Isolated fracture-avulsions of the tibial insertions of the cruciate ligaments of the knee.

This paper discusses the treatment of isolated fracture-avulsions of the tibial insertions of the cruciate ligaments of the knee. The indications for conservative treatment or open reduction and osteosynthesis, are based on a classification by Meyer and McKeever which takes into account the displacement of the avulsed fragment. This is the crucial factor in the choice of treatment.

Popliteal venotomy during posterior cruciate ligament reconstruction in the setting of a popliteal artery bypass graft.

Injury to the vascular structures in the popliteal fossa during arthroscopic cruciate ligament reconstruction can be limb threatening or even life threatening. We present the first report, to our knowledge, of an isolated injury to a popliteal vein during arthroscopic posterior cruciate ligament reconstruction. Unfortunately, the venotomy led to cardiopulmonary arrest and flash pulmonary edema in this patient. Preoperative planning is paramount to assess risk of injury to vascular structures, which may be increased in patients who have had prior procedures on the affected knee. Furthermore, va

Growth arrest following posterior knee release in children with arthrogryposis multiplex congenita: a previously unreported complication.

Knee flexion contractures in children with arthrogryposis multiplex congenita (AMC) significantly impair function and ambulation. Posterior knee soft tissue release is a widely used surgical intervention when conservative management fails. However, literature describing complications following this procedure remains limited. This study aims to report distal femoral and proximal tibial physeal arrest as a previously undocumented complication following posterior knee release in children with AMC. Following institutional review board approval, a retrospective case series was performed. Patients w

Genicular artery embolisation for the management of knee osteoarthritis: a prospective randomised study.

AIM: The aim of this study is to evaluate the efficacy of genicular artery embolisation (GAE) in improving pain and functional outcomes in patients with symptomatic knee osteoarthritis (KOA). MATERIALS AND METHODS: Fifty patients with KOA cases (Kellgren-Lawrence grade 2-4) were enrolled in a prospective, parallel arm, randomised study. The control arm received standard care, while the interventional arm received a single session of GAE with polyvinyl alcohol (PVA) particles, in addition. Visual analogue scale (VAS) and Knee Injury and Osteoarthritis Outcome Score (KOOS) scores were compared

Bilateral Distal Femoral Osteotomy for Valgus Knee Deformity May Result in Improved Patient-Reported Outcome Scores.

BACKGROUND: The lateral opening wedge distal femoral osteotomy (DFO) is an effective treatment for genu valgum to improve mechanical alignment, decrease lateral compartment loads, and decrease the risk of knee osteoarthritis. Multiple studies have utilized outcome scores assessing functional changes related to pain and joint stability, with a focus solely on knee pain and functional outcomes. QUESTIONS/PURPOSES: The primary aim of this study was to evaluate patient-reported outcome measures (PROMs) that assessed knee function, pain, patient's perception of body image, and limb deformity-relat

Large multiplanar changes to native alignment have no apparent impact on clinical outcomes following total knee arthroplasty.

PURPOSE: This study sought to examine if achieved postoperative alignment when compared to the native anatomy would lead to a difference in Patient Reported Outcome Measures (PROMs), and whether the achieved alignment could be broadly categorised by an accepted alignment strategy. METHODS: A retrospective cohort study of prospectively collected data on patients undergoing single primary or bilateral simultaneous total knee arthroplasty (TKA) was carried out. CT scans were used to determine the mean change ("delta values") between the pre and postoperative; hip-knee-ankle angle, lateral distal

Arthroscopic retrieval of knee foreign bodies in pediatric: a single-centre experience.

PURPOSE: The present study was to investigate the clinical features of foreign body (FB) residues in children's knee joints, surgical retrieval, and postoperative function of knee joints. METHODS: This retrospective study included a total of 13 children (8 boys and 5 girls; the mean age is 6.69 years old, range from 2 to 16 years old) who underwent retrieval surgery of knee FBs in our hospital. Related clinical factors were recorded and analyzed to find the influence factors of surgical methods and FBs' location change. RESULTS: The FBs in 11 cases were removed completely by arthroscopy, two

Comparison of KOOS Scores of Middle-Aged Patients Undergoing Total Knee Arthroplasty to the General Dutch Population Using KOOS Percentile Curves: The LOAS Study.

