Minimal epicondylectomy improves neurologic deficits in moderate to severe cubital tunnel syndrome.

BACKGROUND: Previous studies of minimal medial epicondylectomy for cubital tunnel syndrome included patients with mild disease, making it difficult to determine how much this procedure improved sensory and motor impairments in patients with moderate to severe disease. QUESTIONS/PURPOSES: We asked if minimal epicondylectomy improved sensory and motor impairments in patients with moderate to severe cubital tunnel syndrome. METHODS: We retrospectively reviewed 25 patients treated with minimal medial epicondylectomy for advanced cubital tunnel syndrome involving motor weakness between January 20

Early demyelination of primary A-fibers induces a rapid-onset of neuropathic pain in rat.

Some types of peripheral neuropathic pain are associated with damage to myelin rather than to axons of primary sensory neurons. It is extremely important to develop selective demyelination animal models for understanding neuropathic pain caused by demyelination. We induced a rapid-onset and reversible demyelination of peripheral A-fibers and neuropathic pain behaviors in adult rats by a single injection of cobra venom into the sciatic nerve. The relation between A-fiber demyelination and the abnormal pain behaviors was investigated using this model. Microfilament recordings revealed that cobra

Tardy palsy of descending branch of posterior interosseous nerve: sequela to plate osteosynthesis of forearm bones.

We report a case of tardy paralysis of the descending branch of the posterior interosseous nerve as a consequence of plate osteosynthesis for fracture of both bone forearms. The patient had been operated on 23 years earlier and palsy occurred after a gap of 19 years. The most probable antecedent cause of the palsy was the use of a high-profile implant. The patient was treated by removal of the plate and tendon transfer.

[Signal evaluation of continuous vagal nerve stimulation for recurrent laryngeal nerve protection in thyroid surgery].

INTRODUCTION: Newly developed vagal stimulation probes permit continuous intraoperative neuromonitoring of the recurrent laryngeal nerve during thyroid resection. A complete signal loss indicates an interrupted nerve conductance. There are no other criteria so far which may serve as warning criteria for imminent nerve function impairment. METHODS: The vagal stimulation probe V3 was used in 50 patients with thyroid resection (96 nerves at risk) and recorded during the complete operation (V3, adhesive tube electrode, bipolar needle electrode; Fa. Inomed, Teningen, Germany). The signal parameter

Cutaneous silent periods in the assessment of mild cervical spondylotic myelopathy.

STUDY DESIGN: Clinical, neuroradiologic, and neurophysiologic description of 21 patients with compressive cervical spondylotic myelopathy (CCSM). OBJECTIVE: To describe the utility of cutaneous silent periods (CSPs) for functional evaluation of mild CCSM. SUMMARY OF BACKGROUND DATA: Electroneurography, electromyography, and somatosensory and motor evoked potentials (SEPs, MEPs) are routinely used for comprehensive functional neurophysiological evaluation of CCSM. CSPs have been reported in various intramedullary spinal cord lesions, however, they have not been systematically studied in mild

Multimodality-evoked potential study of anti-aquaporin-4 antibody-positive and -negative multiple sclerosis patients.

Neuromyelitis optica (NMO) is claimed to be a distinct disease entity from multiple sclerosis (MS) because of its strong association with NMO-IgG/anti-AQP4 antibody; however, the in vivo role of the antibody remains unknown. Therefore, we aimed to clarify whether the presence of anti-AQP4 antibody is associated with any abnormalities in multimodality-evoked potentials in 111 patients with relapsing-remitting or relapsing-progressive MS, including the opticospinal form of MS, 18 of whom were seropositive for anti-AQP4 antibody. More patients with anti-AQP4 antibody showed a lack of the P100 com

Development of animal model for vasculatic neuropathy: Induction by ischemic-reperfusion in the rat femoral artery.

Ischemic-reperfusion (I/R) is common in various pathological conditions like diabetic complication, complex regional pain syndrome type II (CRPS II), necrotizing vascular occlusive disease and trauma. We have developed an animal model of ischemic-reperfusion injury induced nociceptive sensory neuropathy in rats. The model was validated after 2, 4 and 6h of ischemia followed by prolonged reperfusion. The sensory behavioral assessment revealed thermal and mechanical hyperalgesia in paw and in tail which expressed the peripheral and central neuropathic pain respectively. We observed a decrease in

Interpretation of electrodiagnostic findings in sporadic progressive muscular atrophy.

OBJECTIVE: We present the electrophysiologic data at baseline of 37 patients who were included in our prospective study on sporadic adult-onset progressive muscular atrophy (PMA). The aim was to correlate electrophysiological signs of lower motor neuron (LMN) loss with clinical signs of LMN loss, and to determine the prognostic value of the distribution of electrophysiological abnormalities in patients who presented clinically with only lower motor neuron signs. METHODS: Thirty-seven patients, who met our inclusion criteria for a prospective study on sporadic adult-onset PMA, underwent extens

Carpal tunnel syndrome: electrophysiological grading and surgical results by minimum incision open carpal tunnel release.

The safety and effectiveness of the minimum incision technique were assessed in 138 hands of 108 consecutive patients with carpal tunnel syndrome treated from April 1, 1997 to March 31, 2006. Clinical and electrophysiological examinations were conducted before and after surgical decompression. All hands were divided into early, mild, moderate, and severe groups based on preoperative electrophysiological severity. We examined the surgical outcomes of the affected hands in each group. Nocturnal or daytime dysesthesia, which had been present in 132 (96%) of the 138 hands preoperatively, was compl

Cortical and brainstem LTP-like plasticity in Huntington's disease.

