Investigation of the pentamidine efficacy against myotonic dystrophy type 1 exploiting polymeric- and lipid-based nanoparticles.

Myotonic dystrophy type 1 (DM1) is a genetic disorder caused by a mutation in the DMPK gene, in which an expanded CTG trinucleotide is transcribed into abnormal mRNA that accumulates in nuclear foci. These foci sequester crucial splicing factors, causing a strong mis-regulation of downstream target genes. Consequently, DM1 causes dysfunctions of multiple organs, including skeletal muscle. Currently, no cure exists for DM1, although significant efforts are underway to develop novel therapeutic strategies. Pentamidine (PTM), a drug currently used as an antiparasitic agent, has been shown to redu

Dose-dependent effects of methylene blue on hemodynamics, cytokines, oxidative stress, and organ dysfunction in a rat model of CLP-induced sepsis: An experimental study.

BACKGROUND: Septic shock involves excessive nitric oxide (NO) production and overactivation of the NO/sGC/cGMP pathway, leading to severe vasodilation and microvascular dysfunction. Methylene blue (MB), an inhibitor of this pathway, has been proposed as a catecholamine-sparing adjunct, yet optimal therapeutic dosing remains unclear. This study investigated the dose-dependent effects of MB in a rat model of polymicrobial sepsis. METHODS: Fifty female Wistar rats were randomized into control, CLP-induced sepsis, or CLP+MB groups (10, 50, or 100 mg/kg, i.v.). Sepsis was induced via cecal ligatio

Comparative effectiveness of intravenous versus inhalational antibiotic administration for the management of Acinetobacter pneumonia: A randomized controlled trial.

BACKGROUND: Aerosol antibiotics directly target the respiratory tract, bypassing the poor lung penetration of many intravenous antibiotics. However, they seem like they could be useful for treating ventilator-associated pneumonia (VAP); not enough research has been done on how well inhaled and intravenous treatments work. The existing literature does not give definitive information regarding which method yields superior therapeutic outcomes. OBJECTIVE: To evaluate the therapeutic effects of intravenous versus inhaled colistin in treating Acinetobacter VAP in severe traumatic brain injury pati

Adjunctive Atorvastatin for Sepsis in the Acute Medical Unit: A Randomized Controlled Trial.

BACKGROUND: Sepsis, a devastating syndrome of organ dysfunction triggered by a dysregulated host response to infection, remains a leading cause of global mortality. Statins, renowned for lipid-lowering, also exhibit potent anti-inflammatory and endothelial-stabilizing properties, offering a theoretical advantage in the septic milieu. This study investigated whether adjunctive atorvastatin could improve survival and modulate key sepsis-related outcomes. METHODS: In this open-label, randomized controlled trial conducted in the acute medical unit of a tertiary academic center, adult patients wit

Signatures of the Systemic Effects of a Snake Venom and Antivenom: Multiomics Profiling of the Kidney Pathology.

Animal venoms comprise many toxins that work in concert to break apart the robust homeostatic systems of prey organisms. Conversely, prey organisms actively antagonize each step of envenoming, which displays a complex kinetics involving important changes at molecular, cell, tissue, and organism levels. In this study we explored the mammalian host response to envenoming using proteomics/N-terminomics and phosphoproteomics approaches to evaluate the in vivo effects of Bothrops jararaca venom in the mouse kidney after injection in the thigh muscle (1.6 mg/kg), mimicking a snakebite, and the impac

Disseminated Varicella-Zoster Virus Infection with Internal Organ Involvement: A Scoping Review of 156 Cases.

Visceral disseminated varicella-zoster virus infection (VD-VZV) involves the hematogenous spread of VZV from the skin to the internal organs. Though rare, it is potentially life-threatening, predominantly affecting immunocompromised individuals. Diagnosis is often delayed due to nonspecific symptoms mimicking other viral illnesses. While the vesicular rash is a hallmark sign, it is absent in approximately 5% of cases. Visceral involvement may precede cutaneous lesions, complicate early recognition, and increase the risk of severe complications. This scoping review screened 594 articles of whic

Inhibition of circulating dipeptidyl-peptidase 3 by procizumab in experimental septic shock reduces catecholamine exposure and myocardial injury.

