Adherence to Compression Stockings for Venous Leg Ulcer Prevention: A Pilot Randomised Controlled Trial and Health Economic Analysis, Evaluating a New Multidimensional Tool (PAMCAI).

Venous leg ulcer recurrence can be prevented with daily compression stocking wear; however, stocking effectiveness is often hindered by poor patient adherence. The 'Personalised and Multi-dimensional Compression Assessment and Intervention' (PAMCAI) is a multidimensional decision aid, delivered by clinicians via an iPad application, designed to improve adherence. This study piloted the methodology and feasibility of PAMCAI's efficacy and cost-effectiveness for evaluation in a larger randomised controlled trial (RCT). Using a two-arm, single-blinded pilot RCT with cost-utility analysis, PAMCAI

The Saudi National Policy and Protocol for Epidermolysis Bullosa.

This protocol's objective is to offer evidence-based suggestions for the identification, treatment, and management of epidermolysis bullosa (EB) in Saudi Arabia. EB is a rare genetic condition that results in blistering and skin fragility. Depending on the subtype, it can produce a variety of consequences. The four primary kinds of EB are EB Simplex, Junctional EB, Dystrophic EB, and Kindler EB. This guideline provides a thorough understanding of EB and describes diagnostic techniques including genetic testing and immunofluorescence mapping. The significance of an interdisciplinary team (IDT)

Remote Monitoring in Myasthenia Gravis: Exploring Symptom Variability.

BACKGROUND: Myasthenia gravis (MG) is a rare, autoimmune disorder characterized by fluctuating muscle weakness and potential life-threatening crises. While continuous specialized care is essential, access barriers often delay timely interventions. To address this, we developed MyaLink, a telemedical platform for MG patients. This study evaluated whether frequently assessed clinical outcomes via MyaLink can capture symptom variability between clinical visits. METHODS: In this randomized, controlled, 12-week study, 45 MG patients received either standard care (control, N = 15) or standard care

Surgical Care Through a Community Free Clinic-Ambulatory Surgical Center Partnership.

BACKGROUND: Disparities secondary to underinsurance present throughout the surgical care continuum. Community free clinics are uniquely capable to provide health care services to the medically underserved, but surgery often falls outside their scope of care. METHODS: Retrospective chart review was conducted on consecutive community free clinic patients receiving free surgical services via referral to a partnering ambulatory surgery center between March 2016 and September 2021. Those with documented contact information were recruited 1-3 years post-procedure for long-term quality-of-life (LTQO

CARE: Protocol of a randomised trial evaluating the feasibility of preoperative intentional weight loss to support postoperative recovery in patients with excess weight and colorectal cancer.

AIM: Excess weight increases the risk of morbidity following colorectal cancer surgery. Weight loss may improve morbidity, but it is uncertain whether patients can follow an intensive weight loss intervention while waiting for surgery and there are concerns about muscle mass loss. The aim of this trial is to assess the feasibility of intentional weight loss in this setting and determine progression to a definitive trial. METHODS: CARE is a prospectively registered, multicentre, feasibility, parallel, randomised controlled trial with embedded evaluation and optimisation of the recruitment proc

Nursing Interventions and Care Strategies for Patients with Coronary Heart Disease: A Comprehensive Review.

Cardiovascular diseases are a major cause of death worldwide, and coronary heart disease (CHD) is a prevalent cardiovascular condition and a significant health burden for the population. In this disease, insufficient blood flow to the heart due to plaque accumulation in the coronary arteries causes chest pain, heart attack, and even death. So, it is vital to identify risk factors, prevention, appropriate treatment, and rehabilitation. Nurses play an indispensable role in managing and caring for patients with CHD. Indeed, they possess a deep understanding of the disease and its complexities, en

Mangled extremity: limb salvage for reconstruction versus primary amputation.

PURPOSE OF REVIEW: While MESS has historically influenced limb salvage versus amputation decisions, its universal applicability remains uncertain. With trauma systems expanding and advancements in trauma care, the need for a nuanced understanding of limb salvage has become paramount. RECENT FINDINGS: Recent literature reflects a shift in the management of mangled extremities. Vascular surgery, plastic surgery, and technological advancements have garnered attention. The MESS's efficacy in predicting amputation postvascular reconstruction has been questioned. Machine learning techniques have em

Cancer Survivors' Disability Experiences and Identities: A Qualitative Exploration to Advance Cancer Equity.

Eliminating cancer-related disparities is a global public health priority. Approximately 40% of cancer survivors experience long-term effects of cancer which can lead to activity limitations and participation restrictions; yet discussions of disability are largely absent from clinical and research cancer health equity agendas. The purpose of this study was to explore how cancer survivors experience and make sense of the long-term disabling effects of cancer and its treatments. In this qualitative study, data were collected via in-depth semi-structured interviews with survivors of breast cancer

Frailty in Acute and Chronic Coronary Syndrome Patients Entering Cardiac Rehabilitation.

Worldwide population ageing is partly due to advanced standard of care, leading to increased incidence and prevalence of geriatric syndromes such as frailty and disability. Hence, the age at the onset of acute coronary syndromes (ACS) keeps growing as well. Moreover, ageing is a risk factor for both frailty and cardiovascular disease (CVD). Frailty and CVD in the elderly share pathophysiological mechanisms and associated conditions, such as malnutrition, sarcopenia, anemia, polypharmacy and both increased bleeding/thrombotic risk, leading to a negative impact on outcomes. In geriatric populati

Nonverbal communication tool for children with severe motor and intellectual disabilities using biochemical measurement.

Children with severe motor and intellectual disabilities (SMID) find it almost impossible to communicate whether they are feeling comfortable or uncomfortable. Most of the children with SMID rely on medical care involving disposable-type life support devices such as tracheostomy tubes or gastric tubes. It is believed that the insertion and removal of these medical devices could cause extreme physical pain under certain circumstances. The authors have previously reported a hand-held type salivary amylase (AMY) activity monitor that can be used to evaluate the sympathetic nervous system. This re

Refibrillation, resuscitation and survival in out-of-hospital sudden cardiac arrest victims treated with biphasic automated external defibrillators.

PRIMARY OBJECTIVE: Defibrillation is essential for victims of sudden cardiac arrest (SCA) with ventricular fibrillation (VF), yet it does not terminate the underlying causes of VF. Prior to more definitive interventions, these same causes may result in recurrence of VF following defibrillation (refibrillation). The incidence and course of refibrillation, and its relation to patient outcomes, has not been previously described in the context of treatment of out-of-hospital SCA with biphasic waveform automated external defibrillators (AEDs). MATERIALS AND METHODS: ECGs were recovered from all sh

Health-Related Social Needs in Children With Sickle Cell Disease Are Associated With Worse Health-Related Quality of Life.

BACKGROUND: Children with sickle cell disease (SCD) face multiple acute and chronic medical complications that may impact their quality of life as reported by patients themselves. Health-related social needs (HRSNs), such as food and housing insecurity, are common in people with SCD, but the association between HRSNs and patient-reported quality-of-life outcomes in pediatric SCD patients has not been examined. METHODS: In this cross-sectional study, parents/guardians of children 5-18 completed a demographic questionnaire, an HRSN screener, and the PROMIS Pediatric Parent-Proxy 25 Profile v2.0
