Optimizing non-invasive brain stimulation for obsessive-compulsive disorder: a systematic review of protocol heterogeneity, personalized targets, and adjunctive efficacy.

Obsessive-Compulsive Disorder (OCD) is a chronic psychiatric disorder characterized by intrusive thoughts and repetitive behaviors, leading to significant distress and functional impairment. First-line treatments include selective serotonin reuptake inhibitors and cognitive-behavioral therapy, particularly exposure and response prevention (ERP). However, a substantial proportion of patients exhibit limited or no response to these interventions. Repetitive transcranial magnetic stimulation (rTMS) has shown potential in modulating dysfunctional cortical circuits implicated in OCD pathophysiology

Non-invasive ventilation in the care of patients with chronic obstructive pulmonary disease with palliative care needs: a scoping review.

BACKGROUND: Patients with severe chronic obstructive pulmonary disease (COPD) could have palliative care (PC) needs because of unmet needs such as dyspnoea. This may lead to anxiety and may have an impact on patients' ability to perform daily activities of living. PC can be started when patients with COPD have unmet needs and can be provided alongside disease-modifying therapies. Non-invasive ventilation (NIV) could be an important measure to manage dyspnoea in patients with COPD in need of PC. A scoping review was conducted to gain an overview of the existing research and to identify knowledg

Recovering Hidden Responder Groups in Individuals Receiving Neurofeedback for Tinnitus.

The widespread understanding that chronic tinnitus is a heterogeneous phenomenon with various neural oscillatory profiles has spurred investigations into individualized approaches in its treatment. Neurofeedback, as a non-invasive tool for altering neural activity, has become increasingly popular in the personalized treatment of a wide range of neuropsychological disorders. Despite the success of neurofeedback on the group level, the variability in the treatment efficacy on the individual level is high, and evidence from recent studies shows that only a small number of people can effectively m

Patient-oriented randomisation: A new trial design applied in the Neuroleptic Strategy Study.

BACKGROUND: The 'gold standard' for clinical studies is a randomised controlled trial usually comparing specific treatments. If the scientific study expands to strategy comparison with each strategy including various treatments, the research problems are increasingly complicated. The strategy debate in the psychiatric community is the starting point for the development of our new design. It is widely accepted that second-generation antipsychotics are the therapy of choice in the treatment of schizophrenia. However, their general superiority over first-generation antipsychotics could not be dem

Autologous stem cell transplantation in follicular lymphoma: a systematic review and meta-analysis.

BACKGROUND: The impact of high-dose therapy and autologous stem cell transplantation (ASCT) vs conventional-dose chemotherapy in the initial management of adults with advanced follicular lymphoma (FL) on overall survival remains uncertain. We performed a systematic review of the randomized clinical trials addressing this question. METHODS: We searched MEDLINE, EMBASE, CENTRAL, American Society of Hematology, American Society of Clinical Oncology, BIOSIS, PAPERSFIRST, PROCEEDINGS, clinical trials registries, and bibliographies of relevant studies for randomized clinical trials comparing myeloa

Prevalence of Claudin 18.2 Expression in Gastric and Gastroesophageal Junction Adenocarcinoma: A Systematic Review and Meta-Analysis.

BACKGROUND: Claudin 18 isoform 2 (CLDN18.2) has emerged as a clinically validated therapeutic target in gastric and gastroesophageal junction (GEJ) adenocarcinoma following the regulatory approval of zolbetuximab in combination with first-line chemotherapy. Accurate prevalence data at the clinically validated immunohistochemical threshold are essential for patient selection, healthcare resource planning, and treatment strategy. Reported prevalence estimates vary widely across studies due to differences in populations, methodologies, and immunohistochemical protocols. This systematic review and

Neutrophil extracellular traps in tumor microenvironment for radiotherapy: friend or foe?

Neutrophil extracellular traps (NETs) are chromatin-protein complexes released by neutrophils that have dual roles in the tumor microenvironment (TME), participating in anti-tumor immunity and driving malignant progression by promoting tumor proliferation, metastasis, and immune escape. Radiotherapy (RT) exerts anticancer effects by inducing DNA damage and microenvironmental remodeling, but the inflammatory response it triggers can activate the formation of NETs, which exacerbate radioresistance by degrading the extracellular matrix, encapsulating circulating tumor cells, inhibiting CD8+ T-cel

Transcatheter Chordal Repair for Degenerative Mitral Regurgitation: Systematic Review and Meta-analysis.

Transcatheter chordal repair enables off-pump, echocardiography-guided treatment of degenerative mitral regurgitation, but real-world outcomes remain variably reported. We performed a systematic review and meta-analysis of studies evaluating outcomes after transcatheter chordal repair in adults with primary degenerative mitral regurgitation. Pooled event rates were calculated using random-effects models. Seventeen studies (n = 3,787) were included. Mitral regurgitation ≤ mild was achieved in 93.9% of patients at discharge and 82.5% at 30 days, while moderate mitral regurgitation occurred in 5.

Tumor microenvironments with an active type I IFN response are sensitive to inhibitors of heme degradation.

The tumor microenvironment (TME) is highly heterogeneous and can dictate the success of therapeutic interventions. Identifying TMEs that are susceptible to specific therapeutic interventions paves the way for more personalized and effective treatments. In this study, using a spontaneous murine model of breast cancer, we characterize a TME that is responsive to inhibitors of the heme degradation pathway mediated by heme oxygenase (HO), resulting in CD8+ T cell- and NK cell-dependent tumor control. A hallmark of this TME is a chronic type I interferon (IFN) signal that is directly involved in or

Beyond M1/M2: The role of reactive oxygen species in liver fibrosis and immune modulation.

