Selected patients with minor lower limb injuries and low risk for deep vein thrombosis (DVT), who required cast immobilizing and did not receive medical thrombosis prophylaxis, were included in a prospective study. Lower-limb movements and muscle-activity patterns were assessed from seven normal and seven ambulatory subjects with incomplete spinal-cord injury (SCI) during level and uphill treadmill walking (5, 10 and 15 degrees). A 15-year-old female suffered from a persistent ulcer which developed in her knee. At the ninth operation, intraarticular knee arthrodesis was performed but the ulcer recurred. The most effective treatment was neither thorough debridement nor a free musculocutaneous flap covering, but the application of a plaster cast. Degloving injuries are severe and frequently underestimated lesions. In a retrospective study, 65 patients treated between 1985 and 1991 were reviewed. Therapy consisted of surgical exploration as soon after injury as possible, with defatting of the avulsed skin and its replacement on the most functionally important sites. The remaining raw areas were covered with split skin grafts (SSG). This technique reduced morbidity, hospital stay and work incapacity, as compared with patients treated by other methods.

In the article are presented the results of clinico-electromyographic examination of 57 patients with complicated damages of lumbar part of the spine during acute, early, interim and late periods of treatment. During the acute period of the spine damage are singled out 3 types of manifestation of total spontaneous and induced (N-reflex, M-wave) bioelectrical activity, demonstrated the possibility of their application for prognostication of the operative and restorative treatment results.

We report on 21 cases with combined soft tissue and far reaching bone defects of the lower leg. The bone defects were bridged by the fibula-pro-tibia-operation. In eleven patients the restoration of the full weightbearing leg was achieved. Five patients have to use a supporting walking splint at the end of the medical treatment. In four cases we found fistulas without secretion. Five amputations of the lower leg were unavoidable.

In the period 1972 to 1989, 92 sports men and women were treated for stress fracture. Symptoms included localized bony pain aggravated by activity. Physical examination demonstrated point tenderness and pain on loading the bone. The majority of fractures were in the lower limbs but axial skeleton and upper limb bones were also affected. Radiographs showed evidence in only 49% of fractures. Bone scanning, on the other hand, was found to be very accurate and indicative immediately. Therefore, patients with a clinical history of stress fracture and a normal radiograph should undergo a bone scan.

The use of fasciocutaneous flaps to cover soft tissue defects of the lower leg following trauma, is discussed in this article. Our experience with 15 cases is presented. There have been no complications. We feel that fasciocutaneous flaps are a safe and reliable method for the management of difficult wounds of the lower leg.

Arterial injuries below the adductor hiatus in the lower extremity result in amputation more often than do injuries in any other site. The major deterrents to limb salvage are delay in diagnosis and revascularization and extensive adjacent bone and soft-tissue trauma. Rapid diagnosis and repair are needed for all popliteal artery injuries and for infrapopliteal injuries that compromise distal flow. The optimal management of asymptomatic injuries of single shank arteries with normal flow in uninjured parallel vessels is not defined.

Four patients with the crush syndrome due to prolonged limb compression were treated at Cook County Hospital, Chicago. Oral diosmin (a benzo-pyrone) was used to treat rats with contused lungs, in doses of 50 or 200 mg/kg/day. The contusion was produced by direct trauma. The lungs were examined with the electron microscope, both qualitatively and quantitatively, at 1, 2 and 4 days.
