Echinococcus and Taenia infection are the two most relevant forms of cestode infection in humans. These infections occur worldwide, but with higher prevalence in developing countries like India, where poor hygiene facilitates their transmission. We report a case of a middle-aged man who presented with seizures and was found to have dual worm infection including Taenia and Echinococcus. The patient was treated with steroids and albendazole followed by PAIR (puncture, aspiration, injection of scolicidal agent and reaspiration). After 6 months of follow-up, the patient is asymptomatic and

PURPOSE: To describe two cases of stellate neuroretinitis associated with posterior scleritis.
METHODS: Retrospective observational case series.
RESULTS: Two middle-aged female patients presented with unilateral decreased vision and clinical findings consistent with stellate neuroretinitis. A thorough diagnostic evaluation for known causes of neuroretinitis was negative in each case. Radiography, ultrasonography, and fluorescein angiography revealed findings consistent with a diagnosis of posterior scleritis in each patient.
CONCLUSION: This is the first report of posterior scleritis causin

BACKGROUND: Hibernoma, a benign neoplasm of the brown fat, clinically and radiologically can mimic a lipoma or liposarcoma, especially in the retroperitoneum. As treatment modalities vary, an accurate preoperative diagnosis may help the surgeon to plan treatment. Hence cytologic features of 2 cases of hibernoma are described.
CASES: Two middle-aged men diagnosed clinically as having lipomas were subjected to fine needle aspiration (FNA). Smears revealed typical hibernoma cells with a minor percentage of eosinophilic cells and mature adipocytes.
CONCLUSION: FNA is a highly sensitive and speci

Elastofibroma is an uncommon fibrous pseudotumor that usually occurs in the subscapular region of middle-aged and older adults. Since its seminal description, cases of elastofibroma or elastofibroma-like proliferations have been identified at several anatomic locations, including the foot, hand, thigh, olecranon, gastrointestinal tract, trachea, dorsal spine and eye. Involvement of the oral cavity is rare, with only four cases reported to date. Herein, we report a case of elastofibromatous change in the soft palate of a 55-year-old man and review the literature regarding pathogenesis, clinicop

INTRODUCTION: Omental infarction is a rare and often misdiagnosed clinical event with unspecific symptoms. It affects predominantly young and middle aged women.
CASE PRESENTATION: This is a case report of a 26-year-old Caucasian woman with spontaneous omental infarction two weeks after normal vaginal delivery.
CONCLUSION: Omental infarction is a differential diagnosis in the postpartum acute abdomen. As some cases of omental infarction, which are caused by torsion, can be adequately diagnosed via computed tomography, a conservative treatment strategy for patients without complications should…

Double pituitary adenomas are difficult to recognize pre-operatively as only a single mass may be appreciated on imaging. We present herein a giant prolactin-secreting pituitary adenoma in a middle-aged man that had responded partially to dopamine agonist therapy. The excised specimen demonstrated a double adenoma. The prolactin-producing one displayed the expected morphological changes resulting from medical therapy, while the other, a gonadotroph adenoma, did not. The failure of tumor shrinkage can be attributed to the presence of a double adenoma, a previously unreported cause of failure of

Biliary cystadenoma is a rare cystic tumor of the middle aged woman that usually arises in the liver or occasionally in the extrahepatic bile ducts. It has a strong potential for recurrence and for malignant transformation. The lack of specific clinical and biological features hinders diagnosis before surgery. The spontaneous rupture of a hepatobiliary cystadnoma is a very rare and potentially life-threatening complication, with only two reported cases in the English literature. We report a case with spontaneous rupture of a recurrent hepatobiliary cystadenoma in a 32 year-old woman.

Suprasellar tuberculomas are exceptionally rare. We present the case of a middle aged male who presented with generalised apathy and meningism of two weeks duration. Neuroimaging was suggestive of tuberculomas in suprasellar cistern, while endocrinological investigation showed panhypopituitarism manifesting as pituitary hypothyroidism, hypocortisolism, and hyperprolactinemia. Cerebrospinal fluid examination showed lymphocytic reaction with raised proteins. A diagnosis of suprasellar tuberculoma with panhypopituitarism was made. The patient was started on antituberculous treatment and hormone r…

A middle-aged woman presented from an outside hospital with a diagnosis of Neisseria meningitidis and meningococcemia. A nonpalpable purpuric skin rash evolved into multiple wounds, with gradual necrosis of bilateral lower and upper extremities. Throughout the course of hospitalization, the patient developed ventricular tachycardia, normocytic anemia, thrombocytosis, Clostridium difficile infection, depression, and transient right eye blindness. The finding of decreased CH50 in the complement cascade was considered as the potential cause of the meningococcemia. The subsequent ischemia and necr

Coronary artery fistulae are uncommon form of coronary anomalies, often diagnosed incidentally during coronary arteriography. Dual origin of fistulae from both coronary arteries draining into the pulmonary arterial branches is extremely rare and their natural history is unknown. In this report, we present a case of middle aged female with an incidental finding of bilateral coronary artery to pulmonary artery fistulae on coronary arteriography, which was followed clinically for 5 1/2 years with conservative treatment, remaining in NYHA class I to II with no symptoms directly related to her coro…

