A 61-year-old man presented complaining of pollakisuria and nocturia. A plain radiograph of his kidney, ureter and bladder and intravenous urography revealed numerous calculi in the upper kidney of his left renal pelvis and ureterocele. A transurethral incision of ureterocele (TUI-ureterocele) and extracorporeal shock wave lithotripsy were performed. On TUI-ureterocele, the many calculi were found to be almost the same size and spherical in form. The postoperative clinical course was uneventful.

Percutaneous approaches to upper tract urothelial cancers have been performed in patients unsuitable for radical nephroureterectomy. We present the case of an 82-year-old man with significant comorbidities including dependency on a cardiac pacemaker. Without deactivating the pacemaker, we used bipolar cautery to percutaneously resect a large upper tract urothelial tumor in the renal pelvis. Bipolar cautery is a suitable method of percutaneous or transurethral resection in patients who are pacemaker dependent.

While most BK virus infections are asymptomatic, immunosuppression has been associated with BK virus reactivation and impaired graft function or ureteric ulceration in renal transplant patients and hemorrhagic cystitis in bone marrow transplant patients. Oncogenicity is also postulated and this is the first report of a child with a carcinoma of the donor renal pelvis following BK virus allograft nephropathy. Removal of the primary tumor and cessation of immunosuppression led to regression of secondary tumors and a return to health.

The authors report a case of fibro-epithelial polyp in a young woman with Turner syndrome with features of a right ureteropelvic junction syndrome in a horseshoe kidney. The patient presented a history of chronic low back pain associated with macroscopic haematuria leading to various complementary investigations including ureteroscopy. The endoscopic appearance of the tumour suggested the diagnosis despite the presence of atypical renal pelvis cytology. Pyeloplasty and proximal ureterectomy allowed confirmation and treatment of this rare lesion.

Congenital malformations of the spleen are rare. We report 3 cases of wandering spleen presented as abdominal or pelvi-abdominal mass. Two patients were suffering from chronic lower abdominal pain with thrombosed splenic pedicle and the third patient had an acute abdomen. All patients underwent splenectomies. Abdominal ultrasound, computerized tomography, Doppler ultrasound, and radioisotope studies were used to confirm the diagnosis. The clinical, diagnostic and treatment modalities are discussed.

Over the last few years, laparoscopy has become more common in urology. Nevertheless only a few centres carry out more than 40 laparoscopies a year. A reduction to two standard techniques has led to the simplification and time optimisation in preparation for surgery and thus to an increase in quality. Two standard positionings and trocar placements are described for all important urological laparoscopies. At the same time, operations involving the small pelvis and the retroperitoneum are differentiated.

We report a case of hydatid cyst of the pelvis in a 36-year-old woman presented with right adnexal cystic mass with similar cystic lesions in the liver. Laparatomy revealed a right paraovarian cystic mass densely adhered to the uterus, to the pelvic side wall, and to the right fallopian tube. Histopathological examination of the cyst wall showed the cuticular layer of the cyst. Cystic liver lesion was later proved to be hepatic hemangioma by magnetic resonance imaging.

We report two cases of Amyand's hernia, which is the development of acute appendicitis within an inguinal hernia. Both patients were clinically thought to have incarcerated inguinal hernias, but were correctly prospectively diagnosed as having Amyand's hernia on the basis of preoperative computed tomography (CT) examinations. Our cases again show the utility of CT of the acute abdomen and pelvis in revealing a previously unsuspected diagnosis and rapidly triaging patients to the appropriate management.

Bilateral nephrectomy for transitional cell carcinoma of the renal pelvis is a rare procedure. We report two cases of bilateral nephrectomy for transitional cell carcinoma. Both patients were placed on dialysis. A review of the literature revealed three other cases of transitional cell carcinoma treated with bilateral nephrectomies. Based on our experience and the review of current literature, we recommend a waiting period of 2 years on dialysis post-operatively before proceeding to transplantation.

The novel heptadecapeptide opioid, nociceptin, produced a concentration-dependent (EC50 28 nM) suppression of the inotropic response of the guinea-pig isolated renal pelvis to electrical stimulation, a response mediated by release of tachykinins from sensory nerves. Nociceptin did not affect the response to neurokinin A, indicating a prejunctional site of action on tachykininergic nerves. The effect of nociceptin was unchanged in the presence of the mu, delta and kappa opioid receptor antagonists, naloxone, naltrindole and nor-binaltorphimine.

