Talar dislocations.

From 1987-2003, 36 patients were treated for talar dislocation, 27 patients for subtalar, six for total talar, and three patients for peritalar dislocation. Luxatio pedis sub talo: We found 19 medial closed, seven lateral closed and one third degree open subtalar dislocations. Our therapeutic concept provides for immediate reduction, which is possible by closed procedure for the majority of medial dislocations. If there is a tendency to redislocation, we perform talonavicular K-wire transfixation. In the case of irreducibility, open reduction via lateral approach is the rule. The lateral dislo

Inferior Pole Scaphoid Nonunion in a 12-Year-Old Boy: Lessons on Compliance, Follow-Up, and Surgical Salvage-A Case Report.

Background and Clinical Significance: Scaphoid fractures and nonunion are uncommon in skeletally immature patients. Pediatric nonunion most often follows a missed or delayed diagnosis or failure of conservative treatment. Inferior-pole nonunion is particularly uncommon, and evidence guiding graft selection in children is limited. We report a case in which preoperative and intraoperative assessment of fragment viability supported the use of a non-vascularized graft. Case presentation: A 12-year-old boy sustained a right inferior-pole scaphoid fracture after falling onto an outstretched hand. Th

Influence of articular incongruency on coronal alignment after surgical fixation of lateral tibial plateau fractures.

PURPOSE: The surgical treatment of the tibial plateau fracture aims for anatomic reduction and rigid fixation. A residual articular step-off more than 2mm in the weight-bearing zone has a high impact on developing posttraumatic osteoarthritis. However, the correlation between malreduction and malalignment has not been studied in depth. The purpose of this study was to investigate the correlation between postoperative articular congruency in different fracture zones in sagittal plane and the whole lower limb alignment after surgical treatment of the lateral tibial plateau fractures. Treatment of displaced talar neck fractures using delayed procedures of plate fixation through dual approaches.

PURPOSE: Treatment of talar neck fractures is challenging. Various surgical approaches and fixation methods have been documented. Clinical outcomes are often dissatisfying due to inadequate reduction and fixation with high rates of complications. Obtaining satisfactory clinical outcomes with minimum complications remains a hard task for orthopaedic surgeons. METHODS: In the period from May 2007 to September 2010, a total of 31 cases with closed displaced talar neck fractures were treated surgically in our department. Injuries were classified according to the Hawkins classification modified by

Evaluation of bone healing in trabeculae structure of mandibular corpus and angulus fracture patients with fractal dimension analysis.

BACKGROUND: Fractures of the mandibular corpus and angulus are affected by different forces result from different strength of the muscles that influenced them. Therefore, it is presumed that there is a different in healing rate. Fractal dimension radiologic analysis assesses the trabecular pattern, due to it can be used to observe the healing process of bone fractures. PURPOSE: The study aimed to compare the healing process of mandibular corpus and angulus fractures with fractal dimension analysis (FD) of trabecular patterns on panoramic radiographs through ImageJ software as consideration to

Can formation of avascular necrosis really be prevented in Delbet type 2 femoral neck fractures?

BACKGROUND: Fractures of proximal femur are rarely seen in childhood. These fractures can result in serious complications such as avascular necrosis (AVN), coxa vara, and early closure of the femoral epiphysis. The aim of this study is to investigate retrospectively the Delbet type 2 fractures that are commonly seen and has high rates of AVN. METHODS: Forty-two patients whose Delbet type 2 fractures were treated in our hospital between January 2009 and June 2018 were analyzed. The patients' mechanism of fracture formation, displacement of the fracture, presence of accompanying injuries, timin

Fractures of the medial column of the distal humerus in adults.

PURPOSE: Medial column fractures of the distal humerus are uncommon in adults. Among 26 patients identified in the English language literature, only 2 had articular fragmentation. We reviewed the experience of 2 trauma centers with isolated medial column fractures to evaluate our impression that these fractures often involve complex articular fragmentation. METHODS: There were 10 men and 4 women with an average age of 61 years (range, 44-86 years). The incidence was 3% of all distal humerus fractures at one institution with a prospective fracture database. The mechanism of injury was a fall f

Antibiotic practices in non-condylar mandibular fractures: a Maxillofacial Trainee Research Collaborative (MTReC) UK-wide survey.

