Update on autoimmune polyendocrine syndromes (APS).

Autoimmune Polyendocrine Syndromes (APS) were initially defined as a multiple endocrine gland insufficiency associated to an autoimmune disease in a patient. Neufeld & Blizzard (1980) suggested a classification of APS, based on clinical criteria only, describing four main types. APS-1 is characterized by presence of chronic candidiasis, chronic hypoparathyroidism, Addison's disease. It is a very rare syndrome interesting young subjects correlating to different mutations of AIRE (AutoImmuneRegulator) gene on chromosome 21. APS-2 is characterized by presence of Addison's disease (always present)

A Review of the Scaffold Protein Menin and its Role in Hepatobiliary Pathology.

Multiple endocrine neoplasia type 1 (MEN1) is a familial cancer syndrome with neuroendocrine tumorigenesis of the parathyroid glands, pituitary gland, and pancreatic islet cells. The MEN1 gene codes for the canonical tumor suppressor protein, menin. Its protein structure has recently been crystallized, and it has been investigated in a multitude of other tissues. In this review, we summarize recent advancements in understanding the structure of the menin protein and its function as a scaffold protein in histone modification and epigenetic gene regulation. Furthermore, we explore its role in he

Inter-α-inhibitor heavy chain 4 levels in incident and prevalent patients with primary biliary cholangitis.

BACKGROUND: Primary biliary cholangitis (PBC) is an autoimmune liver disease characterised by progressive bile duct injury. We recently showed that inter-α-inhibitor heavy chain 4 (ITIH4) levels are elevated in patients with PBC. Here, we investigated the potential of ITIH4 as a biomarker for disease severity and treatment response. METHODS: We included 237 patients with primary biliary cholangitis, 80 incident (newly diagnosed), 157 prevalent (with established disease) and 100 healthy controls. All patients were followed for 12 months during initiation or continuation of ursodeoxycholic acid

Safety and diagnostic accuracy of ultrasound-assisted percutaneous liver biopsy in pediatric patients: a retrospective single-center study.

BACKGROUND: Liver biopsy remains essential for diagnosing and managing pediatric liver diseases, despite advances in noninvasive techniques. However, data on the current indications, diagnostic yield, and safety of ultrasound-assisted percutaneous liver biopsy in children are limited. METHODS: We retrospectively analyzed 60 pediatric patients who underwent ultrasound-assisted percutaneous liver biopsy at Agia Sofia Children's Hospital between January 2018 and September 2025. Data collected included demographics, laboratory values, biopsy indications, histopathology and complications. Biopsies

Causal Association Between Circulating Inflammatory Proteins and Autoimmune Liver Disease: a Bidirectional Two-Sample Mendelian Randomization Study.

INTRODUCTION: To investigate whether there is a direct causal relationship between circulating inflammatory proteins and autoimmune liver disease (AILD). MATERIALS AND METHODS: We collected genetic data for various AILD from the Genome Wide Association Studies (GWAS) dataset. The latest research provides GWAS data for 91 proteins associated with inflammation. Perform bidirectional two sample Mendelian randomization (MR) analysis using inverse variance weighted (IVW) to determine the causal relationship between inflammatory proteins and AILD, and use Mendelian randomization Egger method (MR Eg

Bidirectional causal links between diabetes mellitus and autoimmune liver diseases: Insights from Mendelian randomization.

BACKGROUND: Diabetes, a growing global public health issue, is often associated with autoimmune liver diseases (AILD), such as autoimmune hepatitis (AIH), primary biliary cholangitis (PBC), and primary sclerosing cholangitis (PSC), in both type 1 diabetes (T1DM) and type 2 diabetes (T2DM). However, the causal relationship between these conditions remains unclear. METHODS: We used two-sample bidirectional Mendelian randomization (MR) to explore causal links between AILD and diabetes. Genetic variants identified from genome-wide association studies were used as instruments. Sensitivity and mult

Profiling of circulating T follicular helper and T peripheral helper cell subsets in patients with primary biliary cholangitis.