Liver fibrosis is a progressive condition resulting from chronic liver injury and characterized by excessive extracellular matrix deposition and architectural disruption. Reactive oxygen species (ROS), long considered merely cytotoxic, are now recognized as key modulators of immune responses and fibrogenic pathways. In this context, macrophages play a central role. Traditionally classified into M1 (pro-inflammatory) and M2 (anti-inflammatory) phenotypes, macrophages exhibit greater heterogeneity and plasticity than this binary model suggests. This review critically re-evaluates the role of ROS

Sarcopenia and Its Impact on Sexual Health: A Systematic Review.

Sarcopenia is increasingly recognised for its detrimental effects on functional status and quality of life. When combined with obesity, in a condition known as sarcopenic obesity, these effects are further exacerbated by metabolic and hormonal dysfunctions. In this scenario, emerging evidence suggests that these conditions may also negatively impact sexual health. To explore this relationship, we conducted a systematic review (PROSPERO registration: CRD42025646427). Out of 943 identified articles, only seven met the eligibility criteria. The majority of studies, mainly cross-sectional studies

Exploration of Herbal Medicinal Plants and Formulations Available for the Treatment of PCOS: A review.

Polycystic ovarian syndrome (PCOS) is a heterogeneous endocrine disorder with the underlying consequence of ovarian cysts, anovulation, and endocrine variation affecting women. According to the World Health Organization (WHO) estimation over 116 million women (3.4%) are affected by PCOS worldwide. It is typified by polycystic ovaries, hyperandrogenism, and ovulatory dysfunction. Pharmacological interventions that treat symptoms but may have fewer side effects are frequently used in conventional treatments. As a result, interest in herbal medicinal plants and formulations as additional or alter

A large open access dataset of brain metastasis 3D segmentations on MRI with clinical and imaging information.

Resection and whole brain radiotherapy (WBRT) are standard treatments for brain metastases (BM) but are associated with cognitive side effects. Stereotactic radiosurgery (SRS) uses a targeted approach with less side effects than WBRT. SRS requires precise identification and delineation of BM. While artificial intelligence (AI) algorithms have been developed for this, their clinical adoption is limited due to poor model performance in the clinical setting. The limitations of algorithms are often due to the quality of datasets used for training the AI network. The purpose of this study was to cr

Transthyretin Amyloidosis: Role of oxidative stress and the beneficial implications of antioxidants and nutraceutical supplementation.

Transthyretin (ATTR) amyloidosis constitutes a spectrum of debilitating neurodegenerative diseases instigated by systemic extracellular deposition of partially unfolded/aggregated aberrant transthyretin. The homotetrameric protein, TTR, is abundant in the plasma, and to a lesser extent the cerebrospinal fluid. Rate-limiting tetramer dissociation of the native protein is regarded as the critical step in the formation of morphologically heterogenous toxic aggregates and the onset of clinical manifestations such as polyneuropathy, cardiomyopathy, disturbances in motor and autonomic functions. The effect of vertical identification card laws on teenage tobacco and alcohol use.

We study the impact of vertical identification card laws, which changed the orientation of driver's licenses and state identification cards from horizontal to vertical for those under 21 years, on teenage tobacco and alcohol use. We study this question using four national datasets (pooled national and state Youth Risk Behavior Surveillance System, National Youth Tobacco Survey, Current Population Survey to Tobacco Use Supplements, and Behavioral Risk Factor Surveillance System). We improve previous databases of vertical ID law implementation by using original archival research to identify the

Radiological and histopathological correlations in oligodendroglioma: a comprehensive case report.

Oligodendroglial tumors are rare tumors that constitute part of the neuroepithelial tumors of the central nervous system. A diffuse, low-grade astrocytoma (WHO grade II), oligodendrogliomas are typically encountered in adults, with children under the age of 15 accounting for about 25% of cases. Although they can arise anywhere in the central nervous system, oligodendrogliomas typically occur in the cerebral white matter. Radiotherapy is the main mode of treatment since surgical intervention is limited to the role of biopsy and management of secondary effects, due to the deep brain location of

From Cell to Symptoms: The Role of SARS-CoV-2 Cytopathic Effects in the Pathogenesis of COVID-19 and Long COVID.

Severe Acute Respiratory Syndrome CoronaVirus 2 (SARS-CoV-2) infection triggers various events from molecular to tissue level, which in turn is given by the intrinsic characteristics of each patient. Given the molecular diversity characteristic of each cellular phenotype, the possible cytopathic, tissue and clinical effects are difficult to predict, which determines the heterogeneity of COVID-19 symptoms. The purpose of this article is to provide a comprehensive review of the cytopathic effects of SARS-CoV-2 on various cell types, focusing on the development of COVID-19, which in turn may lead

Transplantable stem cell nanobridge scaffolds for accelerating articular cartilage regeneration.

Microfracture technique for treating articular cartilage defects usually has poor clinical outcomes due to critical heterogeneity and extremely limited in quality. To improve the effects of current surgical technique (i.e., microfracture technique), we propose the transplantable stem cell nanobridge scaffold, acting as a protective bridge between host tissue and defected cartilage as well as microfracture-derived cells. Nanobridge scaffolds have a sophisticated nanoaligned structure with freestanding and flexible shapes for imposing direct structural guidance to cells including transplanted st