Non-condylar mandibular fractures are consdered 'open' fractures and as such are thought to require prophylactic antibiotics. There is no overall consensus on the optimal regimen or choice of antibiotic in the preoperative and postoperative periods due to a lack of high-quality evidence. We therefore set out to ascertain the current UK-wide practice of antibiotic prescribing for non-condylar mandibular fractures. We used a web-based online survey (Google Forms) that was disseminated via email and social media platforms to oral and maxillofacial surgery (OMFS) consultants and trainees of all gr

Bipedicled Laterally Based Pericranial Flap for Anterior Skull Base Reconstruction.

Frontal sinus fractures with posterior-wall disruption, dural injury, and cerebrospinal fluid (CSF) leakage require durable separation of the sinonasal cavity from the intracranial space. An anteriorly based pericranial flap is commonly used for this purpose, but its supraorbital and supratrochlear pedicles may be unreliable after frontal soft-tissue trauma or orbital rim fractures. A 67-year-old man sustained complex craniofacial trauma after a fall, including fractures of the anterior and posterior walls of the frontal sinus, a forehead laceration reaching the bone, CSF rhinorrhea, and midfa

Surgical Management of Pediatric Capitellar Fractures: A Case Series of Five Patients.

Capitellum fractures are rare in pediatric patients and pose unique diagnostic and management challenges due to the presence of developing ossification centers and growth plates. Timely recognition and appropriate treatment are essential to prevent long-term complications such as stiffness, avascular necrosis, or growth disturbances. This case series presents five pediatric patients with capitellum fractures, classified as Dubberley type 2A, detailing their surgical management and their postoperative outcomes. The cases were treated at a single institution over a two-year period. Fractures wer

Postoperative Intermaxillary Elastic Traction Contributes to the Stability of Absorbable Plates Fixation for Bilateral Mandibular Fracture.

The use of absorbable plates can be challenging for mandibular fractures involving bilateral dentition. Chewing and mouth opening movements may cause loosening or breakage of absorbable materials, leading to displacement of bone segments and resulting in malocclusion. The use of absorbable materials for bilateral mandibular fracture surgery itself raises concerns for surgeons. Timely intermaxillary elastic traction is essential for these patients after surgery to maintain correct occlusion. The surgical approaches were performed with intraoral mandibular sulcus incisions. During the surgery, i

Treatment of wrist stiffness through posture orthosis and active exercise: A case report.

INTRODUCTION: Wrist fractures, particularly the distal radius, can result in significant stiffness and hand dysfunction if not mobilized early. The variable immobilization period post-fracture depends on fracture type, location, stability, and surgical intervention. Inadequate early mobilization typically leads to structured stiffness, influenced by patient health, injury mechanism, joint surface involvement, associated tissue injuries, and patient motivation. CASE PRESENTATION: A 64-year-old female in good health suffered a distal radius fracture, treated with open reduction and internal fix

Management of Mandibular Condylar Sagittal Fracture Accompanied by Lateral Condylar Crest Defect.

BACKGROUND: The sagittal fracture of the mandibular condyle can be fixed with absorbable long screws. Absorbable long screws are generally inserted from the lateral crest of the condyle and are as close as possible to the medial pole of the condyle to obtain sufficient retention force. However, in clinical practice, patients with locally comminuted condylar fractures and partial defects in the lateral crest are often encountered. We validated the use of absorbable plates and long screws to fix mandibular condylar fractures with lateral crest defects, and postoperative follow-up showed good res

Physical Rehabilitation Post-surgery in a Distal Femur Fracture Post Removal of Implant.

A complicated orthopaedic injury known as a comminuted distal femur fracture occurs when the lower portion of the femur bone breaks into several pieces close to the knee joint. Distal femur fractures are more commonly caused by injuries to the supracondylar and intercondylar regions. Managing comminuted distal femur fractures is a significant clinical challenge. The age of the patient, the condition of the bone, any concomitant injuries, and the level of comminution all influence the treatment plan. Handling distal femur fractures that have comminuted presents a major clinical challenge. One o

Efficacy of Single Titanium Elastic Nail in the Treatment of Child Femur Fractures.

BACKGROUND: Femoral shaft fractures in childhood constitute an important part of emergency visits to orthopedic clinics. The aim of this study was to investigate the efficacy and reliability of a method in the treatment of simple fractures in the middle of the femoral diaphysis in patients with multiple injuries or in the treatment of cases with a medullary canal that is too narrow for two nails, with a single stretch nail thicker than half the diameter of the canal. METHODS: Between July 2002 and November 2006, examinations were made of 11 femoral fractures in 11 patients who were admitted t