BACKGROUND: Dipeptidyl peptidase 3 (DPP3) is a ubiquitous cytosolic enzyme released into the bloodstream after tissue injury, that can degrade angiotensin II. High concentrations of circulating DPP3 (cDPP3) have been associated with worse outcomes during sepsis. The aim of this study was to assess the effect of Procizumab (PCZ), a monoclonal antibody that neutralizes cDPP3, in an experimental model of septic shock. METHODS: In this randomized, open-label, controlled study, 16 anesthetized and mechanically ventilated pigs with peritonitis were randomized to receive PCZ or standard treatment wh

Liver abscess and splenic infarction due to Yersinia pseudotuberculosis bloodstream infection: a case report.

BACKGROUND: Yersinia pseudotuberculosis is an emerging zoonotic intestinal pathogen primarily transmitted through contaminated food and water. Infections caused by Yersinia pseudotuberculosis are typically self-limiting, often presenting as gastroenteritis or "pseudoappendicitis," which is characterized by fever and abdominal pain. Although bloodstream infections with Yersinia pseudotuberculosis are rare, they can lead to multiple distant sites of infection, including abscesses in the liver, spleen, and kidneys, as well as in the hip and knee joints, particularly in individuals with underlying

Up to an Hour of Donor Resuscitation Does Not Affect Pediatric Heart Transplantation Survival.

BACKGROUND: In pediatric heart transplantation, surgeons historically avoided donors requiring cardiopulmonary resuscitation (CPR), despite evidence that donor CPR does not change posttransplant survival (PTS). This study sought to determine whether CPR duration affects PTS. METHODS: All potential brain-dead donors aged <40 years from 2001 to 2021 consented for heart procurement were identified in the United Network for Organ Sharing database (n = 54,671). Organ acceptance was compared by CPR administration and duration. All recipients aged <18 years with donor CPR data were then identified (

Interleukin-6 blockade, a potential adjunct therapy for post-burn hypermetabolism.

Severe burns remain a leading cause of death and disability worldwide. Despite advances in patient care, the excessive and uncontrolled hypermetabolic stress response induced by this trauma inevitably affects every organ system causing substantial morbidity and mortality. Recent evidence suggests interleukin-6 (IL-6) is a major culprit underlying post-burn hypermetabolism. Indeed, genetic deletion of IL-6 alleviates various complications associated with poor clinical outcomes including the adverse remodeling of adipose tissue, cachexia and hepatic steatosis. Thus, pharmacological blockade of I

Wharton's jelly-derived mesenchymal stem cells attenuate sepsis-induced organ injury partially via cholinergic anti-inflammatory pathway activation.

Sepsis induces organ dysfunction due to overexpression of the inflammatory host response, resulting in cardiopulmonary and autonomic dysfunction, thus increasing the associated morbidity and mortality. Wharton's jelly-derived mesenchymal stem cells (WJ-MSCs) express genes and secrete factors with anti-inflammatory properties, neurological and immunological protection, as well as improve survival in experimental sepsis. The cholinergic anti-inflammatory pathway (CAP) is mediated by α7-nicotinic acetylcholine receptors (α7nAChRs), which play an important role in the control of systemic inflammat

Antimicrobial Resistance Surveillance and New Drug Development.

Surveillance represents an important informational tool for planning actions to monitor emerging antimicrobial resistance. Antimicrobial resistance surveillance (ARS) programs may have many different designs and can be grouped in 2 major categories based on their main objectives: (1) public health ARS programs and (2) industry-sponsored/product-oriented ARS programs. In general, public health ARS programs predominantly focus on health care and infection control, whereas industry ARS programs focus on an investigational or recently approved molecule(s). We reviewed the main characteristics of i

Silica nanoparticles trigger hepatic lipid-metabolism disorder in vivo and in vitro.

BACKGROUND: As a promising nanocarrier in biomedical fields, silica nanoparticles (SiNPs) could transfer from the circulatory system to multiple organs. Among these, blood-liver molecular exchange is a critical factor in biological response to NPs. However, the potential effect of SiNPs on hepatic lipid metabolism is unclear. In this study, we employed three models to attempt discover whether and how SiNPs disturb hepatic lipid metabolism in vivo and in vitro. METHODS: Firstly we used ICR mice models to evaulated the effects of SiNPs on the serum and hepatic lipid levels through repeated intr