Recent studies have reported abnormalities in short-term plasticity in patients with Huntington's disease (HD). However, is not known whether long-term potentiation (LTP)-like plasticity is also affected in these patients. We tested cortical and brainstem LTP-like plasticity in eight symptomatic HD patients and in 10 healthy age-matched controls. To probe motor cortex LTP-like plasticity we used paired associative stimulation (PAS), a technique that combines repetitive electric stimulation of the median nerve with subsequent transcranial magnetic stimulation (TMS) of the contralateral motor co

Pitfalls in the electrodiagnostic studies of sacral plexopathies.

This retrospective review characterizes the electrodiagnostic (EDX) features and etiologies of sacral plexopathies (SPs) and discusses difficulties in their identification. The EDX findings of 171 clinically suspected SPs were reviewed using the following criteria: reduced/absent sensory nerve action potentials (SNAPs) of the sural or superficial peroneal nerve, denervation of plexus-innervated muscles, and the absence of paraspinal denervation. Sixty cases localized unequivocally to the sacral plexus. The majority were cancer-related, followed by traumatic, idiopathic, and iatrogenic causes.

Physiologic changes of compound muscle action potentials related to voluntary contraction and muscle length in carpal tunnel syndrome.

The affect of muscle length and voluntary contraction upon compound muscle action potentials (CMAPs) in subjects with carpal tunnel syndrome (CTS) has been evaluated. Twenty-five hands in a CTS patient group and 29 hands in a normal subject control group were studied. The CMAPs from the abductor pollicis brevis induced by median nerve stimulation at the wrist were obtained for five thumb positions: neutral, abduction for shortening with and without contraction, and adduction for lengthening with and without contraction. Upon muscle shortening with relaxation, CMAP duration decreased in both gr

Sensory neuropathy in vasculitis: a clinical, pathologic, and electrophysiologic study.

BACKGROUND: Vasculitis is not usually considered as a cause of symmetric sensory neuropathy. OBJECTIVE AND METHODS: To present the clinical, pathologic, and electrophysiologic features of 17 (16%) cases of sensory neuropathy in vasculitis (SNV) among 106 cases with histologically proven vasculitic neuropathy that were collected over the last 30 years. RESULTS: In 41% of cases, SNV was found as systemic vasculitic neuropathy in association with primary vasculitic disease. The most common clinical presentation was symmetric polyneuropathy, seen in 53% of cases. The most common nerve conduction

Detection of genomic viral RNA in nerve and muscle of patients with HCV neuropathy.

BACKGROUND: Hepatitis C virus (HCV)-associated neuropathy is usually associated with mixed cryoglobulinemia (MC) and vasculitis. MC may contain viral RNA, and tissues showing vasculitis may contain intracellular HCV. Local HCV replication remains to be evidenced. OBJECTIVE: To delineate the spectrum of HCV-associated neuropathy and to assess the presence of HCV in nerve and muscle tissues. METHODS: Thirty consecutive HCV-infected patients with peripheral neuropathy were included. Genomic and replicative strands of HCV RNA were detected in both nerve and muscle biopsy samples using distinctiv

Glycinergic mIPSCs in mouse and rat brainstem auditory nuclei: modulation by ruthenium red and the role of calcium stores.

Spontaneous miniature inhibitory postsynaptic currents (mIPSCs) recorded in central neurons are usually highly variable in amplitude due to many factors such as intrinsic postsynaptic channel fluctuations at each release site, site-to-site variability between release sites, electrotonic attenuation due to variable dendritic locations of synapses, and the possibility of synchronous multivesicular release. A detailed knowledge of these factors is essential for the interpretation of mIPSC amplitude distributions and mean quantal size. We have studied glycinergic mIPSCs in two auditory brainstem n

An evaluation of gender, obesity, age and diabetes mellitus as risk factors for carpal tunnel syndrome.

OBJECTIVES: The aim of this study is to identify gender, high body mass index (BMI), age and diabetes mellitus (DM) as independent risk factors (RF) for carpal tunnel syndrome (CTS) and to analyse the strength of association of these factors, both globally and in individual subgroups. METHODS: We performed a case-control study with 791 CTS cases and 981 controls. Patients were selected from those referred to nerve conduction studies and electromyography in 3 university hospitals and two private services. We calculated the odds ratio between the two groups to analyse the RF. Possible sources o

Dorsal root sensitivity to interleukin-1 beta, interleukin-6 and tumor necrosis factor in rats.

The release of inflammatory cytokines caused by a disrupted disc may play a critical role in pain production at nerve endings, axons, and nerve cell bodies. Herniated disc tissue has been shown to release inflammatory cytokines such as interleukin-1 beta (IL-1beta), interleukin-6 (IL-6), tumor necrosis factor (TNF), and other algesic chemicals. This study was designed to characterize the effects of these proinflammatory cytokines on the somatosensory neural response at the dorsal root level in rats. It is hypothesized that their effects on nerve endings in disc and adjacent tissue contribute t

Vagal and sciatic nerve stimulation have complex, time-dependent effects on chemically-induced seizures: a controlled study.

Previous studies of the effects of electrical vagus stimulation on experimental seizures were without suitable controls or statistical validation, and ignored the potential role of vagally-induced hemodynamic depression on seizure expression. This study addresses these limitations. The effects of periodic left vagus nerve stimulation (LVNS) on chemically-induced seizures in rats were compared with control groups receiving no stimulation (NoS), left sciatic nerve stimulation (LSNS) and LVNS after pretreatment with methyl atropine (MA-LVNS). Stimulation followed a 30 s on-120 s off cycle over 13