BACKGROUND: We aimed to investigate the application of the Knee Injury and Osteoarthritis Outcome Score (KOOS) percentile curves, using preoperative and postoperative data of patients with knee osteoarthritis undergoing total knee arthroplasty (TKA). METHODS: We used Longitudinal Leiden Orthopedics Outcomes of Osteo-Arthritis study data of patients between 45 and 65 years and undergoing primary TKA. KOOS scores (0-100) were obtained preoperatively and 6, 12, and 24 months after TKA. Preoperative knee radiographs were assessed according to Kellgren-Lawrence (KL) in a subset (37%) of patients.

Chronic Knee and Ankle Pain Treatment through Selective Microsurgical Approaches: A Minimally Invasive Option in the Treatment Algorithm for Refractory Lower Limb Pain.

BACKGROUND: Injury or compression of a sensory nerve is an under-reported source of disabling pain in the lower limb. It is known that peripheral nerve microsurgeons can reconstruct and rewire injured nerves to relieve chronic pain but this option remains not completely understood and ignored by most orthopaedic surgeons, neurologists, and pain therapists. In this paper, we describe our experience with knee and ankle peripheral nerve surgery to improve the condition of patients suffering from chronic, posttraumatic lower limb pain. PATIENTS AND METHODS: Between 2015 and 2018, a retrospective

Does Unicompartmental Knee Replacement Offer Improved Clinical Advantages Over Total Knee Replacement in the Treatment of Isolated Lateral Osteoarthritis? A Matched Cohort Analysis From an Independent Center.

BACKGROUND: The purpose of this study is to compare the functional and radiographic results, perioperative complications, satisfaction rate, and mid-term survivorship after unicompartmental knee arthroplasty (UKA) and total knee arthroplasty (TKA) for the treatment of lateral compartmental knee osteoarthritis (LCKO). METHODS: Between March 2007 and September 2017, we identified 35 patients with primary TKAs and 121 patients with lateral UKAs (LUKAs) for LCKO with a minimum follow-up of 2 years (mean 5.3 years, range 2-12.4). The matched variables were age, gender, operation side, body mass in

Myoelectric control algorithm for robot-assisted therapy: a hardware-in-the-loop simulation study.

BACKGROUND: A direct blow to the knee is one way to injure the anterior cruciate ligament (ACL), e.g., during a football or traffic accident. Robot-assisted therapy (RAT) rehabilitation, simulating regular walking, improves walking and balance abilities, and extensor strength after ACL reconstruction. However, there is a need to perform RAT during other phases of ACL injury rehabilitation before attempting an advanced exercise such as walking. This paper aims to propose a myoelectric control (MEC) algorithm for a robot-assisted rehabilitation system, "Nukawa", to assist knee movement during th

Heeled Shoes Increase Knee Work Demand During Repeated Hopping in Dancers.

AIMS: Dancers have high injury rates of the lower extremity. External factors such as footwear likely alter the work demands placed on the lower extremity joints when performing dance-style movements. Research on pedestrians shows an increase in knee work demand when wearing heels, which may occur in dancers as well. The purpose of this study was to assess the effect of heeled shoes on lower extremity joint work demand during a basic dance-specific jump (sauté) in dancers when compared to barefoot. METHODS: Ten healthy, female dancers (age 25.6±4.7 yrs; height 1.7±0.1 m; weight 64.9±9.2 kg; m

No effect of creatine monohydrate supplementation on inflammatory and cartilage degradation biomarkers in individuals with knee osteoarthritis.

The study purpose was to evaluate the effectiveness of creatine monohydrate supplementation (20 grams/day for 1 week and then 5 grams/day for 11 weeks) on inflammation (C-reactive protein, interleukin-1β, interleukin-6, s100 A8/A9, and tumor necrosis factor-α) and cartilage degradation (serum cartilage oligomeric matrix protein) in patients with knee osteoarthritis. We hypothesized that supplementing with creatine monohydrate for 12 weeks would lower biomarkers of inflammation and cartilage degradation in patients with knee osteoarthritis when compared to placebo. A total of 18 patients with m

Effect of femoral tunnel length on the safety of anterior cruciate ligament graft fixation using cross-pin technique: a cadaveric study.

BACKGROUND: A more oblique placement of the anterior cruciate ligament (ACL) graft has been related to better control of rotatory knee stability. Femoral fixation with a transverse system might injure its posterolateral structures. HYPOTHESIS: A cross-pin system, originally developed for transtibial reconstruction of the ACL, can safely be used when creating a lower femoral tunnel through the anteromedial portal. However, a long femoral tunnel must be created to protect the posterolateral structures of the knee. STUDY DESIGN: Controlled laboratory study. METHODS: An ACL was arthroscopically
