Demonstration of a widely distributed atrial pacemaker complex in the human heart.

Atrial depolarization was analyzed in 14 patients with the Wolff-Parkinson-White syndrome undergoing surgery to ablate accessory atrioventricular pathways associated with tachyarrhythmias. Bipolar potentials were recorded simultaneously from 156 atrial epicardial electrodes arranged in three templates to fit the anterior and posterior aspects of both atria. Spontaneous or sinus rhythms were recorded, as were atrial escape rhythms after overdrive pacing at rates of 150 and 200 beats/min. Atrial activation maps revealed different patterns of impulse initiation varying from typical unifocal sinus

Willingness to participate in cardiac telerehabilitation: results from semi-structured interviews.

AIMS: Cardiac rehabilitation (CR) is indicated in patients with cardiovascular disease but participation rates remain low. Telerehabilitation (TR) is often proposed as a solution. While many trials have investigated TR, few have studied participation rates in conventional CR non-participants. The aim of this study was to identify the percentage of patients that would be willing to participate in a TR programme to identify the main perceived barriers and facilitators for participating in TR. METHODS AND RESULTS: Two groups of patients were recruited: CR non-participants and CR participants. Se

Caregiver perceptions of telemedicine in developmental and epileptic encephalopathies: a nationwide Spanish survey.

INTRODUCTION: Developmental and epileptic encephalopathies (DEEs) require intensive and multidisciplinary care. Telemedicine has emerged as a potential tool to improve access and reduce burden for caregivers and healthcare professionals (HCPs), but its acceptance and perceived value in severe epilepsies among caregivers and HCPs remain unclear. METHODS: Two online surveys (August-November 2024) targeted caregivers of Spanish patients with DEEs and HCPs treating patients with DEEs practicing in Spain. Due to low HCP response (n = 18, patients' age: 0-54 years old), only caregiver data (n = 95)

Rhabdomyolysis and Acute Kidney Injury after the New York City Marathon.

UNLABELLED: <p>Background: The 2022 NYC Marathon was the hottest on record. At several hospitals near the race's finish, a larger volume of heat stress-related kidney injury was observed. We sought to examine the incidence of post-marathon rhabdomyolysis observed with respect to race day temperature and humidity. METHODS: All charts of patients who had a creatine phosphokinase (CPK) level >5,000 U/L on the date of the marathon and subsequent 4 days for the years 2017-2023 at four NYC hospitals were reviewed. Patients were included for analysis if marathon participation was indicated in the ch

Implicit Bias and Microaggressions in the Care of People With Prolonged Disorders of Consciousness: Bridging Medicine, Disability Studies, and Family Advocacy.

BACKGROUND: Prolonged Disorders of Consciousness (PDOC), including unresponsive wakefulness syndrome and minimally conscious state, present persistent challenges in clinical assessment and long-term care. Although behavioral observation remains the primary basis for detecting signs of consciousness, interpretation of such observations often depends on clinical judgment and contextual information provided by caregivers. In some cases, family caregivers may detect subtle signs of recovery in individuals with PDOC; however, these observations are not always incorporated into clinical assessment o

The Oral Health Management in Patients With Fixed Orthodontic Treatment: An Evidence-Based Integrated Review.

INTRODUCTION: More and more patients with malocclusion are choosing fixed orthodontics to improve their chewing ability, maintain dental health and aesthetics. However, inappropriate timing of treatment, lack of management programmes during treatment and neglect of follow-up strategies after removal of appliances can lead to a succession of oral health problems. Therefore, oral health management throughout the patient's treatment period is necessary. This review systematically synthesises published evidence on oral health management in fixed orthodontic treatment patients. METHODS: Systematic

Implications of the CMS Ambulatory Specialty Model for Low Back Pain for Interventional Pain Management: The Good, The Bad, And The Ugly.

Since the enactment of the Affordable Care Act (ACA) in 2010, the Centers for Medicare & Medicaid Services (CMS) have embraced value-based care as the organizing principle of national payment reforms. Yet, after more than a decade, the value-based movement has, at best, generated modest returns and at worst increased costs on patients and negatively impacted access. Pursuant to its authority under the ACA, CMS proposed the Ambulatory Specialty Model (ASM) in the CY 2026 Physician Fee Schedule rulemaking and subsequently finalized the model, establishing the agency's first mandatory alternative
