Clinical practice patterns in the pre-endoscopic management of individuals presenting with acute upper gastrointestinal bleeding: results of an international survey.

BACKGROUND: Despite recent guideline recommendations on managing acute upper gastrointestinal bleeding (AUGIB), mortality rates have not substantially improved over the years. OBJECTIVES: We aimed to understand real-life pre-endoscopic assessment and management practices in AUGIB. DESIGN: We conducted an international online survey from April to November 2023. METHODS: Key domains captured respondents' demographics and clinical practice location, definitions of hemodynamic instability, and pre-endoscopic assessment. We provided descriptive proportions of responses. Secondary analyses explor

Advances from Traditional Clinical Decision Support Tools to Machine Learning Applications in Primary Prevention of Thromboembolic Disorders.

Primary thromboprophylaxis for arterial and venous thromboembolism (VTE) still presents many challenges, as patient- and disease-specific risk factors complicate risk assessment and necessitate tailored strategies. To tackle this, multiple thromboembolism risk assessment models have been developed and validated, with machine learning (ML) models now emerging. Some ML models have been integrated into electronic health record (EHR)-based clinical decision support systems (CDSS) to improve guideline adherence and optimize appropriate anticoagulant use. We conducted a narrative review exploring th

Estimating investment costs to improve hypertension care in Lagos, Nigeria.

BACKGROUND: Low- and middle-income countries (LMICs) house 66% of all hypertension patients, many of whom are undertreated, leading to severe health risks and higher healthcare costs. This study examined the required investments to improve hypertension control in Lagos, Nigeria, using real-world medical records and cost data. METHODS: We analyzed routine medical records from 1249 hypertension patients across 74 public and private facilities in Lagos State and combined these with facility-level cost data to estimate the investment required to deliver guideline-based care. Patient-level gaps we

Reported use of anti-asthmatic medication in doping control forms from 2015 to 2019 - mapping retrospective data.

OBJECTIVES: Asthma and bronchial hyperresponsiveness is common in athletes, particularly in sports with high ventilatory demands or exposure to irritants such as cold, dry air, polluted air, or trichloramines. The mechanisms behind asthma in athletes remain unclear, but repeated high-volume ventilation and exposures to irritants, as well as airway inflammation and remodelling likely contribute. Symptoms resemble those of asthma, and airway hyperresponsiveness plays a key role, even in athletes with no other classic asthma features. Treatment of asthma in athletes follows the same approach as a

BCR::ABL1 Mutational Profiling in US Patients With Chronic-phase Chronic Myeloid Leukemia on Tyrosine Kinase Inhibitors.

BACKGROUND/AIM: Widespread use of BCR::ABL1 tyrosine kinase inhibitors (TKIs) has transformed the clinical landscape of chronic myeloid leukemia (CML). However, BCR::ABL1 mutation-driven treatment resistance challenges optimal care and outcomes. The purpose of this study was to ascertain real-world practices concerning such mutational profiling in US hematology and oncology practices, which were mainly community-based. MATERIALS AND METHODS: This is the first chart review of BCR::ABL1 kinase domain mutational profiling in the US. In this study of the Cardinal Health Oncology Provider Extended

Simultaneous Prenatal cfDNA Screening of Aneuploidy, Recessive Single-Gene Conditions, and Fetomaternal Blood Compatibility.

BACKGROUND: The provision of prenatal genetic screening in 3 core clinical contexts-recessive conditions, aneuploidy, and serological incompatibility-involves multiple tests across reproductive partners. This complexity limits utilization and impairs adherence to guideline-recommended care, particularly in carrier screening, where male partners are frequently not tested when a female carrier is identified. Here, we describe the analytical validation of a fetomaternal integrated recessive, serological, and trisomy genetic screen (FIRSTGENE), a single assay that harnesses in silico fragment-leng

Care Gaps Following Newly Detected Dipstick Proteinuria in Treated Hypertension: A Japanese Nationwide Database Study.

