Arthroscopic surgery in children.

Arthroscopic surgery for children and adolescents is developing fast, thanks to technical progress in adult arthroscopy, improved knowledge … e.g., in knee ligament reconstruction) needs to be known. Joint decoaptation often does not require traction, and any stress maneuvers (valgus/varus) need to be gentle. The knee is by far the most frequently implicated joint in child arthroscopy, partly due to a rise in sports injuries. There is a variety of traumatic pathologies (osteochondral or meniscal …

[Influence of manual therapy of cervical spine on typical trigeminal neuralgia: a case report].

This article presents the case of a 43-year-old female patient with pain in the cervical spine area and a typical trigeminal neuralgia (TN; French name "tic douloureux") in the receptive field of the second and the third branches of the left trigeminal nerve. The patient came to our medical practice for a manual therapy of the cervical spine as the application of the standard therapy had not given her any pain reduction in the cervical spine area. As the result of the manual therapy of the cervical spine (nonspecific traction mobilization, specific or segmental mobilization, manipulation), not…

Anti-angiogenic drugs as an adjunctive therapy in the surgical treatment of diabetic retinopathy.

Anti-VEGF drugs may be employed in the surgical treatment of diabetic retinopathy: 1. Prior to surgery. The intravitreal injection of anti-VEGF drugs leads to a significant reduction of neovascularization, with a reduction in the adherence of the fibrovascular complex to the retina. This simplifies viscodelamination and reduces intraoperative bleeding during delamination and segmentation. To minimize the risk of tractional retinal detachment due to the contraction of fibrovascular tissue, vitrectomy must be performed within one week after the injection. 2. To decrease the risk of postoperative…

Objective of pharmacologic vitreolysis.

The goal of pharmacologic vitreolysis is to cleave the vitreoretinal junction, thereby inducing posterior vitreous detachment (PVD), and to liquefy the vitreous gel. There are several reasons to pursue a pharmacologic approach: (1) Mechanical vitrectomy is incomplete. Both at the posterior pole and in the retinal periphery, remnants of the cortical vitreous are left behind at the internal limiting membrane of the retina, causing vitreoretinal traction and (re)proliferation of cells, and leading to surgical failure. (2) Pharmacologic vitreolysis offers complete PVD without mechanical manipulati…

Infantile hip arthroscopy: the relationship between neurovascular anatomy and the portal pathway.

OBJECTIVE: In the past, studies of hip arthroscopy portal path relate with extra-articular structures were done in adults. This study was investigated in the infantile group. MATERIAL AND METHOD: 10 hips of fresh infantile cadavers. K-wire diameter 2.4 mm. are representing scope pathway. Three portals (anterior, anterolateral and posterolateral) in supine position without traction were chose to used in this study. RESULTS: One cadaver was female and four were male. The mean age and weight when death occurred was 74 days and the mean weight was 3584.4 gm. At the anterior portal, the most late…

Suprascapular neuropathy.

Suprascapular neuropathy is a relatively uncommon but significant cause of shoulder pain and dysfunction. The suprascapular nerve follows a tortuous course from the neck to the posterior shoulder. There are several potential causes of nerve entrapment along this path, particularly at the vulnerable suprascapular and spinoglenoid notches, where nerve excursion is limited by bony and ligamentous constraints. Additional extrinsic compression may be caused by glenohumeral joint-related ganglion cysts or soft-tissue masses. Traction neuropathy may occur following excessive nerve excursion during ov…

OBJECTIVE: To assess the clinical value of modified transabdominal preperitoneal (TAPP) repair of groin hernias in adult patients. METHOD: … follow-up for 2-22 months. The patients complained of no intestinal obstruction symptoms including traction pain, abdominal pain, or nausea or urinary bladder stimulation symptoms. CONCLUSIONS: Modified TAPP repair of groin hernias requires simple operation and produces reliable effect, and is therefore of clinical values for wide application.

Suprascapular nerve release for treatment of shoulder and periscapular pain following intracranial spinal accessory nerve injury.

Iatrogenic injury to the spinal accessory nerve is one of the most common causes of trapezius muscle palsy. Dysfunction of this muscle can be a painful and disabling condition because scapular winging may impose traction on the soft tissues of the shoulder region, including the suprascapular nerve. There are few reports regarding therapeutic options for an intracranial injury of the accessory nerve. However, the surgical release of the suprascapular nerve at the level of the scapular notch is a promising alternative approach for treatment of shoulder pain in these cases. The author reports on …

Occurrence of antibodies against Leptospira spp. in horses of the urban area of Londrina, Paraná, Brazil.

A total of 320 horses were studied in this paper, both male and female, between two and 17 years of age, which were used for traction of wagons in the urban area of the municipality of Londrina (PR). These animals were kept, after their daily work, in abandoned areas or plots, in the outskirts of the urban area of the city. When these animals were attended by the veterinarians, between 1996 and 2005, none of them presented symptoms suggesting leptospirosis. The most frequent reasons for the visit were loss of weight, unwillingness for work, parasitism, laminess, and wounds. Microscopic Seroagg…

Trigeminocardiac reflex during skull base surgery: mechanism and management.

BACKGROUND: We study the occurrence and management of the trigeminocardiac reflex (TCR) during neurosurgical procedures for lesions of the skull base. METHOD: Two hundred patients underwent neurosurgical procedures for various skull base lesions and were evaluated retrospectively for the occurrence of the TCR during surgery. This phenomenon was defined as the onset of bradycardia lower than 60 beats/minute and hypotension with a drop in mean arterial blood pressure of 20% or more due to intra-operative manipulation or traction on the trigeminal nerve. FINDINGS: Sixteen patients (8%) had a TC…

Part 1. Injuries to the brachial plexus: mechanisms of injury and identification of risk factors.

Upper-arm weakness (paresis) or paralysis indicates peripheral-nerve damage to the brachial plexus, a network of lower cervical and upper thoracic spinal nerves supplying the arm, forearm, and hand. Physical findings reflect muscle paralysis from spinal nerve roots. The mechanism of injury includes maternal, obstetric, and infant factors that apply traction on or compression to the anatomically vulnerable brachial plexus. Nerve regeneration can occur if nerve tissue components are preserved. Recovery is affected by multiple factors, including the type and site of injury, intervention timing, a…
