Attenuation of spinal cord ischemia and reperfusion injury by erythropoietin.

BACKGROUND: Paraplegia remains a devastating complication for patients undergoing thoracic aortic procedures. Although surgical adjuncts have evolved to reduce the risk of paraplegia, no pharmacologic therapies have proven efficacious in attenuating spinal cord ischemia-reperfusion injury. Effects of erythropoietin in spinal cord ischemia-reperfusion injury, however, have not yet been elucidated. We hypothesized that pretreatment with erythropoietin would attenuate functional and cytoarchitectural spinal cord injury related to high-risk aortic procedures. METHODS: Adult male mice were subject

U74389F, a 21-aminosteroid antioxidant, improves neoendothelial morphology, but not neointimal thickening after balloon catheter injury.

U74389F is a compound in a family of 21-aminosteroids devoid of classical glucocorticoid action that inhibit lipid peroxidation. These compounds improve neurologic function and tissue survival after head or spinal cord injury. Dexamethasone inhibits development of intimal hyperplasia (IH) and improves attenuated nitric oxide (NO) production of the rabbit aorta subsequent to balloon catheter injury. We tested the hypothesis that U74389F is protective in a catheter-induced endothelial-denuded and arterial injury model. A 4-Fr Fogarty balloon (BALL) embolectomy catheter was passed through the tho

Exclusion of aortic tear in the unstable trauma patient: the utility of transesophageal echocardiography.

OBJECTIVE: The goal of this study was to investigate the value of biplanar transesophageal echocardiography (TEE) as a screening tool for aortic tear in unstable trauma patients. METHODS: During a 1-year period, a prospective trial to exclude aortic tear was conducted at a level I trauma center. Ten of 53 patients (19%) sustaining severe blunt thoracic trauma were deemed too unstable to undergo safe transport to aortography and underwent TEE. Mechanism of injury was motor vehicle crash in eight patients and pedestrians struck in two. Patients had a mean Injury Severity Score = 34 (range, 17 t

Diagnostic accuracy of eFAST in the trauma patient: a systematic review and meta-analysis.

OBJECTIVES: Performing an extended Focused Assessment with Sonography in Trauma (eFAST) exam is common practice in the initial assessment of trauma patients. The objective of this study was to systematically review the published literature on diagnostic accuracy of all components of the eFAST exam. METHODS: We searched Medline and Embase from inception through October 2018, for diagnostic studies examining the sensitivity and specificity of the eFAST exam. After removal of duplicates, 767 records remained for screening, of which 119 underwent full text review. Meta-DiSc™ software was used to

[Injuries of neck, thorax and abdomen due to suicide or aggressive actions].

288 patients were treated for suicide or auto-aggressive injuries of neck, thorax and abdomen, which amounted 12,8% of all patients with injuries of such localization. Able-bodied men with prolonged depressive disorders pre-dominated among them. Neck injuries were observed in 21,5%, thorax injuries - in 25,7%, abdominal injuries - in 33,7% of patients. Combined injuries of neck, thorax and abdomen were the most hard for surgical treatment (19,1%). Types of injuries and severity of clinical course in postoperative period depends on character and intensity of psychic disorders.

Injury profiles in pedestrian motor vehicle trauma.

One hundred fifteen consecutive pedestrians who were struck by motor vehicles were studied to determine the magnitude and patterns of the injuries sustained. The mortality rate was 22%, and 17 of 25 patients who died did so during the initial resuscitative efforts, primarily due to head, chest, and or abdominal injury. The average Injury Severity Score (ISS) among all patients was 20; however, it was significantly higher (46) in nonsurvivors. The majority of the victims were men (72%), and the average age of all patients was 35 years. As the patient's age increased, so did the likelihood of mo

Sacubitril/Valsartan as a Cardiac Radioprotector.

PURPOSE: The role of the natriuretic peptides in radiation heart injury (RHI) has not been thoroughly examined. Pharmacologic modulation of the natriuretic peptide system with sacubitril/valsartan (sac/val) has led to improvements in heart failure therapy, and preliminary data suggest that RHI is associated with decreased atrial natriuretic peptide (ANP). In this study, we assessed sac/val as a radioprotector in a partial-heart irradiation mouse model and explored preliminary trends in patients receiving thoracic irradiation. METHODS AND MATERIALS: Female 8-10-week-old C57BL/6J mice were rand

En Bloc Resection of Thoracic and Upper Lumbar Spinal Tumors Using a Novel Rotation-Reversion Technique through Posterior-Only Approach.

