The incidence of testicular infiltrates in 68 boys with acute lymphoblastic leukemia in first remission (1974-81), was prospectively investigated through careful clinical examination and routine bilateral biopsies at 2-3 years of remission. Full skeletal survey was performed on 54 children presenting over a 6-year period (1973-1979) with acute lymphoblastic leukaemia (ALL). Findings that were considered attributable to leukaemia included metaphyseal bands, osteolytic lesions and periosteal reactions. One or more of these roentgenographic findings was observed in 24 children (44%). The mean duration of remission and of survival was much shorter in cases with multiple bone involvement (3 or more lesions) than in those where bone involvement was absent or limitated at 1 or 2 bone lesions.

Bone marrow and peripheral blood cultures of chronic lymphocytic leukemia patients were mitogenically stimulated with TPA (12-O-tetradecanylphorbol-13-acetate). Clonal cytogenetic abnormalities were detected in frequencies varying from 15% to 100%, in five of the six patients studied. We designed a protocol (VAPA) that featured 14 mo of intensive postremission induction chemotherapy in an effort to improve remission durations for patients with acute myelogenous leukemia (AML). 5 patients developed acute nonlymphocytic leukemia (ANLL) 2-4 years following the use of various cytotoxic agents for other primary disease. Various chemical inducers have effects on the induction of terminal differentiation of human myelogenous leukemia cell lines. We studied morphological and functional changes of human leukemia cells freshly obtained from patients using 12-0-tetradecanoyl phorbol-13-acetate (TPA), retinoic acid (RA) or dimethyl sulphoxide (DMSO). A white female neonate with clinical manifestations of Down's syndrome was found to have a peripheral white count of 74,000/mm3 with 65% blast forms. Bone marrow aspirate revealed 36% blasts. On chromosomal analysis of bone marrow and peripheral blood, two cell lines were found, one with trisomy 21 and another with pentasomy 21. Colony cultures on the patient's bone marrow cells revealed an adequate growth pattern of CFU/GM. Cells were reactive with myeloblast alloantisera. At the age of 3 weeks, peripheral counts were within normal limits as was morphology of the repeat bone marrow aspirate.

Quantitative and qualitative investigations of the erythropoiesis in the sternal marrow of untreated 52 patients with a plasmacytoma and 46 patients with a chronic lymphatic leukaemia resulted in about half the cases in an indicated or more distinct megaloblastic disturbance of maturation in a part of the erythroblasts. These changes of the erythroblastic cells might be the expression of a neoplastic transformation and of a disturbance of the differentiation of the pluripotent stem cells in these diseases.

In an attempt to establish a possible correlation between clinical course and initial characteristics of the disease, 88 children with ALL (diagnosed between 1970-1978) were studied. From 1972 to 1981 10 patients with hairy cell leukaemia were observed in the Medical University Clinic Cologne, this represents 1.8% of all leukaemias. Typical clinical signs are splenomegaly, no or only slightly enlarged lymph nodes and a moderate hepatomegaly.
