How to monitor and discriminate the causes of lower limb swelling during home-based rehabilitation after total knee arthroplasty? A delphi study.

PURPOSE: Swelling in the lower limbs after total knee arthroplasty (TKA) affects surgical outcomes. Prolonged swelling requires monitoring and remote management during home-based rehabilitation. Causes of swelling vary but, so far, no indicators are available to monitor and identify causes of lower limb swelling, making it difficult to implement targeted interventions. This study aimed to find the indicators to monitor and identify the causes of lower limb swelling during home-based rehabilitation after TKA by literature research and consulting experts from various disciplines. METHODS: The D

Safety and efficacy of a novel ANGPTL4 inhibitory antibody for lipid lowering: results from phase 1 and phase 1b/2a clinical studies.

BACKGROUND: Genetic studies have established angiopoietin-related protein 4 (ANGPTL4) as a key regulator of triglyceride metabolism and a promising target to reduce atherosclerotic cardiovascular disease (ASCVD) risk beyond traditional risk factors. Human ANGPTL4 loss-of-function shows no adverse consequences and is associated with reduced triglycerides and remnant cholesterol, and a reduced risk of type 2 diabetes and ASCVD. Nonetheless, development of ANGPTL4 inhibitors has been delayed due to adverse findings in ANGPTL4-knockout mice fed a high saturated fat diet, including lipid accumulati

Claudin-18 isoform 2-specific CAR T-cell therapy (satri-cel) versus treatment of physician's choice for previously treated advanced gastric or gastro-oesophageal junction cancer (CT041-ST-01): a randomised, open-label, phase 2 trial.

BACKGROUND: Claudin-18 isoform 2 (CLDN18.2) has emerged as a promising therapeutic target in gastric or gastro-oesophageal junction cancer. Satricabtagene autoleucel (satri-cel; also known as CT041), an autologous CLDN18.2-specific chimeric antigen receptor (CAR) T-cell therapy, showed encouraging activity in previously treated patients with advanced gastric or gastro-oesophageal junction cancer in phase 1 clinical trials. In this Article, we report the primary results from the phase 2 pivotal trial (CT041-ST-01) investigating the efficacy and safety of satri-cel for gastric or gastro-oesophag

Comparative effects of mechanical and functional alignment in bilateral robotic total knee arthroplasty: a randomized controlled trial.

BACKGROUND: Functional alignment (FA) in total knee arthroplasty (TKA) can achieve soft tissue balance by fine-tuning adjustments of bony resections and component alignment with less soft tissue release. However, joint line orientation relative to the floor in the knee and ankle after TKA is not well studied. METHODS: A randomized-controlled trial was performed in 30 patients with robotic-assisted bilateral TKA using FA and mechanical alignment (MA) in the same patient. The outcome measures were as follows: (1) standing radiographic knee and ankle alignment; (2) clinical outcomes at 1, 3 and

Chat GPT vs an experienced ophthalmologist: evaluating chatbot writing performance in ophthalmology.

PURPOSE: To examine the abilities of ChatGPT in writing scientific ophthalmology introductions and to compare those abilities to experienced ophthalmologists. METHODS: OpenAI web interface was utilized to interact with and prompt ChatGPT 4 for generating the introductions for the selected papers. Consequently, each paper had two introductions-one drafted by ChatGPT and the other by the original author. Ten ophthalmology specialists with a minimal experience of more than 15 years, each representing distinct subspecialties-retina, neuro-ophthalmology, oculoplastic, glaucoma, and ocular oncology

Diagnostic utility of artificial intelligence software through non-mydriatic digital retinography in the screening of diabetic retinopathy: an overview of reviews.

OBJECTIVE: To evaluate the capability of artificial intelligence (AI) in screening for diabetic retinopathy (DR) utilizing digital retinography captured by non-mydriatic (NM) ≥45° cameras, focusing on diagnosis accuracy, effectiveness, and clinical safety. METHODS: We performed an overview of systematic reviews (SRs) up to May 2023 in Medline, Embase, CINAHL, and Web of Science. We used AMSTAR-2 tool to assess the reliability of each SR. We reported meta-analysis estimates or ranges of diagnostic performance figures. RESULTS: Out of 1336 records, ten SRs were selected, most deemed low or cri

Aflibercept 2 mg biosimilar (Tyalia)-real-world experience from IRAN (ATRIA study).

PURPOSE: To evaluate the early real-world clinical outcomes regarding safety and efficacy after administering the aflibercept 2 mg biosimilar (Tyalia, Cinnagen, Tehran, Iran). METHODS: A retrospective, uncontrolled observational study was conducted with a total of 499 Tyalia injections given in 189 eyes of 148 patients for variable indications. All patients were treated with at least one intravitreal injection of Tyalia 2 mg; 102 eyes with neovascular age-related macular degeneration (n-AMD), 67 eyes with diabetic macular oedema (DMO), and 20 eyes with retinal vein occlusion (RVO) associated

Retinal optical coherence tomography angiography (OCTA) biomarkers of cardiovascular disease: a review article.

This review article systematically assesses existing literature on studies employing retinal optical coherence tomography angiography (OCTA) metrics as surrogate biomarkers for cardiovascular disease. A comprehensive, literature review of published peer-reviewed research was conducted within PubMed utilizing the following medical subject headings (MeSH) terms: "optical coherence tomography", "cardiovascular diseases", "retina", and "retinal vessels". A total of 840 articles were reviewed and selectively filtered with ultimately 50 articles being included. This review article elucidates key fin

Predicting post-surgery change in visual acuity after successful repair of macula-off retinal detachments: findings from a large prospective UK cohort study.

OBJECTIVE: To identify risk factors affecting the change in visual acuity in successfully re-attached macula-off rhegmatogenous retinal detachment (RD) surgery. DESIGN: A cohort study, using an online database, of visual outcomes for 2770 macula-off retinal detachments that were successfully re-attached by vitrectomy and internal tamponade. The database included detailed retinal diagrams of each detachment. MAIN OUTCOME MEASURE: The change in LogMAR visual acuity from pre-operatively to post-operatively. RESULTS: Male patients accounted for 64.6% of the sample and the median age was 63 year

Specific quantification of intact lipid nanocapsules in rats using FRET: biodistribution and PBPK model development.

AIMS: One major challenge is to quantify intact nanoparticles specifically to understand the pharmacokinetics (PK) of nanomedicines. Lipid nanocapsules (LNC) carrying Förster resonance energy transfer (FRET) trackers have been previously developed, and a quantification method has been applied in blood samples. MATERIALS & METHOD: A quantification method in liver, spleen, and lungs was developed, and the biodistribution of intact FRET-LNC of 50 nm (FRET-LNC-50) in rats after intravenous injection was performed. RESULTS: FRET-LNC-50 were extracted from organs using a newly developed extraction
