Update on extraesophageal manifestations of gastroesophageal reflux.

PURPOSE OF REVIEW: Symptoms/complications related to extraesophageal reflux (EER) are increasingly prevalent presentations and pose significant challenges for clinicians. We summarize and discuss clinical advances and developments in pathophysiology, testing and treatment algorithms of upper/lower airway manifestations of EER. RECENT FINDINGS: Growing evidence supports likely multifactorial causes of laryngeal symptoms, including EER, oropharyngeal pathologies, allergic conditions, and cognitive-affective processes (brain-larynx interaction). Diagnostic paradigm for laryngopharyngeal reflux (

Investigation of missense mutation-related type 1 diabetes mellitus through integrating genomic databases and bioinformatic approach.

Though genes are already known to be responsible for type 1 diabetes mellitus (T1DM), the knowledge of missense mutation of that disease gene has still to be under covered. A genomic database and a bioinformatics-based approach are integrated in the present study in order to address this issue. Initially, nine variants associated with T1DM were retrieved from the GWAS catalogue. Different genomic algorithms such as PolyPhen2.0, SNPs and GTEx analyser programs were used to study the structural and functional effects of these mutations. Subsequently, SNPnexus was also employed to understand the

Identification of Active Molecules against Thrombocytopenia through Machine Learning.

Thrombocytopenia, which is associated with thrombopoietin (TPO) deficiency, presents very limited treatment options and can lead to life-threatening complications. Discovering new therapeutic agents against thrombocytopenia has proven to be a challenging task using traditional screening approaches. Fortunately, machine learning (ML) techniques offer a rapid avenue for exploring chemical space, thereby increasing the likelihood of uncovering new drug candidates. In this study, we focused on computational modeling for drug-induced megakaryocyte differentiation and platelet production using ML me

The constrained-disorder principle defines the functions of systems in nature.

The Constrained Disorder Principle (CDP) defines all systems in nature by their degree of inherent variability. Per the CDP, the intrinsic variability is mandatory for their proper function and is dynamically changed based on pressures. The CDP defines the boundaries of inherent variability as a mechanism for continuous adaptation to internal and external perturbations, enabling survival and function under dynamic conditions. The laws of nature govern the world's natural phenomena and underlie the function of all systems. Nevertheless, the laws of physics do not entirely explain systems' funct

Artificial intelligence-based forecasting model for incinerator in sulfur recovery units to predict SO2 emissions.

Pollutant emissions from chemical plants are a major concern in the context of environmental safety. A reliable emission forecasting model can provide important information for optimizing the process and improving the environmental performance. In this work, forecasting models are developed for the prediction of SO2 emission from a Sulfur Recovery Unit (SRU). Since SRUs incorporate complex chemical reactions, first-principle models are not suitable to predict emission levels based on a given feed condition. Accordingly, artificial intelligence-based models such as standard machine learning (ML

Kinetic Modeling in Temperature-Modulated Semiconductor Gas Sensor Utilizing Eley-Rideal Mechanism and Its Application in Discriminative Detection of VOCs.

This study presents a comprehensive kinetic investigation for describing the dynamic sensing process of semiconductor metal oxide (SMO)-based gas sensors, with a focus on the Eley-Rideal mechanism as a valid pathway. The modeling elucidates the direct interactions between volatile organic compounds (VOCs) and adsorbed oxygen on the material surface, providing insights into the temperature-dependent response characteristics of the sensor and addressing selectivity toward different VOCs fundamentally. By incorporating the effects of thermal modulation, the kinetic model was validated through the

Complex interactions and composite burden of risk factors in vascular cognitive impairment.

Vascular cognitive impairment (VCI) stresses the vascular contributions to cognitive decline, ranging from mild to major forms. Except for symptomatic treatment for relevant vascular diseases, the other recommended strategy is to intervene in key vascular risk factors (VRFs) as early as possible. A considerable amount of previous research delineated the association of a specific factor with dementia, involving each risk factor discussed in the present review. However, due to the heterogeneity and complexity of VCI, managing a single factor is insufficient to reduce its incidence and prevalence

BACKGROUND OBJECTIVES: Discovery of new antibiotics is the need of the hour to treat infectious diseases. An ever-increasing repertoire of multidrug-resistant pathogens poses an imminent threat to human lives across the globe. However, the low success rate of the existing approaches and technologies for antibiotic discovery remains a major bottleneck. In silico methods like machine learning (ML) deem more promising to meet the above challenges compared with the conventional experimental approaches. The goal of this study was to create ML models that may be used to successfully predict new anti

Deep learning in optical coherence tomography: Where are the gaps?

