Hemiarthroplasty for the rotator cuff-deficient shoulder. Surgical technique.

BACKGROUND: Hemiarthroplasty is a common treatment for cuff tear arthropathy and glenohumeral arthritis associated with a massive tear of the rotator cuff; however, to our knowledge, long-term outcomes and preoperative factors affecting results have not been reported. METHODS: Thirty-four shoulders in thirty-one patients with cuff tear arthropathy or a massive rotator cuff tear with glenohumeral arthritis underwent hemiarthroplasty at an average age of seventy-two years. Outcomes in all patients were evaluated by direct physical examination and according to the limited goals criteria of Neer

Observations from 450 shoulder dystocia simulations: lessons for skills training.

Poor neonatal outcomes after shoulder dystocia have been associated with inappropriate management. Until there are significant developments in the prediction and subsequent prevention of shoulder dystocia, improving shoulder dystocia management through practical training may be the most effective method of reducing the associated morbidity and mortality. Four hundred fifty simulated shoulder dystocia scenarios, managed by 95 midwives and 45 doctors from six U.K. hospitals during the course of 1 year, were video recorded during a study of obstetric emergency training. Analysis of recorded data

Biomechanics of the elbow joint in tennis players and relation to pathology.

Elbow injuries constitute a sizeable percentage of tennis injuries. A basic understanding of biomechanics of tennis and analysis of the forces, loads and motions of the elbow during tennis will improve the understanding of the pathophysiology of these injuries. All different strokes in tennis have a different repetitive biomechanical nature that can result in tennis-related injuries. In this article, a biomechanically-based evaluation of tennis strokes is presented. This overview includes all tennis-related pathologies of the elbow joint, whereby the possible relation of biomechanics to pathol

Biceps tenodesis associated with arthroscopic repair of rotator cuff tears.

Associated lesions of the biceps tendon are commonly found during arthroscopic repair of rotator cuff tears. These lesions are treated with tenodesis, classically performed through an open approach. However, it seems reasonable to seek a single approach to correct both lesions; therefore, we have proposed a new arthroscopic technique that allows an exclusive arthroscopic tenodesis by including the biceps tendon in the rotator cuff suture, a surgical technique with a single suture of the rotator cuff that includes the biceps tendon. We treated 97 shoulders in 96 patients arthroscopically for co

Supraspinatus tears: propagation and strain alteration.

It was hypothesized that there would be an alteration in strain when macroscopically normal supraspinatus tendons were subjected to three patterns of surgically created tear. The propagation of joint-side partial-thickness tears was also examined. Cadaveric shoulders were tested on a purpose-built rig with static loading from 20 to 200 N and during glenohumeral abduction from 0 degrees to 120 degrees with a 100-N tensile load. Differential variable reluctance transducers were used to calculate strain. Six-millimeter-wide midsubstance full-thickness tears (n = 2) caused an increase in bursal-si

MR observations of postraumatic osteolysis of the distal clavicle after traumatic separation of the acromioclavicular joint.

PURPOSE: The purpose of this work was to characterize the MR features of post-traumatic osteolysis of the distal clavicle in patients who have sustained a previous separation of the ipsilateral acromioclavicular (AC) joint. METHOD: We studied eight male patients (mean age 25 years) with intractable pain in the AC joint after sustaining a traumatic joint separation. With use of the Rockwood classification, the separations were classified as Type 1 in one patient, Type 2 in two patients, and Type 3 in five patients. The MR studies were evaluated for periarticular soft tissue swelling, cortical

The association between irregular greater tuberosities and rotator cuff tears: a sonographic study.

OBJECTIVE: Although an association between irregular greater tuberosities and rotator cuff tears has been suggested, no formal studies have been done. This study was designed to investigate the relationship between greater tuberosity irregularities, rotator cuff tears, and age. SUBJECTS AND METHODS: Sonographic examinations of both shoulders of 77 asymptomatic individuals (154 shoulders) between 30 and 80 years old were performed. The rotator cuffs and greater tuberosities were evaluated. Full- and partial-thickness rotator cuff tears were given equal significance. A two-tailed Fisher's exact

The rotator cuff. Large and massive tears. Technique of open repair.

Symptomatic large and massive rotator cuff tears can be successfully managed with operative repair. Five factors that facilitate favorable results include: (1) performing an adequate subacromial decompression; (2) maintaining the integrity of the deltoid origin; (3) mobilizing the torn tendons, performing an interval slide when indicated; (4) repairing the tendons to bone; and (5) carefully staging and supervising the rehabilitation program.

Retrospective cohort study on radial head arthroplasty comparing long-term outcomes between valgus type injury and fracture dislocation.

