A 50-year experience of male breast cancer: is outcome changing?

During the period 1933-1983, 124 men (median age 62.5 years, range 33-86 years) were treated for breast carcinoma. Median length of follow-up was 6.7 years and was complete for 93% of patients. Presenting complaints were most frequently a mass (95%) or pain (31%) while physical examination revealed the tumour to be central in 95% of patients with nipple or skin retraction in 36% and associated gynaecomastia in 12%. Twenty-seven per cent of the patients had a positive family history of breast cancer, 6% noted previous breast trauma and 7% had prior chest wall irradiation. Mean tumour size was 2

Detection of occult metastatic lobular carcinoma in axillary lymph nodes using anticytokeratin monoclonal antibodies.

To examine the importance of immunocytochemically detectable occult axillary lymph node metastases in patients with lobular carcinoma of breast, tumor registry data from 54 cases indexed as lobular carcinoma during the period 1973-82 were reviewed. Recurrences and/or deaths due to cancer were essentially confined to the group of patients with a component of invasive lobular carcinoma (ILC), therefore this subset was selected for further study. Seven of 20 cases had lymph node metastases diagnosed histologically at the time of mastectomy. Follow-up of these patients showed four dead of disease

Risk factors for immediate prosthetic postmastectomy reconstruction.

The charts of 44 women who underwent 47 immediate postmastectomy prosthetic breast reconstructions (IPMPBR) with subpectoral prostheses (long-term implant, long-term expandable implant or tissue expanders followed by long-term prosthetic placement) were retrospectively reviewed. Follow-up was from 3 to 49 months (median 18 months). Patient ages ranged from 31 to 77 years (median 42) but 82 per cent were under 60 years old. Indications for mastectomy were infiltrating cancer in 30 patients, intraductal cancer in 11, lobular carcinoma in situ in two and prophylaxis in one. There were 11 patients

Conservative treatment of early breast cancer. Long-term results of 1232 cases treated with quadrantectomy, axillary dissection, and radiotherapy.

One thousand two hundred and thirty-two women with invasive breast cancer lesions measuring less than 2 cm in diameter, clinically assessed as T1N0-1M0, were treated from 1970 to 1983 at the National Cancer Institute of Milan with quadrantectomy, axillary dissection, and radiotherapy (QUART). Pathologic evidence of lymph-nodes metastases was found in 32% of the patients. Overall survival at 5 and 10 years from surgery was 91% and 78%, respectively. The cumulative probability of survival tends to decrease with increasing tumor size: the 7-year survival rate was 84% in cases in which lesions mea

Prospective evaluation of clinical and pathologic detection of axillary metastases in patients with carcinoma of the breast.

Complete axillary dissection was performed in 287 patients undergoing modified radical mastectomy between 1984 and 1987 to identify patterns of axillary node metastases, as well as discontinuous axillary node ("skip") metastases. Positive pathologic findings were compared with preoperative clinical examinations in 266 patients and showed only 60 cases (22.6%) clinically suspicious for tumor, in contrast to 131 (45.6%) with pathologically confirmed positive lymph nodes. Axillary contents were classified level I, II, or III based on their relationship to the pectoralis minor muscle. An average o

Breast cancer: risk of axillary recurrence in node-negative patients following partial dissection of the axilla.

The estimated probability of false negative nodal staging was analysed in breast cancer patients who had lower axillary dissection in conjunction with mastectomy and who were initially characterized as node negative. The series comprised 3128 consecutive female patients with invasive mammary carcinoma treated by surgery alone according to a nationwide DBCG protocol, 77-1a (Danish Breast Cancer Co-operative Group). The median number of removed lymph nodes was four, range 0-30. Median followup period was 6.5 years, quartiles 5.5 and 8.0 years. Ipsilateral axillary recurrences appeared in 178 pat

[Stewart-Treves pseudo-syndrome caused by cutaneo-lymphatic metastases of contralateral breast carcinoma].

The pathogenesis of Stewart-Treves syndrome remains controversial: angiosarcoma or epithelial cell metastases from a mammary carcinoma? The case reported here, with clinical signs of Stewart-Treves syndrome on one side and mastectomy for carcinoma on the other side of the body, revives the debate. Case-history. The patient was an 89-year old woman whose left breast had been removed in June, 1981 for carcinoma with lymph node involvement. One year after the operation, multiple lymphadenopathy developed in her right armpit and subclavian region. In December, 1984, her right arm became swollen by

Subareolar sclerosing duct hyperplasia of the breast.