Nine patients with history of hydatid disease have been examined by CT. Localization of the hydatid cysts in the pelvis was established by anatomical criteria. Occasionally, the transverse plane can be confusing for the precise localization of a lesion. A central location, close to the boundaries of the bladder and rectum, can define peritoneal location. Further posterolateral retrovesical location can be considered retroperitoneal. Using these criteria, 8 cysts were situated within the peritoneum and 1 within the retroperitoneum.

One-hundred newborn children at high risk of hip instability were prospectively assessed clinically and by ultrasound. The decision to treat was based only on the clinical examination. At the age of three months all the children were evaluated clinically and with an anteroposterior radiograph of the pelvis. None of the standard ultrasound measurements of acetabular depth and femoral head cover correlated with the outcome at three months. Dynamic assessment of stability was the only ultrasound technique that had a significant relation with outcome.

Twenty eight women with suspicion for extrauterine pregnancy were examined by a vaginal transducer. Extrauterine pregnancy was diagnosed in 18 of them, which was confirmed at the operation. There were no echographic data for tubal pregnancy in 10 of the women. Their further follow-up rejected the presence of extrauterine pregnancy. Three tubal pregnancies were discovered by routine monitoring of ovarian cycles. The immediate proximity of the vaginal transducer to the examined organs in the pelvis gives a possibility for exact topical diagnosis.

Flexible, passively deflectable fiberoptic ureteroscopes were used in 18 selected patients. The flexible endoscope must be stabilized with a cystoscope, flexible introducer sheath, or guide wire for placement through the urethra and bladder into the ureter. Irrigation through the instrument is beneficial in clearing the visual field and distending the lumen. These instruments can visualize the lumen of the ureter, and the medial and superior portion of the renal pelvis and intrarenal collecting systems. Their diagnostic value is limited to selected clinical situations.

Endometriosis affects 5-20% of menstruating women and occurs primarily within the pelvis. Although histologically benign, ectopic endometrial tissue possesses the ability to grow, infiltrate and disseminate with life threatening results/consequences. This is illustrated by a case of asymptomatic endometriosis involving the ureters and rectum. The importance of evaluating the integrity of the urinary tract in endometriosis, especially cases involving the uterosacral ligaments, is stressed. Appropriate therapy followed by periodic surveillance for progression of the disease is discussed.

A technique for performing antegrade pyelography during nephrotomy is described--puncture with a 21-gauge, 20-cm Cope needle, placement of a 0.018-inch SMG mandril "coat-hanger"-shaped guidewire, with the needle exchanged for a 3-French multi-sidehole dilator. Use of the 3-French dilator provides more security and less trauma to the renal pelvis; nephrotomy time is also curtailed. In 25 patients the technique was used successfully with 2 minor complications.

The non-ionic and low-osmotic pressure contrast media Iopamilon 370 was used on 20 outpatients who had not been found to have abnormal renal function. The results were compared with those obtained with Urographin (76%), currently used at our department, on 13 of the 20 patients. The nephrograms and renal pelvis obtained using Iopamilon 370 tended to have a clearer contrast than those obtained with Urographin. No side effects were produced.

Computed tomography (CT) has dramatically changed the imaging of pediatric oncologic disease. CT precisely displays and characterizes cross-sectional anatomic pathology. This provides important information for diagnosing and staging tumors in infants and children. The principles, techniques, and indications for computed tomography in the evaluation of pediatric extracranial tumors are reviewed in this article. Selected applications of computed tomography for pediatric tumors of the mediastinum, chest wall, lung parenchyma, abdomen, pelvis, and extremities are discussed and illustrated.

133 patients with inflammatory or tumorous metastases of the thoracic spine were operated upon within the last 11 years. Radical removal of inflammatory lesions with consecutive corticalis-spongiosa-plasty, taken from the christa pelvis, and chemotherapy are a therapeutical unit. Excision of the tumor with supplement bone grafting or "Pallacosplombe" and stabilization according to the principals of osteosynthesis are rewarding in individual cases. Four out of nine tumor patients survived two years after surgery.

Endometriosis is a chronic estrogen-dependent disorder that is generally localized in the pelvis affecting up to 10 % of women in the reproductive age. Extra-pelvic locations are estimated to 9 % of endometriosis women. However, the spread of the disease to parenchymal organs such as the liver is rare. Moreover, malignant transformation of hepatic endometriosis is exceptional. Here, we present a case of hepatic clear cell carcinoma developed on endometriosis lesion in a patient with prior surgery for deep endometriosis with colorectal involvement.