AIMS: Clinical guidelines recommend follow-up quantitative testing after dipstick-positive proteinuria and, when proteinuria is confirmed, subsequent initiation of renin-angiotensin system inhibitors (RASi) in patients with hypertension. However, real-world adherence to these recommendations remains unclear. We investigated management patterns following newly detected dipstick proteinuria in patients with treated hypertension. METHODS: Using the JMDC claims database, we identified treated hypertensive patients with newly detected dipstick proteinuria (≥1+, with prior results <1+) between Janu

Use of standardized order sets and associated outcomes in the Emergency Department: A scoping review.

Standardized order sets are common in Emergency Departments (EDs), but the extent of use, effectiveness, and associated outcomes remain unclear. The objective of this scoping review was to describe the current evidence regarding standardized order set use in the ED by examining conditions for which order sets are used, implementation and evaluation strategies, and clinical outcomes. We searched MEDLINE, EMBASE, CINAHL, Scopus, Web of Science, and the Cochrane Library from inception to April 3, 2025 for studies comparing the use of standardized order sets to usual care for ED patients. Two revi

Discharge medical treatment implementation and predictors of a successful decongestion in patients with acute heart failure: first data from the BRING-UP 3 Heart Failure Study.

BACKGROUND: Real-world uptake of guideline-directed medical therapy (GDMT) at hospital discharge and clinical predictors of complete decongestion in acute heart failure (AHF) populations remain insufficiently described. METHODS: The BRING-UP 3 HF study is an observational, prospective, nationwide investigation involving 179 Italian cardiology sites. This report summarizes baseline data from the first enrollment phase hospitalized cohort and assesses the predictors of decongestion via a machine learning model. RESULTS: Among 1373 patients (mean age 71 years; 30% females; 43% de-novo HF), HF w

[Vascular exercise and vascular medical rehabilitation].

Vascular-specific sports and exercise therapy should be offered as first-line therapy to all patients with intermittent claudication, provided that the patient is able to perform this therapy. The results should be evaluated after 3-6 months in terms of quality of life, and the vascular team should then discuss the next steps on an individual basis. Supervised walking training in a vascular sports group with a trained vascular trainer is the basic therapy. Home exercise training with targets and goal-oriented monitoring is equivalent to a structured training program in terms of the walking dis

Fixed-Dose Combinations for Hypertension: Barriers to Use and Opportunities for Health Systems.

BACKGROUND: Uncontrolled hypertension is a leading driver of heart disease, chronic kidney disease, heart failure, dementia, stroke, and pregnancy-related complications. Fixed-dose combinations (FDCs) are guideline-endorsed first-line therapies that improve blood pressure control by reducing pill burden, improving adherence, and targeting multiple pathophysiological mechanisms. However, FDC utilization remains low across U.S. health systems. Few studies examine the factors influencing FDC uptake; existing qualitative studies rarely interrogate structural barriers or formulate stakeholder-infor

The syndemic nexus: How vaccine hesitancy and COVID-19 interact to influence vaccine uptake among university students.

This study examined whether COVID-19 Vaccine Hesitancy (VH) and COVID-19-related factors interact to influence COVID-19 booster doses uptake among university students in Canada, from a syndemic perspective. A cross-sectional survey was conducted among 4453 students at the University of Waterloo in 2024. VH was measured toward both COVID-19 primary and booster doses. Change in VH scores were computed to capture shifts in hesitancy over time. Logistic regression models assessed the main effects of VH and COVID-19-related factors on booster uptake. Interactions were tested using additive and mult

Extended and comprehensive e-Health modalities to improve outcome in patients with worsened heart failure: design and rationale of the EXCEED-HF study.

AIMS: Remote monitoring (RM) strategies, such as telemonitoring and nurse-guided home care, aim to detect worsening heart failure (WHF) events early on to prevent hospitalization. Previous studies on RM strategies demonstrated benefits, but they were conducted before the widespread adoption of contemporary heart failure (HF) therapies. The EXCEED-HF study, which we present in this manuscript, evaluates whether an extended RM intervention-combining telemonitoring, nurse-guided care, and a digital communication platform-reduces WHF episodes and mortality within 180 days in patients with recent W