Sclerosing papillary duct hyperplasia occurs in various locations in the breast. In the nipple, the process produces a distinct clinicopathological entity referred to as nipple adenoma or florid papillomatosis (FP). Peripheral lesions have been termed radial scar and nonencapsulated sclerosing lesions. This report describes 12 women who presented with sclerosing papillary duct hyperplasia in the subareolar portion of the breast. The term subareolar sclerosing duct hyperplasia (SSDH) is proposed for this condition. The average age of the patients was 46 years (26-73 years). None of the lesions,

Associations of surgical breast cancer treatment at regional or community hospitals with patient-reported experience measures.

PURPOSE: High-volume hospitals deliver improved outcomes for patients with breast cancer, supporting referrals for breast cancer surgical care to high-volume regional facilities. This retrospective cohort study investigated how multidisciplinary cancer care trajectories and patient care experiences varied by surgical facility classification (regional vs. community as a proxy for high- vs. low-volume) and whether effects differed by rural-residence. METHODS: We used Surveillance, Epidemiology, and End Results-Consumer Assessment of Healthcare Providers and Systems (SEER-CAHPS) data for Medicar

Meeting expectations and its impact on the quality of life following surgery in breast cancer patients.

INTRODUCTION: Patients' and physicians' expectations may not always align, which can potentially impact postoperative quality of life. We evaluated the extent to which expectations of well-being and treatment-related information were fulfilled. We also examined its impact on the immediate and short-term quality of life following surgery in breast cancer patients. METHODS: We prospectively followed a cohort of non-pregnant breast cancer patients who underwent surgery and had no history of breast reconstruction, prophylactic, palliative, or intensive care admission (n = 100). Expectations and t

Fluorescence imaging with indocyanine green during breast cancer surgery and postmastectomy breast reconstruction: A double systematic review and cost analysis.

BACKGROUND: Level I evidence argues that indocyanine green fluorescence imaging, used alone, is either noninferior or superior to current gold standards of technetium Tc 99m ± blue dye for detecting sentinel lymph nodes during breast cancer surgery. Extensive literature also shows that perfusion assessments using indocyanine green fluorescence angiography reduce the rates of perfusion-related complications, including flap necrosis and necessary reoperations during breast reconstruction after mastectomy. Our objective was to examine the relative costs of using indocyanine green fluorescence ima

Economic and Health System Impact of Implementing the SOUND Trial Approach in Early-Stage Breast Cancer.

BACKGROUND: Axillary management in early-stage breast cancer is increasingly shifting toward de-escalation. While sentinel lymph node biopsy (SLNB) remains standard, evidence suggests axillary ultrasound (AUS) may safely replace SLNB in selected patients. The SOUND trial demonstrated noninferior oncologic outcomes with AUS-based staging among clinically node-negative patients with small (≤ 2 cm), unifocal invasive breast cancers and negative preoperative AUS. However, the economic and health system impact of implementing this strategy in routine practice remains unclear. PATIENTS AND METHODS:

Breast Cancer Patient Awareness and Reasons for Foregoing Breast Reconstruction. Early Findings From the ROSE (Breast Cancer and Reconstruction Outcome Survey) Study.

BACKGROUND: Breast reconstruction (BR) can improve the quality of life following breast cancer surgery, yet some patients forgo the option. Research has focused mainly on system-level barriers, while patient-reported reasons for declining reconstruction have been understudied. METHODS: The ROSE (Breast Cancer and Reconstruction Outcome Survey) study is an ongoing prospective cohort study at an academic hospital. Women ≥18 years undergoing lumpectomy or mastectomy with or without BR were included and classified by reconstruction status. Clinical data were collected, and patients who did not un

Prediction of one-year recurrence among breast cancer patients undergone surgery using artificial intelligence-based algorithms: a retrospective study on prognostic factors.

BACKGROUND AND AIM: Breast cancer is highly prevalent, with an increasing trend in women globally. Although the survival of breast cancer is relatively high, the recurrence rate is also high, demanding effective predictive solutions to breast cancer prognosis among post-operative patients. So far, Artificial intelligence algorithms integrated with various clinical data have demonstrated potential predictive capability regarding breast cancer recurrence. OBJECTIVE: This study aims to specifically conduct a predictive analysis of one-year recurrence of breast cancer by comparing and analyzing d
