Planning and Developing a Symptom Diary Intervention for Breast Cancer Survivors With Concerns About Medication Brands (ENABLE Study): User-Centered Design Approach.

BACKGROUND: Approximately 80% of breast cancers are estrogen receptor positive, and following initial tumor treatment, patients are prescribed hormone therapy (HT) drugs (tamoxifen, letrozole, anastrozole, and exemestane) for 5-10 years. These drugs are known to cause several side effects. Additionally, a small number of studies have identified that changing medication brands (generics) can negatively affect patients' side effects, attitudes, and acceptance of HT. However, no effective intervention currently exists to address patients' concerns about generic switching. OBJECTIVE: This study e

Nitrite increases mitofusin-1 levels to inhibit vascular smooth muscle cell proliferation and prevent intimal hyperplasia.

Vascular disease remains a leading cause of morbidity and mortality and is driven by maladaptive vascular remodeling following injury. Stent-induced vascular injury induces vascular smooth muscle cell (VSMC) phenotypic switching from a contractile to a proliferative state, resulting in intimal hyperplasia (IH), restenosis and compromised vessel function. Nitrite, an endogenous oxidation product of nitric oxide and a dietary constituent, attenuates IH after vascular injury; however, its underlying mechanisms remain incompletely understood. Nitrite is known to modulate mitochondrial structure an

A cost-benefit and sustainability model for high-efficiency reverse osmosis implementation in Moroccan Dialysis Centers.

BACKGROUND: Standard reverse osmosis systems for hemodialysis operate at 60-75% recovery rates, wasting 25-40% of feed water as brine. This inefficiency poses a critical challenge in water-scarce regions like Morocco, where freshwater availability is projected to fall below 500 m³ per capita by 2030. METHODS: This study assessed all 437 dialysis centers across Morocco, analyzing current reverse osmosis system capacity, water rejection rates, and operating hours to calculate the potential benefits of switching to high-efficiency systems with rejection rates below 15%. RESULTS: Upgrading to hi

GRF1 fine-tunes the copper-responsive transcription module of CPK3-bHLH107 by manipulating interaction and phosphorylation.

Previously, the crucial role of calcium-dependent protein kinase 3 (CPK3) in Cu2+-triggered immunity (CuTI) has been established in Arabidopsis. CPK3 promotes the phosphorylation of basic helix-loop-helix 107 (bHLH107), thereby activating the expression of 1-aminocyclopropanecarboxylic acid synthesis 8 (ACS8) and downstream defense responses. However, the precise mechanisms underlying the fine-tuning of CPK3-mediated phosphorylation of bHLH107 remain unclear. To elucidate the mechanism underlying the inactivation of the CPK3-bHLH107 module, we identified a 14-3-3 protein, GRF1 (GF14χ), among c

Disease modification in lupus nephritis: towards a pathophysiology-based treatment paradigm.

Glucocorticoids and antiproliferative agents, such as mycophenolate and cyclophosphamide, form the backbone of current therapy for lupus nephritis. Despite their ability to suppress inflammation, such treatments are often limited by their incomplete efficacy, substantial toxicity and failure to prevent disease progression. Many patients continue to relapse and accumulate kidney damage, underscoring the urgent need for new strategies. Advances in our understanding of the pathophysiology of lupus nephritis have provided insights into the mechanisms by which the impaired clearance of nuclear self

Association between initial treatment strategy and progression to difficult-to-treat rheumatoid arthritis (D2T-RA): evidence from a 20-year-old real-world cohort.

BACKGROUND/OBJECTIVES: Despite major advances in rheumatoid arthritis (RA) management, a substantial proportion of patients evolve to difficult-to-treat RA (D2T-RA). We estimated the probability of progression to D2T-RA and assessed whether the initial choice of biological/targeted synthetic disease-modifying antirheumatic drugs (b/tsDMARDs) is associated with this risk. METHODS: We retrospectively analysed 675 patients with established RA from the first consultation through all b/tsDMARD lines to the last follow-up or D2T-RA classification. Kaplan-Meier, log-rank and multivariable Cox regres

Smart hydrogels modulate neutrophil fate to restore immune surveillance and homeostasis in diabetic wound healing.

Diabetic wounds are highly susceptible to bacterial infection and exhibit impaired healing largely driven by defective neutrophil function. Herein, we developed an immunomodulatory hydrogel hybridized with a diselenide-bridged polymer (HHD) to regulate neutrophil fate for effective wound management. HHD demonstrates stepwise, glucose- and ROS-responsive drug release. The initially released l-arginine and pyridoxamine improve wound microenvironment by promoting angiogenesis and suppressing advanced glycation end products. Upon infection, this remodeled microenvironment enhances the recruitment

Management of hepatitis B virus coinfection in people with human immunodeficiency virus: Current guidelines, challenges, and future perspectives in Japan.

