Twenty-three year-old with pleuritic chest pain.

CLINICAL INTRODUCTION: A 23-year-old woman followed at another medical centre for congenital heart disease (CHD) presented to our emergency clinic with 3 weeks of bilateral pleuritic chest pain. She returned from holiday in Greece 6 weeks earlier where a tattoo and nasal piercing had been performed. There was no history of night sweats or fever. Her temperature was 37.5°C, heart rate 120 beats/min, oxygen saturations 94% on room air and blood pressure 110/74. Her chest was clear and there was systolic murmur on auscultation. The chest radiograph showed peripheral bilateral lower zone atelectasi

[Treatment of Acute and Chronic Ruptures of the Extensor Mechanism of the Knee Joints].

Ruptures of the extensor mechanism of the knee joint are rare but disabling injuries. Injuries occur on the basis of degeneration or acute trauma. The understanding of anatomy and biomechanical function plays an essential role for choosing the right diagnostic methods and initiating successful treatment. Besides clinical examination ultrasound is a cost efficient primary diagnostic measure. Additional MRI can be beneficial for partial tears or chronic ruptures. Non-operative treatment is suitable for minor partial ruptures with sustained function. The most common methods for reattachment of th

Paradoxical Examination and Imaging Findings in a Case of Paratesticular Cutaneous Angioyxoma.

Angiomyxomas are rare soft tissue neoplasms rich in myxoid matrix and blood vessels that typically present as nodules on the trunk, head, and limbs in adults. They are classified as either cutaneous or aggressive based on histological findings and extent of local growth. There are less than 150 reported cases in the literature. In this case report we describe the first paratesticular case of a cutaneous angiomyxoma in a pediatric patient, who presented with a transilluminating paratesticular mass consistent with a hydrocele, but showed solid and vascular features on ultrasound.

[Initial management of traumatic ventral shoulder dislocation].

BACKGROUND: In the current literature a consensus on the specific management of primary anterior traumatic shoulder instability has not been reached. While the steps of the initial diagnostic and therapeutic procedures are mostly well-defined, a variety of factors need to be considered for the planning of further treatment. OBJECTIVE: This article aims at giving an overview of the essential aspects of the initial management in the rescue center, clinical and radiological diagnostic procedures and the subsequent treatment options. MATERIAL AND METHODS: The content of this article is based on

Predicting the effect of treatment in paediatric OSA by clinical examination and functional respiratory imaging.

OBJECTIVE: The aim of this study was to investigate whether functional respiratory imaging (FRI) or clinical examination could predict treatment outcome for obstructive sleep apnea (OSA) in normal-weight, non-syndromic children. METHODS: Normal weight children diagnosed with OSA by polysomnography were prospectively included. All children got a thorough evaluation and an ultra-low dose computed tomography scan of the upper airway (UA). A 3-D reconstruction was built combined with computational fluid dynamics for FRI. Decisions on the need and type of surgery were based upon findings during dr

Geriatric Anorexia Nervosa.

Eating disorders are not commonly diagnosed in individuals aged >50 years, yet they are associated with significant psychiatric comorbidities and overall morbidity. Anorexia nervosa is the most common eating disorder among this age group, and women are affected most often. We present the fatal case of a 66-year old woman with severe malnutrition and newly diagnosed anorexia nervosa. Inpatient refeeding was unsuccessful, and she succumbed to multisystem organ failure. The timely recognition of eating disorders among older people is important for family physicians who care for patients across th

Assessment of the Postural Stability of Female and Male Athletes.

OBJECTIVE: Postural stability is often affected by sport-related injuries and subsequently evaluated during postinjury examinations. Intrinsic factors, however, may also affect postural control. We sought to compare the postural control of female and male athletes as measured simultaneously by (1) the modified balance error scoring system (mBESS) and (2) a video-force plate system. DESIGN: Cross-sectional study. SETTING: Sports injury prevention center. PARTICIPANTS: Pediatric, adolescent, and young adult athletes who performed mBESS during an injury prevention evaluation. Blunt chest trauma: a clinical chameleon.

The incidence of blunt chest trauma (BCT) is greater than 15% of all trauma admissions to the emergency departments worldwide and is the second leading cause of death after head injury in motor vehicle accidents. The mortality due to BCT is inhomogeneously described ranging from 9% to 60%. BCT is commonly caused by a sudden high-speed deceleration trauma to the anterior chest, leading to a compression of the thorax. All thoracic structures might be injured as a result of the trauma. Complex cardiac arrhythmia, heart murmurs, hypotension, angina-like chest pain, respiratory insufficiency or dis

Equinus and Lengthening Techniques.

Equinus is linked to most lower extremity biomechanically related disorders. Defining equinus as ankle joint dorsiflexion less than 5° of dorsiflexion with the knee extended is the basis for evaluation and management of the deformity. Consistent evaluation methodology using a goniometer with the subtalar joint in neutral position and midtarsal joint supinated while dorsiflexing the ankle with knee extended provides a consistent clinical examination. For equinus deformity with an associated disorder, comprehensive treatment mandates treatment of the equinus deformity. Surgical treatment of equi

Nurse-midwives' ability to diagnose acute third- and fourth-degree obstetric lacerations in western Kenya.

BACKGROUND: Obstetric fistula devastates the lives of women and is found most commonly among the poor in resource-limited settings. Unrepaired third- and fourth-degree perineal lacerations have been shown to be the source of approximately one-third of the fistula burden in fistula camps in Kenya. In this study, we assessed potential barriers to accurate identification by Kenyan nurse-midwives of these complex perineal lacerations in postpartum women. METHODS: Nurse-midwife trainers from each of the seven sub-counties of Siaya County, Kenya were assessed in their ability to accurately identify

Leg pain location and neurological signs relate to outcomes in primary care patients with low back pain.

BACKGROUND: Low back pain (LBP) patients with related leg pain and signs of nerve root involvement are considered to have a worse prognosis than patients with LBP alone. However, it is unclear whether leg pain location above or below the knee and the presence of neurological signs are important in primary care patients. The objectives of this study were to explore whether the four Quebec Task Force categories (QTFC) based on the location of pain and on neurological signs have different characteristics at the time of care seeking, whether these QTFC are associated with outcome, and if so whethe

Ultrasound entheseal abnormalities at the distal interphalangeal joints and clinical nail involvement in patients with psoriasis and psoriatic arthritis, supporting the nail-enthesitis theory.

OBJECTIVE: It has been shown that nail involvement in psoriasis is associated with systemic enthesopathy. Our objective was to evaluate the association of nail involvement and enthesopathy at distal interphalangeal joint (DIP) level in psoriasis (PsO) and psoriatic arthritis (PsA) patients. METHODS: Consecutive patients (54 PsO and 56 PsA) seen at the outpatients clinic in this cross-sectional study were included. All patients underwent both clinical and ultrasound (US) assessment on the same day. RESULTS: US revealed enthesopathy in at least 1 DIP joint in 9 patients with PsO (17%, 95% CI:

[Definition, primary examination and differential diagnostics in acute dyspnea].

The topic of acute dyspnea is presented in two separate articles. This first part deals with the definition and pathophysiology of dyspnea as well as important considerations on the history of the present illness, physical examination, initial therapy and differential diagnostic considerations. The second part covers relevant diagnostic investigations and principles for the initial management. The causes, consequences and perception of acute dyspnea can be very different. The adult patient with acute dyspnea presents difficult challenges in the diagnosis and management. The emergency clinician…
