"Virtual Lateral Wall" Technique to Overcome Nail Entry Pathway Errors and Optimize Success of Intramedullary Nailing in Pertrochanteric Fractures.

OBJECTIVE: The use of intramedullary implants for the treatment of pertrochanteric fractures is well-established. Nail entry pathway error is a common intraoperative challenge, particularly in pertrochanteric fractures with unstable greater trochanter fragment. This study introduces our innovative virtual lateral wall technique, which allows "lateral wall fracture" simplification from unstable to stable and effectively guides the reamer to create an accurate entry pathway for nail insertion in the treatment of pertrochanteric fractures. METHODS: A retrospective study was conducted on 12 conse

Comparison of robotic-assisted and laparoscopic-assisted surgery in the treatment of children with Hirschsprung's disease: a systematic review and meta-analysis.

BACKGROUND: This study aims to systematically evaluate the differences between robotic-assisted surgery (RAS) and laparoscopic-assisted surgery (LAS) in intraoperative parameters, postoperative complications, and prognostic outcomes for children with Hirschsprung's disease (HSCR). By conducting a meta-analysis, evidence-based insights for clinical practice were sought. METHODS: Following PRISMA guidelines, PubMed, Embase, and Web of Science were searched up to May 10, 2025, to identify comparative studies of RAS and LAS for HSCR. Two reviewers independently screened literature and assessed qu

Drive Pressure-Guided Individualized Positive End-Expiratory Pressure in Traumatic Brain Injury Surgery: A Randomized Controlled Trial.

AIM: Intraoperative lung-protective ventilation strategies (LPVS) have been shown to improve lung oxygenation and prevent postoperative pulmonary problems in surgical patients. However, the application of positive end-expiratory pressure (PEEP)-based LPVS in emergency traumatic brain injury (TBI) has not been thoroughly explored. The purpose of this study is to evaluate the effects of drive pressure-guided individualized PEEP on perioperative pulmonary oxygenation, postoperative pulmonary complications, and recovery from neurological injury in patients with TBI. METHODS: A total of 111 TBI pa

Evaluation of Factors Associated with, and Outcomes for Patients with Nonhome Discharge Destinations Following Carotid Endarterectomy.

INTRODUCTION: Following a carotid endarterectomy (CEA) procedure, patients are discharged to their homes or other locations than home such as an acute care facility or skilled nursing facility based on their functional status and level of medical attention needed. Decision-making for discharge destination following a CEA to home or nonhome locations is important due to the differences in survival and postoperative complications. While primary outcomes such as mortality and occurrence of stroke following CEA have been extensively studied, there is a paucity of information characterizing outcome

Complications of thyroidectomy and parathyroidectomy in the rural community hospital setting.

The objective of this study was to examine the complications encountered in a series of 150 consecutive thyroid and parathyroid procedures performed by a single surgeon in a rural community hospital setting. The goal was to demonstrate that these procedures may be performed safely and with outcomes equivalent to those of academic hospitals in major metropolitan areas despite the lack of availability of specific technologies that are being increasingly used for these procedures in tertiary care settings. Specifically, these include intraoperative parathyroid hormone measurements, intraoperative

Intramedullary reamer failure in orthopaedic surgery: Two case reports and practical recommendations for prevention.

Intramedullary reaming has been proven to be a safe and effective method for enhancing the union rates of long bone fractures. However, there is a risk of equipment failure, which can lead to severe complications. We present two cases of reamer failure during femoral nailing which illustrate the rare occurrence of intraoperative instrument failure. Our report also underscores the importance of routinely inspecting reaming equipment and provides technical insights to reduce the risk of failure.

Intraconal orbital dermoid cyst: a rare location.

Intraconal dermoid cysts are very unusual in routine clinical practice. Clinical symptoms depend upon the site and extension of the lesion. Though rare, proptosis, diplopia, and orbital pain are the presenting symptoms encountered in patients with an intraorbital dermoid cyst. Although radiology can be diagnostic, a complete correlation with the final histopathology is always mandatory for its confirmation. Endoscopic excision of the cyst ensures a complete cure for the disease without any intraoperative/postoperative complications.

Treatment resistant fungal keratitis caused by Colletotrichum gloeosporioides.

CASE REPORT: A 56 year old woman suffered corneal injury from a branch of an orange tree. Forty days later she suffered a severe ocular infection, positive to Colletotrichum gloeosporioides (C. gloeosporioides). The patient did not respond to traditional treatment or crosslinking, and had to be treated with keratoplasty, suffering intraoperative and postoperative complications. DISCUSSION: Ocular infections due to C. gloeosporioides can occasionally be refractory to traditional and new treatments, such as crosslinking.

Laparoscopic hysteropexy in a patient with spina bifida and ventriculoperitoneal shunt.

Laparoscopic repair of pelvic organ prolapse in patients with ventriculoperitoneal shunts has not been previously described. The optimum management of patients with ventriculoperitoneal shunts undergoing laparoscopy is uncertain. We describe the case of a 21-year-old female patient with spina bifida and ventriculoperitoneal shunt who underwent laparoscopic hysteropexy for severe pelvic organ prolapse. The implications of performing laparoscopy on patients with ventriculoperitoneal shunts are reviewed along with strategies to reduce potential intraoperative complications.

Prepancreatic postduodenal portal vein: a new hypothesis for the development of the portal venous system.

The prepancreatic postduodenal portal vein (PPPV) is an unusual anomaly, having been reported in only 11 cases including ours. Although its incidence is rare, this anomaly can cause intraoperative complications such as hemorrhage from the abnormal vein. Radiologists therefore need to be aware of this anomaly and, when identifi ed, report it to surgeons before surgery. Here we report two cases of PPPV and present a new hypothesis for development of the portal venous system.

Progressive systemic sclerosis: clinical manifestations and anesthetic considerations.

Progressive systemic sclerosis is a complex disease that involves multiple organ systems. Patients with progressive systemic sclerosis can pose a significant challenge for the anesthesiologist. As the disease progresses, profound musculoskeletal, gastrointestinal, pulmonary, renal, and cardiac system changes occur. The anesthesiologist should understand these complex pathophysiological processes so as to minimize potential risks, including aspiration and other pulmonary complications. In addition, the potential for reduced renal function and intraoperative hypothermia-induced vasospasm must be

[Our experience with the surgical treatment of cholelithiasis].

The treatment results in 312 patients with cholelithiasis, covering the period 1989 to 1993, are retrospectively analyzed. Overall postoperative lethality amounts to 4.4 per cent. The obtained results are fully comparable in terms of both missed concrements left behind within the extrahepatic bile ducts, and in terms of postoperative complications, associated with the indications for choledochotomy. This is attributed to the therapeutic approach algorithm adopted with respect to the indications for intraoperative cholangiography and choledochotomy.

Persistent left superior vena cava: Review of embryologic anatomy and considerations for cardiopulmonary bypass.

The purpose of this report is to alert the perfusionist to the anatomy of persistent left superior vena cava (PLSVC) and the intraoperative complications that this anomaly may present. The perfusionist should be able to devise a venous circuit that will accommodate three cannulae. He should also be aware that, during cardiopulmonary bypass, a copious volume of deoxygenated blood from the left heart sump, with simultaneous cerebral congestion, may signal the presence of undocumented PLSVC.
