Intratumoral hemorrhage from brain metastases following nivolumab therapy for malignant melanoma: illustrative case.

BACKGROUND: Malignant melanoma is a highly aggressive type of cancer that frequently causes brain metastases, leading to poor outcomes. Immune checkpoint inhibitors (ICIs), such as nivolumab, play a crucial role in treating metastatic melanoma, yet intracranial hemorrhage (ICH) is a rare but serious complication. Although pseudoprogression and hyperprogressive disease are recognized phenomena associated with ICIs, the precise mechanisms connecting ICIs to ICH are not yet fully understood. OBSERVATIONS: The authors report the case of a 75-year-old man with stage IV malignant melanoma who exper

The Banded One-Anastomosis Gastric Bypass Trial (RiMini Trial): Protocol of a Prospective Single-Center Randomized Controlled Trial.

BACKGROUND: Long-term recurrent weight gain remains a persistent challenge in metabolic bariatric surgery (MBS). One strategy for managing recurrent weight gain involves the placement of a non-adjustable silicone ring around the reduced stomach pouch. This technique may lead to more significant weight loss and a reduced risk of long-term recurrent weight gain. Although several studies have demonstrated the effectiveness of silicone rings in combination with Roux-en-Y gastric bypass (RYGB), randomized studies providing long-term data on the effectiveness of primary banded one-anastomosis gastri

Dietary salt, vascular dysfunction, and cognitive impairment.

Excessive salt consumption is a major health problem worldwide leading to serious cardiovascular events including hypertension, heart disease, and stroke. Additionally, high-salt diet has been increasingly associated with cognitive impairment in animal models and late-life dementia in humans. High-salt consumption is harmful for the cerebral vasculature, disrupts blood supply to the brain, and could contribute to Alzheimer's disease pathology. Although animal models have advanced our understanding of the cellular and molecular mechanisms, additional studies are needed to further elucidate the

Carvedilol versus flecainide treatment in idiopathic ventricular extrasystoles and tachycardias (CARFLECT IV) trial: Rationale and design.

INTRODUCTION AND OBJECTIVES: Idiopathic symptomatic non-sustained ventricular arrhythmias (INVA), including premature ventricular contractions and non-sustained ventricular tachycardias, are generally considered benign. Symptoms and left ventricular dysfunction are indications for treatment with antiarrhythmic drugs. The SCN5A gene encodes part of the voltage-dependent sodium channel type V alpha, which is the primary target of flecainide. The H558R polymorphism of this gene has been associated with a better response to flecainide in atrial fibrillation. The aim of this study is to describe th

From Stress to Substance Use Disorders: The Expanding Role of Microglia-Astrocyte Crosstalk in Neuroimmune and Glutamate Alterations in the Nucleus Accumbens.

This review examines convergent neurobiological mechanisms linking stress and drugs that drive stress-induced drug-related behaviors. It first outlines the main theoretical frameworks explaining substance use disorders (SUDs), emphasizing vulnerability factors-particularly stressful life events-that increase addiction risk. The analysis integrates preclinical evidence demonstrating that chronic stress facilitates cross-sensitization to psychostimulants and accelerates drug self-administration, underscoring how stress and drugs converge on glutamatergic and dopaminergic transmission within the

Efficacy of exercise therapies on functional dyspepsia: A systematic review and meta-analysis.

Drug therapy is the primary treatment for functional dyspepsia (FD), but exercise therapy may offer a promising, safe, and effective adjunct. This meta-analysis evaluated the effectiveness of exercise therapy compared to conventional treatments for FD. We reviewed randomized controlled trials (RCTs) published up to October 13, 2024, from five databases. Pooled estimates were calculated using random-effects models, with outcomes expressed as risk ratio (RR), mean difference (MD), or standardized mean difference (SMD), and 95 % confidence intervals (CI). Subgroup analyses were performed based on

Long-term efficacy and safety of mitapivat in non-transfusion-dependent α- or β-thalassaemia: An open-label phase 2 study.

Non-transfusion-dependent thalassaemia (NTDT) can result in serious complications and comorbidities that can impact patients' quality of life. Mitapivat, a first-in-class, oral, small-molecule allosteric activator of red blood cell pyruvate kinase, is under investigation in adults with thalassaemia. Through its mechanism of action, mitapivat increases adenosine triphosphate, leading to improvements in red blood cell health, ineffective erythropoiesis and haemolysis. An open-label, multicentre, phase 2 study (NCT03692052) is evaluating mitapivat 100 mg twice daily in adults with NTDT. We previo

Cardiometabolic risk factors in adults with cystic fibrosis undergoing elexacaftor/tezacaftor/ivacaftor therapy.

The introduction of elexacaftor/tezacaftor/ivacaftor (ETI) therapy has further extended life expectancy of adults with cystic fibrosis (awCF), highlighting the need for increased attention to potential long-term health issues. Given the increasing prevalence of cardiovascular diseases in the ageing population and the presence of cardiovascular risk factors associated with CF, understanding the impact of ETI on cardiometabolic risk factors is a crucial clinical concern. The aim of our prospective observational study was to explore early changes in cardiac and metabolic biomarkers after 6 months

Despite popular intuition, positive world beliefs poorly reflect several objective indicators of privilege, including wealth, health, sex, and neighborhood safety.

OBJECTIVES: We tested whether generalized beliefs that the world is safe, abundant, pleasurable, and progressing (termed "primal world beliefs") are associated with several objective measures of privilege. METHODS: Three studies (N = 16,547) tested multiple relationships between indicators of privilege-including socioeconomic status, health, sex, and neighborhood safety-and relevant world beliefs, as well as researchers and laypeople's expectations of these relationships. Samples were mostly from the USA and included general population samples (Study 2) as well as focused samples of academic

Genetic insights into the age-specific biological mechanisms governing human ovarian aging.

There is currently little evidence that the genetic basis of human phenotype varies significantly across the lifespan. However, time-to-event phenotypes are understudied and can be thought of as reflecting an underlying hazard, which is unlikely to be constant through life when values take a broad range. Here, we find that 74% of 245 genome-wide significant genetic associations with age at natural menopause (ANM) in the UK Biobank show a form of age-specific effect. Nineteen of these replicated discoveries are identified only by our modeling framework, which determines the time dependency of D
