients with 29 fractures were interviewed for work status, an average of 39 months after treatment. Twent

Severe sciatica: a 13-year follow-up of 342 patients.

This study involved 342 patients hospitalized because of severe, persistent sciatica suggestive of a lumbar intervertebral disc herniation. After standard clinical evaluation, EMG and myelography, 220 patients underwent lumbar discectomy. The remaining 122 patients were treated conservatively. Follow-up examinations were arranged after 1, 5 and 13 years. The study focused on the rehabilitation outcome in general and differences in outcome between the two treatment groups. Several indicators showed a rather poor outcome for sciatica patients during the 13-year follow-up period. In the operated

Clinical and demographic factors associated with high levels of depression symptoms.

Beck Depression Inventory (BDI) scores were analyzed from 427 consecutive, nonpregnant, adult family medicine patients to delimit demographic and clinical parameters which would predict high levels of depression symptoms. BDI scores at or above the chosen cutoff score of 17 were present in 51 patients (12%). Unemployment, less than high school education, high usage of inpatient and outpatient services, complaints of nervousness or of a functional nature, and chronic illness were shown by univariate and multiple regression analyses to be associated with BDI scores in the depressed range. Nearly…
