Arthrodesis of the Ankle.

About 90 % of all cases of painful posttraumatic ankle arthritis can be very successfully treated with a minor invasive ankle arthrodesis technique by using a small anterior approach and a fixation with four 6. 5 mm screws of which the posteromedial and transfibular one are inserted percutaneously. The results with this standardized procedure have been reported previously as excellent and good in a mid-term run of 6 years (34). This technique leads to a high union rate of 99% (92 of 93) with rapid bone healing within 8 ± 2 weeks, it causes a low minor complication rate of 8 % and enables a sig

Post-operative infection following ankle fracture surgery: a current concepts review.

PURPOSE: The most common early complication of operative treatment of ankle fractures is a surgical site infection (SSI) with an incidence rate varying between 1.5 and 16%, depending on various risk factors. A SSI has multiple disadvantages, including worse outcome and a socio-economic burden. The aim of this review is to provide an updated overview of the current concepts pertinent to SSI in ankle fractures. METHODS: A descriptive literature review was performed to provide the overview. RESULTS: Well known risk factors for SSI are higher age, diabetes, open fractures and fracture dislocatio

Oral myiasis after cerebral infarction in an elderly male patient from southern China: A case report.

BACKGROUND: Myiasis is a rare but risky pathology caused by a parasitic infestation of humans and animals by the dipterous larva. Oral myiasis occurs when soft tissues of the oral cavity are invaded by the larvae of flies. It is not a common disease for the reason that the oral cavity is not easily reachable for the fly to lay eggs. But it can cause pain, infection, uncomfortable feeling when the worms move, tissue destruction and/or even life-threatening hemorrhages. CASE SUMMARY: We reported a case of oral myiasis after cerebral infarction in a 78-year-old male patient from southern China (

Rapid diagnosis and quantification of Francisella tularensis in organs of naturally infected common squirrel monkeys (Saimiri sciureus).

Francisella tularensis, a small Gram-negative facultative intracellular bacterium, is the causative agent of tularaemia, a severe zoonotic disease transmitted to humans mostly by vectors such as ticks, flies and mosquitoes. The disease is endemic in many parts of the northern hemisphere. Among animals, the most affected species belong to rodents and lagomorphs, in particular hares. However, in the recent years, many cases of tularaemia among small monkeys in zoos were reported. We have developed a real-time PCR that allows to quantify F. tularensis in tissue samples. Using this method, we iden

Local gentamicin application for perineal wound healing following abdominoperineal rectum excision.

A major complication of abdominoperineal rectum excision for rectal or anal carcinoma is local wound infection. The main reason for this infection is that systemically administered antibiotic prophylaxis does not reach sufficiently high concentrations of antibiotic in the tissue of the sacroperineal wound. Since gentamicin-polymethylmethacrylate (PMMA) in the form of chains of beads has been confirmed as a useful tool in the treatment of local infection in bone and soft tissue surgery, we have evaluated its effect on the abdominoperineal rectum excision in a prospective, randomized trial. Non-invasive assessment of negative pressure wound therapy using high frequency diagnostic ultrasound: oedema reduction and new tissue accumulation.

Tissue oedema plays an important role in the pathology of chronic and traumatic wounds. Negative pressure wound therapy (NPWT) is thought to contribute to active oedema reduction, yet few studies have showed this effect. In this study, high frequency diagnostic ultrasound at 20 MHz with an axial resolution of 60 µm was used to assess the effect of NPWT at - 80 mmHg on pressure ulcers and the surrounding tissue. Wounds were monitored in four patients over a 3-month period during which changes in oedema and wound bed thickness (granulation tissue) were measured non-invasively. The results showed

Pedicled myocutaneous flap transplantation for a large chest wall defect with infection in a 72-year-old female.

BACKGROUND: In the realm of thoracic surgery, addressing chest wall defects accompanied by infections remains a formidable task. Despite the availability of a spectrum of surgical options, attaining clinical resolution is particularly challenging in intricate cases involving extensive chest wall defects in elderly patients. Thorough debridement followed by the utilization of autologous tissue for repair and reconstruction has emerged as a prevalent approach in current clinical practice. CASE PRESENTATION: Herein, we present a 72-year-old female patient with a large chest wall defect and infec

Wound infection caused by Staphylococcus arlettae: a case report and metal characterization.

BACKGROUND: Retained metallic foreign bodies can lead to implant-associated wound infections through bacterial colonization and biofilm formation. We report a case of a wound infection associated with a retained metallic fragment caused by Staphylococcus arlettae (S. arlettae) and evaluate the organism's early biofilm formation on common implant metals. CASE PRESENTATION: A 33-year-old man sustained a crush injury to his right hand and forearm, resulting in extensive soft-tissue damage and vascular injury. Emergency surgical management included meticulous debridement and vascular reconstructi

Arthroscopic treatment of septic arthritis of the metacarpophalangeal joint: Case report and technical description.

