Penetrating popliteal artery injuries in children.

Two children, aged 8 and 12, were seen recently at the Vanderbilt University Medical Center and Metropolitan Nashville General Hospital with gunshot injury to the popliteal artery. Both patients presented with late complications following missed penetrating injury to the popliteal artery. At presentation, distal pulses were palpable despite significant proximal arterial injury. Arteriography detailed traumatic popliteal artery aneurysms in both children, and an arteriovenous fistula in one child. These two children represent the youngest patients recorded in the surgical literature with gunsho

Iatrogenic vascular injury. A reducible problem.

During a six-year period 46 patients were treated for iatrogenic vascular injuries at the University of Texas Health Science Center, San Antonio. Diagnostic procedures led to 24 injuries, while therapeutic procedures were responsible for 22 vascular injuries. Trauma to the brachial and femoral arteries and the subclavian vein accounted for the majority of injuries. The most frequently encountered injuries were intimal tear, thrombosis, and laceration. Lateral suture, thrombectomy, and intimal repair were the most commonly employed forms of vascular repair. Postoperative complications were not

Patellar tendon avulsion as a complication of Osgood-Schlatter's disease.

The etiology of Osgood-Schlatter's disease remains a topic for debate. Previous studies by Ogden and Southwick (Osgood-Schlatter's disease and tibial tubercle development. Clin Orthop 116: 180 - 189, 1976) strongly support the concept of multiple microavulsions of the anterior surface of the developing ossification center of the tibial tubercle as the underlying cause. Orthopaedic literature fails to associate Osgood-Schlatter's disease with traumatic disruption of the extensor mechanism of the knee. Though the mechanism of injury for such trauma is as common as Osgood-Schlatter's disease itse

Outcomes of Surgical Treatment of Femoral Head Fractures with a Minimum of 1-Year Follow-up.

BACKGROUND: Femoral head fractures are uncommon injuries, usually resulting from high-energy trauma and often accompanied by other musculoskeletal injuries, which complicate management. Despite advancements in surgical techniques, complications such as posttraumatic osteoarthritis (OA) and heterotopic ossification (HO) remain prevalent, and no standardized treatment protocol has been established. Furthermore, most existing reports are small-scale or multi-center studies with heterogeneous management. The aim of this study was to evaluate the radiological and functional outcomes, as well as com

Prevalence of mental disorders and related risk factors in refugees and asylum seekers in Campania.

INTRODUCTION: In recent years, the increasing presence of refugees and asylum seekers displaced from their country of origin, determined significant social, economic, humanitarian and public health implications in host countries, including Italy. These populations are exposed to several potential stressful experiences which make them vulnerable to psychological distress. In fact, the majority of studies addressing the topic found a higher prevalence of mental disorders, especially post-traumatic stress disorder and major depressive disorder, in refugees and asylum seekers with respect to the g

Seasonal variation and preoperative risk factors for polymicrobial infection following open fracture.

INTRODUCTION: Despite modern approaches to open fracture management, fracture-related infection (FRI) rates remain high. Recent studies demonstrated the seasonal and regional variation of causative organisms in FRI. This study aims to better understand the causative organisms and identify preoperative risk factors for the primary outcome of FRIs at a Level I trauma center. MATERIALS AND METHODS: This retrospective cohort study examined all patients that underwent irrigation and debridement of an open fracture at a single Level I trauma center between 2007 and 2019. Exclusion criteria included

Child Abuse, Misdiagnosed by an Expertise Center-Part II-Misuse of Bayes' Theorem.

A newborn girl had, from two weeks on, small bruises on varying body locations, but not on her chest. Her Armenian grandmother easily bruised, too. Her mother was diagnosed with hypermobility-type Ehlers-Danlos-Syndrome (hEDS), an autosomal dominant connective tissue disorder, with a 50% inheritance probability. Referral to a University Medical Center located "Dutch Expertise Center for Child Abuse" resulted (prior to consultation) in physical abuse suspicion. Protocol-based skeletal X-rays showed three healed, asymptomatic rib fractures. A protocol-based Bayesian likelihood ratio guesstimatio

Management of Traumatic Femur Fractures: A Focus on the Time to Intramedullary Nailing and Clinical Outcomes.