BACKGROUND: Radial head arthroplasty (RHA) has been commonly adopted for irreparable radial head fractures while little information is addressed on valgus type injury. The purpose of this study is to report long-term outcomes and radiographic analysis in RHA for valgus type injury with comparison to fracture dislocation injury. METHODS: A retrospective cohort study was conducted in patients receiving unilateral RHA with loose-fit, modular metal prosthesis for irreparable radial head fractures between 2004 and 2012. Totally, 33 patients with a mean follow up of 9 years (range, 7 to 15 years) w

Application of Autogenous Urine-Derived Stem Cell Sheet Enhances Rotator Cuff Healing in a Canine Model.

BACKGROUND: A repaired rotator cuff (RC) often heals with interposed scar tissue, making repairs prone to failure. Urine-derived stem cells (USCs), with robust proliferation ability and multilineage differentiation, can be isolated from urine, avoiding invasive and painful surgical procedures for harvesting the cells. These advantages make it a novel cell source for autologous transplantation to enhance RC healing. HYPOTHESIS: Implantation of an autogenous USC sheet to the injury site will enhance RC healing. STUDY DESIGN: Controlled laboratory study. METHODS: USCs isolated from urine were

Functional outcomes for unstable distal radial fractures treated with open reduction and internal fixation or closed reduction and percutaneous fixation. A prospective randomized trial.

BACKGROUND: Despite the recent trend toward internal fixation of distal radial fractures, few randomized trials have examined whether volar plate fixation is superior to other stabilization techniques. The purpose of the present study was to compare (1) open reduction and internal fixation with use of a volar plate and early mobilization with (2) percutaneous fixation and casting or external fixation for the treatment of dorsally displaced unstable extra-articular and simple intra-articular fractures of the distal part of the radius, with a specific emphasis on early functional recovery. Characteristics of Iatrogenic Nerve Injury from Orthopedic Surgery Correlate with Time to Subspecialty Presentation.

UNLABELLED: There is no current literature examining iatrogenic nerve injury resulting from orthopedic procedures across subspecialties and anatomic areas. This study uses a single peripheral nerve surgeon's experience to investigate the variable time to presentation of adult patients with iatrogenic nerve injury after orthopedic surgery. METHODS: A retrospective review of patients examined in Peripheral Nerve Clinic (PNC) from January 1, 2012, to April 1, 2018, at a single, private, university hospital was performed. Fifty-eight adult patients met inclusion criteria. Charts were reviewed to

Biological effects of extracorporeal shockwave therapy in tendons: A systematic review.

Extracorporeal shockwave therapy was initially used for kidney stone disintegration and its application was then extended to calcific tendinitis. The therapeutic field expanded and included numerous types of tendinopathies, from shoulder to plantar fascia. The clinical benefits were documented in trials and the effects and mechanisms were studied on models including animal and human tendons. The present systematic review outlines a large spectrum of biological effects. First, an optimal dose is adapted for each species and each tendon; exceeding the optimal dose may lead to structural injury.

Original portals for arthroscopic decompression of the suprascapular nerve: an anatomic study.

Operative treatment of suprascapular nerve entrapment consists of decompression of the nerve, either at the suprascapular notch or the spinoglenoid notch. The aim of this study was to describe new arthroscopic portals to approach these 2 notches at the same time. Twenty shoulders in 10 fresh frozen cadavers were investigated. Four portals were used in line with the scapular spine (S1, S2, S3, S4). The suprascapular pedicle was visualized passing under the supraspinatus muscle. The technique was performed for each specimen. The efficacy and safety of the technique were assessed by open dissecti

Accessory nerve: anatomy and surgical identification.

The XIth cranial nerve or accessory nerve provides the motor supply to the sternocleidomastoid and trapezius muscles. It is frequently encountered during neck surgery, and as such is at risk of iatrogenic injury, resulting in the 'shoulder syndrome'. Historically, the nerve was sacrificed on oncological grounds during radical neck dissection. However, the basis for sacrifice is unfounded in the majority of cases, and accessory nerve sparing selective neck dissection has equal oncological efficacy. The path of the nerve in the neck is very variable, and there is not a wholly reliable landmark f

Peripheral nerve injury in developing rats reorganizes representation pattern in motor cortex.

We investigated the effect of neonatal nerve lesions on cerebral motor cortex organization by comparing the cortical motor representation of normal adult rats with adult rats that had one forelimb removed on the day of birth. Mapping of cerebral neocortex with electrical stimulation revealed an altered relationship between the motor cortex and the remaining muscles. Whereas distal forelimb movements are normally elicited at the lowest threshold in the motor cortex forelimb area, the same stimuli activated shoulder and trunk muscles in experimental animals. In addition, an expanded cortical rep

PURPOSE: Complete high ulnar nerve injury can cause serious sequelae, including residual sensation and loss of movement and especially dysfunction of the intrinsic muscles of the hand. As a solution to treat complete high ulnar nerve injury, we proposed a new repair method for ulnar nerve injury based on nerve-magnified regeneration. METHODS: Twenty-two patients with complete division of the ulnar nerve at a high level who were treated from May 2013 to December 2016 were divided into two groups. The proposed repair method for complete high ulnar nerve injury was performed in group I (11 patie
