Rethinking the 10% strain rule in fracture healing: A distal femur fracture case series.

Since the 1970s, the 2%-10% rule has been used to describe the range of interfragmentary gap closure strains that are conducive for secondary bone healing. Interpreting the available evidence for the association between strain and bone healing remains challenging because interfragmentary strain is impossible to directly measure in vivo. The question of how much strain occurs within and around the fracture gap is also difficult to resolve using bench tests with osteotomy models because these do not reflect the complexity of injury patterns seen in the clinic. To account for these challenges, we

Clinical Efficacy of Bone Reconstruction Surgery with Frozen Cortical Bone Allografts for Nonunion of Radial and Ulnar Fractures in Toy Breed Dogs.

OBJECTIVE: To evaluate the effectiveness of frozen cortical bone allografts (FCBA) in the treatment of severe radial and ulnar atrophic nonunion fractures. ANIMALS: Toy breed dogs with nonunion of radial and ulnar fractures (n = 15). METHODS: Severe atrophic nonunion fractures were treated with FCBA (eight infected and seven non-infected fractures). Radiographs obtained immediately after surgery, and 1, 2, 3, 6 and 12 months later were evaluated and scored for the periosteal reaction at the bone regeneration sites, the healing process in the bone connection areas at both the proximal and dis

Role of valgus osteotomy and fixation by double-angle blade plate in neglected displaced intracapsular fracture of neck of femur in younger patients.

Head preservation is the mainstay of management in younger patients with intracapsular fracture neck of femur. In this prospective study, we have tried to evaluate the role of valgus osteotomy and internal fixation by double-angle blade plate in patients below 60 years of age with displaced intracapsular fractures and whose time since injury is equal to or more than 3 weeks. Among the 20 fractures which are managed by this procedure, 18 fractures united. We recommend this procedure for the treatment of neglected displaced intracapsular fractures in young patients.

Total fractures of the tibial pilon.

Complete fractures of the tibial pilon are rare and their treatment difficult. The pathophysiology includes three groups: (A) high-energy trauma (motor vehicle injuries), with severe articular and soft tissue lesions, (B) rotation trauma, (skiing accidents), with modest articular and soft tissue damage, and (C) low-energy trauma in elderly people. These three groups occasion very different problems and complications. In emergency situations, these fractures should be stabilized, most often using external fixation to restore length and prepare definitive fixation. The second stage can be applie

Use of a pectus bar for surgical stabilization of cardiopulmonary resuscitation induced flail chest.

A flail chest can occur when cardiopulmonary resuscitation causes extensive rib fractures. Despite successful cardiopulmonary resuscitation, if the flail chest is not treated, the patient may not survive regardless of the correction of the primary condition that caused the cardiac arrest. Therefore, if flail chest persists despite the appropriate conservative management to correct the flail chest, active surgical management is essential. We present a successful surgical treatment with a pectus bar for a patient with flail chest, caused by extensive segmental rib fractures sustained during card

Subacute posttraumatic ascending myelopathy in a 15-year-old boy.

Secondary injury following initial spinal cord trauma is uncommon and frequently attributed to mismanagement of an unprotected cord in the acute time period after injury. Subacute posttraumatic ascending myelopathy (SPAM) is a rare occurrence in the days to weeks following an initial spinal cord injury that is unrelated to manipulation of an unprotected cord and involves 4 or more vertebral levels above the original injury. The authors present a case of SPAM occurring in a 15-year-old boy who sustained a T3-4 fracture-dislocation resulting in a complete spinal cord injury, and they highlight t

Minimal Invasive Fixation Can Decrease Infection Rates in Diabetic and Obese Patients With Severe Ankle Fracture and Syndesmotic Injury.

BACKGROUND: Ankle fractures involving syndesmosis disruption cause severely unstable joint conditions. Traditional invasive operations put certain patient groups at an increased risk of infection. There is limited literature discussing the outcomes of minimally invasive fixation of severe ankle fractures including syndesmotic injury, as clinicians may be tempted to treat these difficult cases with open reduction internal fixation (ORIF). METHODS: A retrospective case-control study was conducted on patients treated at a level one trauma center. Patients were divided into 2 groups based on pres

The proximal fibula should be examined in all patients with ankle injury: a case series of missed maisonneuve fractures.

BACKGROUND: The Maisonneuve fracture is a spiral fracture of the proximal third of the fibula. It occurs from violent twisting of the ankle that characteristically causes ligament damage and severe instability. Most patients complain of significant ankle pain but very little pain over the fracture. The clinical and radiographic examination is usually directed to the ankle region; and the proximal fibula is often ignored. OBJECTIVE: The authors intend to show the ease of missing the proximal fibular fracture when the clinical examination is directed to the ankle region. They discuss the import

Percutaneous nailing in the management of osteogenesis imperfecta.

Marked deformities of the skeleton may develop in the severe form of osteogenesis imperfecta. Internal fixation can be effective in the treatment and prevention of fractures and deformities. Eight years ago, axial correction of deformities of the femora and tibiae by manual osteoclasis was carried out in a 7 year old girl in whom osteogenesis imperfecta had produced severe bowing of the long bones of the legs. Internal metal splints were introduced percutaneously to lie partially in, and partially alongside, the bone, to obtain fixation in the correct position. This treatment resulted in the p

Girl with Multiple Fractures: A Unique Case of Polytrauma.

INTRODUCTION: Polytrauma is defined as multiple severe injuries with an abbreviated injury scale ≥3 in two or more body regions, often resulting from high-energy trauma. Managing such cases, especially in adolescents with complex fractures, requires a well-coordinated, physiologically guided approach. This case report discusses the management of an 18-year-old female with multiple fractures, including the lower limb, upper limb, and pelvis, following a high-energy fall. Early appropriate care (EAC) was essential in guiding treatment decisions. CASE REPORT: An 18-year-old female fell from a he

Incidence and outcomes of radial nerve palsy in distal third diaphyseal humerus fractures after surgical fixation.

BACKGROUND: Radial nerve palsy (RNP) is a common complication of diaphyseal humerus fractures, with primary RNP rates varying by fracture location. However, the relationship between fracture location and the risk of secondary RNP following surgical fixation remains unclear. This study aimed to evaluate the incidence of postoperative RNP based on fracture location within the middle and distal thirds of the humeral diaphysis and to describe the outcomes of RNP over time. METHODS: A retrospective cohort study was conducted of patients undergoing open reduction and internal fixation for middle an