BACKGROUND: Femur shaft factures (FSF) are common injuries following high-energy mechanisms mainly involving motor vehicle crashes (MVC). We evaluated the timings of nailing management and analyzed the pattern of fracture union and outcome in a level 1 trauma center. METHODS: This was a retrospective observational study of all the admitted trauma patients who sustained femoral fractures between January 2016 and September 2020. Data were analyzed and compared based on time to Intramedullary Nailing (IMN) (<12 h, 12-24 h and >24 h) and outcomes of FSF (union, delayed union and nonunion). RESUL

Airway management practices among emergency physicians: An observational study.

OBJECTIVES: Emergency airway management is an integral part of patient stabilization. It is an essential skill for an emergency physician to master. There is a paucity of literature on airway management from low-to-middle-income countries like India where emergency medicine (EM) specialty is still in its infancy. We conducted this study to identify the existing airway management practices among emergency physicians in our tertiary care center. This study could pave the way for national airway registries. METHODS: This prospective, observational study was conducted in the emergency department

Use of Intraoperative Computed Tomography in Craniomaxillofacial Trauma Surgery.

PURPOSE: The use of technology to aid in assessment, planning, and management of complex craniomaxillofacial injuries is increasingly common. Preoperative computed tomography (CT) evaluation is considered the standard of care, and intraoperative imaging is becoming increasingly accessible. Limited data exist regarding the implication of intraoperative CT on decision making in the management of all sites of facial fractures. The purposes of this study were to characterize the use of an intraoperative CT scanner for craniomaxillofacial surgery within our institution, to quantify the effect of in

Blunt cerebrovascular injuries in association with craniocervical distraction injuries: a retrospective review of consecutive cases.

BACKGROUND CONTEXT: Blunt cerebrovascular injuries (BCVIs) have the potential to cause brain, cerebellar, and/or spinal cord ischemia. Certain subtypes of spine fractures, such as vertebral subluxation, fractures through the foramen transversarium, and C1-C3 fractures have been linked to a higher incidence of BCVI. On the other hand, BCVI in association with craniocervical distraction injuries (CCDs) have been only anecdotally reported. PURPOSE: We hypothesized that because CCD is also caused by a high-energy hyperflexion/hyperextension distraction mechanism, it could also be associated with

Characterizing ambulation potential following acute traumatic spinal cord injury: a populational cohort study on 1209 patients.

STUDY DESIGN: Population cohort study. OBJECTIVES: Propose a new paradigm of using a descriptive approach - rather than a predictive tool - to estimate the ambulation potential after traumatic spinal cord injury (TSCI). SETTING: 30 trauma and rehabilitation centers across Canada participating in the Rick Hansen SCI Registry. METHODS: A retrospective study of 1209 patients from the Rick Hansen Spinal Cord Injury Registry was conducted. The outcome was the level of outdoor walking 12 months following the SCI: dependent vs. limited vs. independent community ambulators. The actual ambulation ou

Clinical Characteristics Associated with Fetomaternal Hemorrhage: A Retrospective Multicenter Descriptive Study (2009-2026).

Fetomaternal hemorrhage refers to the transfer of fetal erythrocytes into the maternal circulation. Although clinically significant fetomaternal hemorrhage is rare, it can result in severe fetal anemia with substantial morbidity and mortality. Early recognition remains challenging due to a nonspecific clinical presentation. This study aimed to identify maternal and obstetric characteristics associated with fetomaternal hemorrhage that may serve as early clinical warning signs. In this retrospective multicenter descriptive study, singleton pregnancies with a positive Kleihauer-Betke test from f

Social Factors Associated with Ultrasound Versus Computed Tomography Utilization Among Children Diagnosed with Pyelonephritis in the Emergency Department.

RATIONALE AND OBJECTIVES: Acute pyelonephritis can complicate lower urinary tract infections (UTI) in children. While often unnecessary, imaging may be helpful if clinical symptomatology is atypical or complications are suspected. As social determinants of health (SDoH) can impact care, this study examines the correlational relationship between SDoH and computed tomography (CT) versus ultrasound (US) when imaging is used in pediatric patients with pyelonephritis in the emergency department. MATERIALS AND METHODS: A multivariate logistic regression analysis was performed using data from the Na