Primary biliary cholangitis (PBC) is a common chronic cholestatic autoimmune liver disease that may progress to liver fibrosis or cirrhosis if not accurately diagnosed and promptly treated. T follicular helper (Tfh) cells and T peripheral helper (Tph) cells, as subsets of T cells that assist B cells in producing antibodies, are closely related to the pathogenesis of autoimmune diseases. This research aims to explore the potential of Tfh and Tph cells as biomarkers for PBC disease progression. We recruited 30 PBC patients, 25 post-hepatitis B cirrhosis (PHBC) patients, and 30 healthy subjects.

Immune checkpoint inhibitors: From friend to foe.

Immune checkpoints are crucial in regulating the activation of cell-mediated and humoral immune responses. However, cancer cells hijack this mechanism to evade the immune surveillance and anti-cancer response. Typically, receptors like PD-1 and CTLA4, expressed on immune cells, prevent the activation and differentiation of T cells. They also inhibit the development of autoimmune reactions. However, ligands such as PD-L1 for the receptor PD-1 are also expressed on the surface of cancer cells that help prevent the activation of anti-cancer immune responses by blocking the signalling pathways med

Erosive Lichen Planus With Ocular Involvement: A Case Report.

BACKGROUND Lichen planus, a chronic autoimmune inflammatory disorder, primarily affects the skin and mucous membranes but may occasionally involve the ocular surface. Ocular manifestations are rare, often underrecognized, and potentially similar to other cicatrizing conjunctivitides. If left untreated, ocular lichen planus may lead to substantial morbidity and permanent visual impairment. Early identification and interdisciplinary management are essential to prevent irreversible damage and systemic complications. Awareness of this entity among ophthalmologists and dermatologists is crucial for

The Impact of COVID-19 and COVID-19 Vaccination on Detection, Assessment, and Management of Suspected Acute Drug-Induced Liver Injury Occurring during Clinical Trials: Consensus Recommendations from the IQ DILI Initiative.

While the acute impact of the coronavirus disease 2019 (COVID-19) pandemic has waned, implications for clinical trials remain. In particular, guidance for evaluation of elevated liver tests due to COVID-19, its treatments, and COVID-19 vaccination is lacking. The IQ DILI Initiative, composed of experts from academia, regulatory agencies, and industry herein propose recommendations to address this gap. Extensive literature review was conducted and structured discussions were held between IQ DILI industry members, regulators, and academic experts in hepatology and DILI. Liver-related manifestati

Genetically predicted immune cells mediate the association between gut microbiota and autoimmune liver diseases.

BACKGROUND: Increasing evidence suggests an association between gut microbiota and Autoimmune Liver Diseases (AILDs). However, causal inference remains controversial due to confounding bias in observational studies. Additionally, there is currently no clear evidence indicating that immune cells act as intermediate phenotypes in the pathogenesis of AILDs. This study utilizes the Mendelian Randomization (MR) method to investigate the causal relationships among gut microbiota, immune cells, and AILDs. METHODS: Initially, we conducted a two-sample MR analysis to predict the causal relationships a

Elevated liver enzymes: unusual presentation of anti-MDA5 antibody-associated juvenile dermatomyositis.

Anti-melanoma differentiation-associated gene 5 antibody positive juvenile dermatomyositis (Anti-MDA5 JDM) is a subtype of JDM characterized by cutaneous ulcerations, arthritis, weight loss and progressive interstitial lung disease that can lead to significant morbidity and mortality, often warranting early aggressive treatment. It can present without the classical features of JDM (proximal muscle weakness, heliotrope rash and Gottron’s papules), making diagnosis challenging. We describe a 4-year-old male with Anti-MDA5 JDM who presented with significantly elevated liver enzymes, arthralgia, e
