Study with the Doppler method has been associated with "cavernometrics" and "cavernography" in erection pathologies for some time. Successful treatment of a case of unconsummated marriage involved the expertise of a urologist, gynecologist, and psychologist working together in an interdisciplinary clinic to treat sexual dysfunction. Total duration of treatment was 18 months, and components included surgical correction of congenital chordee, surgical excision of a hymenal remnant blocking the vaginal introitus, laser treatment of endometriosis, and sex therapy evaluation and follow-up totalling five sessions for both partners. At the end of treatment the couple was having pleasurable sexual intercourse.

We report a case of nonischemic priapism in an 11-year-old boy that occurred after blunt perineal trauma. Color flow Doppler sonography ruled out ischemic priapism and proved itself valuable as part of the emergency diagnostic evaluation of priapism in childhood.

The onlay island flap urethroplasty is useful in patients with distal, mid shaft and proximal hypospadias who have a well developed urethral plate and exhibit little or no curvature after release of chordee. The technique described has 2 main features: 1) use of the entire inner prepuce of the foreskin facilitates its mobilization and protects the vascular pedicle, and 2) additional soft tissue coverage is gained by removing the excess preputial mucosa not used for the neourethra. We have experienced 2 cases of intracavernous induration after injuries to the external genital parts caused by a motorcycle tank. Case 1. A 28-year-old male was admitted to our department complaining of painless indurations of the penile radix. Cavernosography showed segmental filling defect in left corpus cavernosum. The intracavernous injection of papaverine for diagnostic purposes in the impotent subject can facilitate the velocimetric exploration of the penile arteries and may induce a rigid erection which may have a therapeutic application. The external meatus anomalies should be diagnosed prior to the MAGPI in coronal hypospadias, but should also be looked for if a flap technique is considered in distal penile cases. The incidence of hypoplasia in the perimeatal area has been found to be 19% and 12% in coronal and distal penile hypospadias, respectively. Hypoplastic meatus prevents safe elevation of the skin flap, and needs its incision or wedge excision before the flap has been outlined.

Five cases of cysts of the tunica albuginea and two cases of cysts of the testicular parenchyma are described. The cysts of the albuginea were located in the anterior, lateral, and inferior faces of the testis (distant from the epididymis) and covered by a thin albuginea. In patients with erectile dysfunction (ED), the diagnostic procedure depends on the efficiency of the method and the implications of the results for treatment. The procedures to be considered can be integrated into a flowchart. Following screening by means of intracavernous drug injection the etiology can be classified with an adequate degree of certainty into one of three main groups. A preliminary diagnosis of nonvascular ED can be confirmed by psychological (interview, psychometric tests) and neurophysiological investigations. Of 400 patients with erectile lesions Doppler flowmetry in combination with nitroglycerin stimulation showed a decreased penile arterial blood flow in 42 males. Dynamic cavernosography and cavernosometry showed a concomitant increase of venous drainage in 38 of them. Most researchers believe that hypospadias arises from malformation of the penile urethra. However, this concept has been recently rejected, and it has been suggested that the opening of the urethra is "pushed forward" by growth of the perineum. This report is based on 7 children with balanitis xerotica obliterans. Of these patients 5 had stenosis of the urethral meatus that required meatotomy and the postoperative results were good. The frequency of balanitis xerotica obliterans in children at our center, as well as the findings of other authors, suggests that possibly more cases would be diagnosed during infancy if all dried foreskin were examined systematically.
