Impacted maxillary canines--a perennial problem.

UNLABELLED: Ectopic eruption with impaction of maxillary permanent canine teeth is a frequently encountered clinical problem. Surgical exposure and attachment of a gold chain to impacted maxillary canines is often required in order to bring them into the dental arch and to allow good alignment. The principle of radiographic parallax is employed to determine whether the impacted canine is placed buccal or palatal to adjacent teeth. Good flap design allows adequate access to the impacted tooth. Bone removal should be adequate to free the impacted crown to its greatest circumference. An orthodont…

Severe root resorption and orthodontic treatment: clinical implications after 25 years of follow-up.

Stability and prognosis of teeth with severe root resorption is problematic in orthodontics. Because severe root resorption, when it occurs in orthodontic patients, is an iatrogenic consequence of treatment, periodic analysis for all patients is encouraged. This article reports a case of severe root resorption 25 years after the completion of orthodontic treatment. The root resorption did not exhibit significant progression and the patient had a healthy periodontal status. Orthodontic patients with sharp, pointed roots who require premolar extraction might have a greater risk of severe root re

Physical and dental manifestations of oral-facial-digital syndrome type I.

Oral-facial-digital (OFD) syndrome is the collective name of a group of rare inherited syndromes characterized by malformations of the face, oral cavity, hands and feet. OFD syndrome type I, also known as the Papillon-League-Psaume syndrome warrants our attention because early diagnosis from an odontologic viewpoint will minimize the sequalae of developing physical and dental abnormalities. The present article highlights the clinical as well as the radiographic findings and the treatment that was done of a 10-year-old girl child diagnosed with OFD I.

Surgical management of Binder's syndrome: lessons learned.

Maxillonasal dysplasia, commonly known as Binder's syndrome, is unmistakably characterized by midfacial hypoplasia and a retruded flat nose. The condition is variably expressed, and reconstruction must be tailored to the individual. Controversy still exists over the optimal age for surgery and the ideal treatment strategy. In a review of 24 patients with Binder's syndrome treated at the Chang Gung Craniofacial Centre over a period of 17 years, the authors examine the evolution of their experience treating patients with this condition. Maxillary osteotomies were rarely required and were reserve…

How to...mechanically erupt a palatal canine.

Management of ectopic permanent maxillary canines represents one of the greatest challenges to orthodontists. This paper outlines a variety of techniques and mechanics which may facilitate expedient, predictable and safe eruption of palatal canines. While each method may be useful in isolation, the varying presentations of palatal canines ensure that the ability to apply an array of techniques is essential if successful outcomes are to be consistently achieved.

Retrospective analysis of the factors influencing mesiodentes eruption.

AIM: This study aimed to evaluate the factors that predict the spontaneous eruption of mesiodens. DESIGN: From the records of 431 patients (346 boys and 85 girls, average age 8.8 years) who visited Samsung Medical Center from January 2002 to December 2006, 471 mesiodentes were reviewed. The eruption rate was investigated according to the width/length ratio, angulation, location, and shape determined from periapical or panoramic radiographs. RESULTS: The regression model showed that the width/length ratio and angulation were important determinants influencing the eruption of mesiodentes (P <

Endodontic treatment of a maxillary canine with type 3 dens invaginatus and large periradicular lesion: a case report.

Dens invaginatus type 3 is an anomaly characterized by an infolding of enamel and dentin that can extend up to the root apex and might present difficulties with respect to its diagnosis and treatment because of canal morphology. This article describes an unusual case of the maxillary canine having a type 3 dens invaginatus, necrotic pulp, and an associated large periradicular lesion treated by conventional root canal therapy. After gaining access to the pulp chamber, the primary canal and the invagination were localized, negotiated, enlarged, and filled with calcium hydroxide. Three weeks late…

Development of odontoma-like malformation in the permanent dentition caused by intrusion of primary incisor--a case report.

A case is presented of a 10-year-old girl, referred for consultation for failure of the maxillary central incisor to erupt. At 18 months, the patient underwent intrusion of the right central primary incisor, which re-erupted several months later. Radiographic examination revealed a complex odontoma-like malformation located next to the floor of the nose, away from the permanent incisors. Clinical examination revealed space loss of the corresponding permanent incisor and severe hypoplasia of the permanent right lateral incisor. Because of its deep location, it was decided not to remove the odon…

Bilateral extensive idiopathic apical root resorption in supraerupted maxillary molars: a case report.

Multiple idiopathic external root resorptions of permanent teeth are an infrequent phenomenon. A case report of a patient with multiple idiopathic external apical root resorptions is presented. This case is unusual in that extensive resorption is found bilaterally in supraerupted maxillary molar teeth. The pattern of severe resorption affecting only maxillary supraerupted teeth appears to be unique. No significant systemic, dental, or familial findings could be identified as a possible cause.

Use of the rotational path removable partial denture concept in a Kennedy Class II patient: a case report.

UNLABELLED: This case report describes the treatment of a patient with Sjogren's syndrome who was missing all of the posterior teeth in the maxillary left arch. Implants were contraindicated for this patient because of a chronic sinus infection. Various removable prosthodontic options were considered, and the patient was ultimately treated very conservatively with a rotational path removable partial denture (RPD). This approach required no tooth preparation other than bonding a resin composite cingulum rest on the maxillary left canine. The use of the rotational path RPD eliminated any unsight…

Esthetic options with two jaw surgery.

Aesthetic changes are most frequently achieved by changing the underlying bony support to the facial skeleton. Aesthetic improvement may include additional procedures to mask or enhance the skeletal results. This article deals with aesthetic changes brought about by changes in the skeletal support of the face and focuses on movement of both jaws.

Dens invaginatus type II: case report with 2-year radiographic follow-up.

Dens invaginatus is a malformation of teeth probably resulting from an infolding of the dental papilla during tooth development. Clinically, it is frequently characterized by unusual crown morphology. It is common to see peg- or barrel-shaped teeth. The present case describes dens invaginatus in a lateral maxillary incisor (Oehler type II) with a barrel-shaped crown of a 12-year-old female patient; endodontic treatment and adhesive restoration were performed. After two years the tooth showed no signs of pathosis.

Echographic evaluation of bone lesions of endodontic origin: report of two cases in the same patient.

We report on two periapical lesions of endodontic origin detected in the maxillary bone of the same patient, whose echographic examination was used to address a tentative differential diagnosis between a granuloma and a cystic lesion. The patient on whom two periapical lesions were diagnosed with clinical and conventional radiographic findings and scheduled for endodontic surgery, was also examined using echography and color power doppler at the site of the lesions. The lesions were echographically described in each case according to a scheme from a previous work. A tentative differential diag…