BACKGROUND: En bloc resection is recommended for the treatment of malignant and aggressive benign spinal tumors; however, it often requires a combined anterior-posterior approach, which is usually accompanied by longer surgical duration, increased blood loss, larger trauma, and surgical complexity. The present study describes a novel rotation-reversion technique for en bloc resection of the thoracic and upper lumbar spinal tumors using a posterior-only approach and evaluate its safety and efficacy. METHODS: Thirteen patients with thoracic and upper lumbar (L1-L3) spinal tumors were treated wi

Time-Sequential Monitoring of the Early Mesothelial Reaction in the Pleura after Cryoinjury.

(1) Background: An early mesothelial reaction of the pleura, leading to fibrosis, has been reported in animals after chemical or heavy metal exposure. However, the visual monitoring of early time-sequential mesothelial reaction-associated cryoinjury has not been fully investigated. Therefore, this study aimed to evaluate and visualize the early mesothelial reactions seen following cryoinjury using rabbit pleura. (2) Methods: We monitored the early mesothelial reaction in rabbit pleurae after cryoinjury using optical coherence tomography (OCT), in real-time, which was then compared with patholo

Spinal Fractures in Ankylosing Spondylitis: Patterns, Management, and Complications in the United States - Analysis of Latest Nationwide Inpatient Sample Data.

OBJECTIVE: Ankylosing spondylitis (AS) is a rheumatic inflammatory disease marked by chronic inflammation of the axial skeleton. This condition, particularly when severe, can lead to increased risk of vertebral fractures attributed to decreased ability of the stiffened spinal column to sustain normal loads. However, little focus has been placed on understanding the locations of spinal fractures and associated complications and assessing the correlation between these. In this review, we aim to summarize the complications and treatment patterns in the United States in AS patients with spinal fra

Experimental two-stage simulated repair of extensive thoracoabdominal aneurysms reduces paraplegia risk.

BACKGROUND: In a pig model, we compared spinal cord injury after extensive segmental artery (SA) sacrifice in a single stage with recovery after a two-stage procedure: lumbar artery followed by thoracic SA sacrifice. METHODS: Twenty juvenile Yorkshire pigs were randomly assigned to undergo extensive SA sacrifice at 32 degrees C in a single operation (group 1, n = 10), or thoracic SA ligation 7 days after lumbar artery sacrifice (group 2, n = 10). Spinal cord perfusion pressure (SCPP) was monitored using a catheter placed in the distal stump of L1. Hind limb function was evaluated intraoperati

The effect of left subclavian artery coverage on morbidity and mortality in patients undergoing endovascular thoracic aortic interventions: a systematic review and meta-analysis.

OBJECTIVES: Thoracic endografts (stent grafts) have emerged as a less invasive modality to treat various thoracic aortic lesions. The intentional coverage of the left subclavian artery (LSA) during the placement of these endografts is associated with several complications including stroke, spinal cord ischemia, and arm ischemia. In this review, we synthesize the available evidence regarding the complications associated with LSA coverage. METHODS: We searched electronic databases (MEDLINE and EMBASE) from January 1990 through February 2008 for studies that included patients who received thorac

Trauma as a critical condition in the very old: assessment, care pathways and outcome differences.

Trauma in very old adults represents a growing and distinct form of critical illness, reflecting demographic aging and increasing levels of frailty, multimorbidity, and dependency. Unlike younger populations, where trauma is typically driven by high-energy mechanisms, injury in older adults most commonly results from low-energy falls. Despite this, the consequences are often severe, with high rates of hemorrhage, traumatic brain injury and thoracic trauma, and subsequent multi-organ dysfunction requiring critical care. Age-related physiological changes, combined with multimorbidity and polypha

Robot-Assisted Reinforcement with a Free Pericardial Fat Pad for Double-Lumen Tube-Related Injury of the Left Main Bronchus Incidentally Detected during Lobectomy: A Case Report.

INTRODUCTION: Tracheobronchial injury associated with double-lumen tube (DLT) placement is rare and typically diagnosed postoperatively. Intraoperative detection of subtle airway lesions is extremely uncommon. We report a case of DLT-related injury of the left main bronchus that was incidentally detected during robot-assisted thoracic surgery (RATS) and successfully reinforced with a free pericardial fat pad. CASE PRESENTATION: An 82-year-old man underwent robot-assisted left lower lobectomy and mediastinal lymph node dissection for suspected primary or metastatic lung cancer. During subcarin