Hepatitis B virus (HBV) coinfection remains a clinically relevant challenge in people with human immunodeficiency virus (PWH) because of shared transmission routes. Moreover, the recent development of novel antiretroviral therapy (ART) without anti-HBV potency highlights the importance of managing HBV coinfection in this population. This review aims to discuss the epidemiology of HBV coinfection, current recommendations, unresolved challenges, and future perspectives for HBV management in PWH in Japan based on published literature. The prevalence of HBV coinfection among PWH remains substantia

Transcriptional adaptation upregulates utrophin in Duchenne muscular dystrophy.

Duchenne muscular dystrophy (DMD) is a muscle-degenerating disease caused by mutations in the DMD gene, which encodes the dystrophin protein1,2. Utrophin (UTRN), the genetic and functional paralogue of DMD, is upregulated in some DMD patients3-5. To further investigate this UTRN upregulation, we first developed an inducible messenger RNA (mRNA) degradation system for DMD by introducing a premature termination codon (PTC) in one of its alternatively spliced exons. Inclusion of the PTC-containing exon triggers DMD mutant mRNA decay and UTRN upregulation. Notably, blocking nonsense-mediated mRNA

PARP1 contributes to intimal hyperplasia by regulating METTL3-mediated m6A methylation of TRAIL.

Poly (ADP-ribose) polymerase 1 (PARP1) promotes vascular intimal hyperplasia (IH) while contributing to N6-methyladenosine (m6A) methylation regulatory processes. The present study focuses on whether the PARP1 inhibitor PJ34 can improve vascular IH by regulating m6A methylation modification. Mice with femoral artery wire injury-induced IH and platelet-derived growth factor-BB (PDGF-BB)-challenged mouse vascular smooth muscle cells (VSMCs) were utilized in the study. PJ34 treatment significantly alleviated neointimal formation, suppressed VSMC proliferation and phenotypic switching, and reduced

Adenylate kinase 4 (AK4) deficiency prevents vascular smooth muscle cell phenotypic switching by regulating mitochondrial dysfunction through AMPKα inactivation.

BACKGROUND AND AIMS: Mitochondrial dynamics are key mechanism regulating the conversion of vascular smooth muscle cells (VSMCs) from a contractile to a synthetic phenotype, which is involved in neointima formation and restenosis. However, the underlying mechanisms leading to mitochondrial dysfunction are not fully understood. METHODS: Western blot was used to detect the expression of relevant molecules at the protein level. CCK-8, EdU assays, and transwell were used to test cell proliferation and migration capacity. Flow cytometry was used to assess cell cycle and ROS. RESULTS: AK4 was upreg

[Role of the TGF-β1 signaling pathway in modulating in vivo adaptation of vascular grafts].

Intimal hyperplasia and pseudoaneurysm formation after vascular graft implantation are key factors affecting long-term graft patency. The transforming growth factor-beta 1 (TGF-β1) signaling pathway, through Smad-dependent and non-Smad pathways, differentially regulates smooth muscle cell phenotypic switching and proliferation, shapes the immune-inflammatory microenvironment, influences the quality of the re-endothelialization process, and maintains the balance between extracellular matrix synthesis and degradation, thereby playing a central role in the occurrence and progression of graft sten

Clinical effectiveness and implementation outcomes of pMDI-to-DPI switch in children between 5 and 12 years of age: a scoping review protocol.

INTRODUCTION: Inhalers are critical in asthma treatment, and inappropriate inhaler use leads to poor asthma outcomes. In adults and adolescents, dry powder inhalers (DPIs) are safe and effective alternatives to mainstay pressurised metered dose inhalers and could bridge the asthma care gap while also reducing the environmental burden of asthma care. Despite being licensed for use in ages 5 years old and older, the evidence for clinical effectiveness is less clear for patients between ages 5 and 12 years. This protocol describes a scoping review. The primary aim of the review is to identify and

Engineering a Hydrogen Peroxide-Activated Hydrogen Sulfide Donor-Based Fluorescent Agent for Integrated Diagnosis and Therapy of Chronic Wounds.

Diabetic chronic wounds are one of the most severe complications of diabetes mellitus, which are pathologically characterized by persistent oxidative stress-inflammatory cascades. Thus, it is vital for effective diagnosis and treatment of diabetic chronic wounds by targeting the critical pathological oxidative stress feature in the tissue microenvironment. Herein, regarding the overexpressed hydrogen peroxide (H2O2) at the oxidative stress sites, we innovatively developed a H2O2-activated hydrogen sulfide (H2S) donor-based near-infrared fluorescent theragnostic agent (NDTA) to achieve synergis

Soyasapogenol B prevents sarcopenia by increasing skeletal muscle mass and function through the Sirt1/PGC-1α and PI3K pathway.

Sarcopenia, caused by aging, is characterized by the reduction of muscle mass and function. In this study, we investigated the effects of soyasapogenol B on skeletal muscle and the underlying mechanisms to determine its potential as a prevention for sarcopenia. Soyasapogenol B, a natural triterpenoid found in soybeans, has biological effects that inhibit cancer, inflammation, and obesity; however, its effects on skeletal muscle remain unclear and require further investigation. C57/BL6 mice were fed soyasapogenol B for 8 weeks, after which skeletal muscle mass, function, and protein analysis fo