INTRODUCTION: The yellow nail syndrome is usually described as the combination of yellow nails with lymphoedema and often with respiratory manifestations such as pleural effusions, chronic sinusitis and bronchiectasis. The syndrome is most often seen in the middle-aged individuals.
CASE PRESENTATION: We present a 6-year-old girl with yellow nail syndrome having pansinusitis and bronchiectasis.
CONCLUSION: The components comprising the classical triad of yellow nail syndrome in children may not necessarily be present altogether. Therefore, yellow nail syndrome should be suspected in children

Rosacea is a chronic inflammatory skin disease appearing in the central area of the face of middle-aged patients. It is characterized by flushing, permanent erythema, telangiectasia, papules, pustules, and the absence of comedones. Its underlying pathophysiological mechanisms are not completely understood, although a number of hypotheses point to vascular abnormalities and infection by microorganisms such as Demodex folliculorum. Rosacea is classified into 4 subtypes, which determine the therapeutic approach based on skin care, topical antiinflammatory agents, topical and oral antibiotics and

Angioleiomyoma is a solitary form of leiomyoma that usually occurs as a painful subcutaneous lesion, with a predilection for the lower extremities of middle-aged women. In this report the case of an 11-year-old boy with angioleiomyoma mimicking giant cell tumor of the tendon sheath (GCTTS) in the palm is presented. After marginal excision of the tumor, histopathologic and immunohistochemical studies confirmed a typical case of angioleiomyoma and ruled out the preoperative diagnosis of GCTTS. There was no evidence of local recurrence at the 1-year follow-up examination.

A case of bilateral anterior glenohumeral dislocation in a middle aged horse rider was presented. The patient was an amateur rider who sustained the injury when the horse reared suddenly. The rider fell back from the saddle while holding the halter. The shoulders were dislocated by a violent traction when shoulders were in internal rotation and flexion in sagittal plane and slight abduction in coronal plane. To the best of our knowledge this case is the third glenohumeral dislocation by forward traction. A review of the literature is presented.

Elective combined operations were performed at 228 (10.96%) geriatric patients (aged from 60 to 85 years, 75.3% male, 24.7% female). It is demonstrated that more than 10% patients at specialized geriatric surgical departments require elective simultaneous operations, 86.8% of them are the surgeries of light and middle severity degree. Mini-invasive technologies and differential approach to the anesthetic management choice permit to increase contingent of geriatric patients who can treated with elective simultaneous surgical procedures, that decreases cost in 1.8 times compared with staged surg

Whipple's disease is an uncommon, multisystem disease that typically strikes middle-aged men causing abdominal pain, arthralgia and malabsorption. The characteristic patholological finding is infiltration of the small intestinal mucosa and accompanying lymph nodes with PAS positive macrophages. Similar histological findings can be found in other involved organs. Tropheryma whippleii, the causative organism, usually responds to antibiotic therapy. Polymerase chain reaction is used to confirm the diagnosis. This report summarizes, arguably, Connecticut's first documented case 50 years ago, and t

HCL typically presents in middle-aged men, and is characterized by splenomegaly and cytopenias. Hepatomegaly may be present, but it usually is not a salient feature. Peripheral adenopathy is uncommon. Other organ manifestations occur, but are unusual. patients are now presenting with a less tumor burden, as a result of earlier diagnosis. Leukocytosis/lymphocytosis should suggest HCl variant. Infectious complications, which were common in the past and the major cause of death, have become rare in the era of purine analog therapy. Extratesticular varicocele is a common clinical condition, occurring in up to 20% of the normal adult male population. It is characterized by dilatation of the veins of the spermatic cord and has well-described sonographic appearances. Intratesticular varicocele is characterized by dilated intratesticular veins and is extremely rare. It usually, but not always, occurs in association with an ipsilateral extratesticular varicocele. We describe the sonographic and colour Doppler appearances of this poorly described entity in a middle-aged man referred for evaluation of a painful left testicular s

Hemangiomas are benign, slow-growing skeletal and soft tissue tumors that commonly involve the bony spine. When found in the spine, they are usually asymptomatic, but can sometimes be associated with local pain and/or neurological deficits. We report the case of a middle-aged Indian male with multiple hemangiomas involving almost all the vertebrae contiguously from C2 to S1 vertebral levels. These were discovered incidentally when a magnetic resonance scan was performed for backache. To the best of our knowledge, such extensive involvement of the spine has not been reported previously.

Xanthogranulomatous pyelonephritis (XGP) is a rare, chronic inflammatory lesion of the kidney associated with both chronic infection and obstruction. Most common in middle-aged females, it is important to recognize in children because it may be confused with childhood renal malignancies particularly Wilms' tumor. We report a case of a 1-year old male with antenatally detected and postnatally confirmed hydronephrosis. Further investigation revealed a left incomplete duplex collecting system with obstructing lower pole stones, pelvicaliectasis and widespread foci of calcification. He underwent a