BACKGROUND: Cat bites to the hand are associated with a high risk of infection due to deep bacterial inoculation into closed anatomical spaces, frequently leading to severe complications such as septic arthritis. Although open arthrotomy has traditionally been the standard surgical treatment, arthroscopic management has become accepted in larger joints but remains rarely reported for metacarpophalangeal (MCP) joint infections. OBJECTIVE: To describe the arthroscopic technique for the treatment of septic arthritis of the metacarpophalangeal joint and to report the clinical outcome of a patient

Paediatric type I open tibia fractures: are antibiotics alone sufficient?

AIMS: The conventional management of the soft-tissue component of an open fracture involves emergent debridement. There is, however, evidence that questions this approach in the management of Gustilo-Anderson type I open fractures in paediatric patients. This systematic review aims to explore differences in infection rates between nonoperative management with antibiotics and operative debridement in children with type I open lower limb tibial fractures that do not require surgical fixation. METHODS: A systematic review following the PRISMA guidelines was conducted. Patients aged under 18 year

Transdermal delivery of antibacterial peptides in the management of methicillin-resistant Staphylococcus aureus.

Bacterial antimicrobial resistance (AMR) is of major public concern around the globe that arises when bacteria which earlier responded to th … s (AMPs) have emerged as an effective therapeutic option for the treatment of MRSA-induced skin and soft tissue infections (SSTIs). They offer broad-spectrum antibacterial activity with lower probability of drug resistance development. However, these AMPs face limitations such as proteolytic degradation, poor bioavailability, and potential cytotoxicity limiting their ef …

Necrotizing Fasciitis by Extended-Spectrum Beta-Lactamase-Producing Escherichia coli as the Initial Presentation of Hairy Cell Leukemia: A Case Report.

Necrotizing fasciitis (NF) is a rapidly progressive and life-threatening soft tissue infection. Fournier's gangrene (FG), a subtype involving the perianal and genital regions, is especially severe in immunocompromised individuals. Although rare, hairy cell leukemia (HCL) can initially present with severe infections due to profound immunosuppression. We report the case of a 53-year-old man with hypertension who developed a perianal abscess caused by extended-spectrum beta-lactamase (ESBL)-producing Escherichia coli. Despite surgical drainage and carbapenem therapy, the infection progressed. Lab

Plasma Proteomics Reveals Distinct Signatures in Occult and Microfilaremic Loa loa Infections.

BACKGROUND: Loiasis, caused by the filarial nematode Loa loa, imposes a significant disease burden in endemic regions in West and Central Africa. Manifestations include adult worms in soft tissue (e.g., the conjunctiva of the eye) and microfilaria in peripheral blood, with clinical presentations ranging from asymptomatic infections to life-threatening organ involvement. Diagnosis remains challenging due to variable microfilaria counts, frequent amicrofilaremic occult infections, and unreliable serological tests. The untargeted plasma proteome reflects broad (patho-)physiological responses, pro

Increasing the Use of Enteral Antibiotics in Hospitalized Children With Uncomplicated Infections.

BACKGROUND: Route of administration is an important component of antimicrobial stewardship. Early transition from intravenous to enteral antibiotics in hospitalized children is associated with fewer catheter-related adverse events, as well as decreased costs and length of stay. Our aim was to increase the percentage of enteral antibiotic doses for hospital medicine patients with uncomplicated common bacterial infections (community-acquired pneumonia, skin and soft tissue infection, urinary tract infection, neck infection) from 50% to 80% in 6 months. METHODS: We formed a multidisciplinary tea

Amphiphilic Dendrimer as Potent Antibacterial against Drug-Resistant Bacteria in Mouse Models of Human Infectious Diseases.

Modern medicine continues to struggle against antibiotic-resistant bacterial pathogens. Among the pathogens of critical concerns are the multidrug-resistant (MDR) Pseudomonas aeruginosa, Staphylococcus aureus, and Klebsiella pneumoniae. These pathogens are major causes of nosocomial infections among immunocompromised individuals, involving major organs such as lung, skin, spleen, kidney, liver, and bloodstream. Therefore, novel approaches are direly needed. Recently, we developed an amphiphilic dendrimer DDC18-8A exhibiting high antibacterial and antibiofilm efficacy in vitro. DDC18-8A is comp

Staphylococcus aureus Panton-Valentine Leukocidin worsens acute implant-associated osteomyelitis in humanized BRGSF mice.

Staphylococcus aureus is the most common pathogen that causes implant-associated osteomyelitis, a clinically incurable disease. Immune evasion of S. aureus relies on various mechanisms to survive within the bone niche, including the secretion of leukotoxins such as Panton-Valentine leukocidin (PVL). PVL is a pore-forming toxin exhibiting selective human tropism for C5a receptors (C5aR1 and C5aR2) and CD45 on neutrophils, monocytes, and macrophages. PVL is an important virulence determinant in lung, skin and soft tissue infections. The involvement of PVL in S. aureus pathogenesis during bone in…
