Changes in blood pressure and renal function following subtotal parathyroidectomy in renal transplant patients presenting with persistent hypercalcemic hyperparathyroidism.

The aims of this retrospective study were to assess renal function and blood pressure after subtotal parathyroidectomy (PTx) performed in renal transplant (RT) patients presenting with persistent hypercalcemic hyperparathyroidism. We identified 34 patients (group A) from our records who had undergone PTx between 1981 and 1994. Group A included 18 women and 16 men with a mean age of 45 +/- 12 years and a mean time on dialysis therapy of 102 +/- 59 months. Thirty of the patients received cyclosporine A (CsA) with or without steroids and/or azathioprine (AZA) and the remaining 4 patients received…

Healthy elderly French women living at home have secondary hyperparathyroidism and high bone turnover in winter. EPIDOS Study Group.

It was recently demonstrated that calcium and vitamin D supplements were capable of decreasing the incidence of hip fractures in institutionalized elderly subjects through a reduction of senile secondary hyperparathyroidism. As there are no appropriate data to recommend such a supplement to the elderly living at home, the aim of this study was to determine the incidence of senile secondary hyperparathyroidism in old French women from the general community, its relation to vitamin D status, and its contribution to bone turnover. Four hundred and forty women, aged 75-90 yr, were randomly selecte…

Primary hyperparathyroidism: is technetium 99m-Sestamibi/iodine-123 subtraction scanning the best procedure to locate enlarged glands before surgery?

Technetium99m (99mTc)-Sestamibi/123I subtraction scanning was prospectively performed in 30 patients with primary hyperparathyroidism in an attempt to locate enlarged glands before first surgery. Imaging results were compared to surgical findings; the surgeon tried to identify all parathyroid glands. Twenty-seven patients were found to have a solitary adenoma during surgery, and 3 had multiglandular parathyroid hyperplasia. Twenty-six parathyroid adenomas (96%) were accurately located before surgery. The smallest gland detected weighted 125 mg. Preoperative detection of two mediastinal adenoma…

Lower Limb Fractures in Primary Hyperparathyroidism: A Systematic Review and Meta-Analysis of Incidence, Biochemical Risk Factors, and Surgical Management.

OBJECTIVE: Although skeletal fragility in primary hyperparathyroidism (pHPT) is well-recognized, data specific to lower limb fractures, the most disabling fractures, remain fragmented. This study aims to systematically synthesize evidence based on the existing literature to describe lower limb fractures in patients with pHPT, with emphasis on fracture characteristics and surgical management, and to perform a meta-analysis quantifying the risk of lower limb fracture in these patients. METHODS: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses 2020 guidelines, we …

Effect of parathyroidectomy versus non-surgical management on renal outcomes in primary hyperparathyroidism: protocol for a systematic review and meta-analysis.

INTRODUCTION: Primary hyperparathyroidism (PHPT) increases the risk of renal stones and progressive renal dysfunction. Parathyroidectomy is recommended for patients with renal involvement, yet whether surgery improves renal outcomes compared with non-surgical management remains unclear. Prior reviews have focused mainly on biochemical or skeletal outcomes, included few renal events and largely predate recent large cohort studies using contemporary methods to evaluate renal stones, chronic kidney disease (CKD) progression and long-term estimated glomerular filtration rate (eGFR) decline. A cont…

The role of ESRRA and JUNB in oxyphil cells: insights from spatial transcriptomics and single-cell transcriptomics in uremic secondary hyperparathyroidism.

Oxyphil cells are a primary cellular component of the parathyroid gland. Due to the limitations of techniques for separating oxyphil cells from chief cells, the characteristics and functions of oxyphil cells in uremic secondary hyperparathyroidism (SHPT) remain largely unclear. Therefore, we integrated spatial transcriptomics and single-cell transcriptomics to investigate the characteristics of oxyphil cells and the association between oxyphil cells and calcitriol resistance in SHPT. 6 uremic SHPT samples and 3 normal parathyroid samples were used for single-cell transcriptomics, while the mos…

Can the outpatient parathormone value be used instead of the initial value in intraoperative parathormone measurement during primary hyperparathyroidism surgery?

Intraoperative parathyroid hormone (IOPTH) measurement is widely used to confirm surgical success in primary hyperparathyroidism (PHPT). The Miami criterion is the most widely used. Despite the significant advantages of IOPTH, its cost remains a matter of debate. This study aimed to investigate the necessity of pre-excision parathyroid hormone (PTH) measurement during parathyroidectomy in PHPT patients. The files of 159 patients who underwent parathyroidectomy in our department between January 2022 and December 2023 were retrospectively reviewed. Patients aged 18 years and older who underwent …

A machine learning prediction model and online calculator for postoperative recurrence of secondary hyperparathyroidism: A dual-center development and validation study.

BACKGROUND: Secondary hyperparathyroidism carries a high recurrence risk after parathyroidectomy (PTX), requiring early identification of high-risk patients. Using a two-center cohort, we developed a machine learning prediction model and deployed it as an online calculator. METHODS: We included 391 SHPT patients undergoing PTX at two hospitals. Cohort 1 was split 7:3 into training and internal validation sets; cohort 2 served as external validation. Feature selection used LASSO and Boruta, SMOTE handled imbalance, and six models (random forest, XGBoost, etc.) were built. After cross-validatio…

Determinants of Severe Hypocalcemia After Parathyroidectomy in Patients with End-Stage Kidney Disease and Renal Hyperparathyroidism: A Retrospective Cohort Study.

Background: Parathyroidectomy (PTX) is generally curative in renal hyperparathyroidism (RHPT) that is refractory to medical treatment in end-stage kidney disease (ESKD) patients. Severe hypocalcemia is a common complication of PTX and results in increased monitoring, interventions, lengths of stay, and costs of care. This study aimed to find the determinants and cutoff values of the biochemical determinants, if any, for severe post-operative hypocalcemia after PTX in adult patients with ESKD. Methods: Severe post-operative hypocalcemia was defined as a lowest adjusted serum calcium level < 2 m…