Optical coherence tomography (OCT) is a non-invasive optical imaging modality, which provides rapid, high-resolution and cross-sectional morphology of macular area and optic nerve head for diagnosis and managing of different eye diseases. However, interpreting OCT images requires experts in both OCT images and eye diseases since many factors such as artefacts and concomitant diseases can affect the accuracy of quantitative measurements made by post-processing algorithms. Currently, there is a growing interest in applying deep learning (DL) methods to analyse OCT images automatically. This revi

Deep Learning Prediction and Visualization of Gender Related Brain Changes from Longitudinal Structural MRI Data in the ABCD Study.

Deep learning algorithms for predicting from neuroimaging data have shown considerable promise. Deep learning models that take advantage of the data's 3D structure have been proven to outperform ordinary machine learning on a number of learning tasks[1]. The majority of past research in this area, however, has focused on data from adults. Within the Adolescent Brain and Cognitive Development (ABCD) dataset, a major longitudinal development research, we examine the use of structural MRI data to predict gender and to identify gender related changes in brain structure. The results demonstrate tha

Classification of Normal and Malicious Traffic Based on an Ensemble of Machine Learning for a Vehicle CAN-Network.

Connectivity and automation have expanded with the development of autonomous vehicle technology. One of several automotive serial protocols that can be used in a wide range of vehicles is the controller area network (CAN). The growing functionality and connectivity of modern vehicles make them more vulnerable to cyberattacks aimed at vehicular networks. The CAN bus protocol is vulnerable to numerous attacks, as it is lacking security mechanisms by design. It is crucial to design intrusion detection systems (IDS) with high accuracy to detect attacks on the CAN bus. In this paper, we design an e

Small RNA-Seq Reveals Similar miRNA Transcriptome in Children and Young Adults with T-ALL and Indicates miR-143-3p as Novel Candidate Tumor Suppressor in This Leukemia.

We aimed to identify miRNAs and pathways specifically deregulated in adolescent and young adult (AYA) T-ALL patients. Small RNA-seq showed no major differences between AYA and pediatric T-ALL, but it revealed downregulation of miR-143-3p in T-ALL patients. Prediction algorithms identified several known and putative oncogenes targeted by this miRNA, including KRAS, FGF1, and FGF9. Pathway analysis indicated signaling pathways related to cell growth and proliferation, including FGFR signaling and PI3K-AKT signaling, with the majority of genes overrepresented in these pathways being predicted tar…

Clinical stratification improves the diagnostic accuracy of small omics datasets within machine learning and genome-scale metabolic modelling methods.

BACKGROUND: Recently, multi-omic machine learning architectures have been proposed for the early detection of cancer. However, for rare cancers and their associated small datasets, it is still unclear how to use the available multi-omics data to achieve a mechanistic prediction of cancer onset and progression, due to the limited data available. Hepatoblastoma is the most frequent liver cancer in infancy and childhood, and whose incidence has been lately increasing in several developed countries. Even though some studies have been conducted to understand the causes of its onset and discover pot

Non-Selective Thoracic Computed Tomography in Trauma Patients Results in Injury Severity Score Inflation.

INTRODUCTION: Extensive research relying on Injury Severity Scores (ISS) reports a mortality benefit from routine non-selective thoracic CTs (an integral part of pan-computed tomography (pan-CT)s). Recent research suggests this mortality benefit may be artifact. We hypothesized that the use of pan-CTs inflates ISS categorization in patients, artificially affecting admission rates and apparent mortality benefit. METHODS: Eight hundred and eleven patients were identified with an ISS >15 with significant findings in the chest area. Patient charts were reviewed and scores were adjusted to exclude
