Potent antimycobacterial activity of mouse secretory leukocyte protease inhibitor.

Secretory leukocyte protease inhibitor (SLPI) has multiple functions, including inhibition of protease activity, microbial growth, and inflammatory responses. In this study, we demonstrate that mouse SLPI is critically involved in innate host defense against pulmonary mycobacterial infection. During the early phase of respiratory infection with Mycobacterium bovis bacillus Calmette-Guérin, SLPI was produced by bronchial and alveolar epithelial cells, as well as alveolar macrophages, and secreted into the alveolar space. Recombinant mouse SLPI effectively inhibited in vitro growth of bacillus C

Management of empyema thoracis at Lusaka, Zambia.

Of the 39 consecutive patients with empyema thoracis managed by one of the five general surgical units at Lusaka, Zambia, 26 suffered from human immunodeficiency virus (HIV) infection and 19 were diagnosed as suffering from pulmonary tuberculosis within 3 years of developing empyema thoracis. Thirty patients were between 16 and 40 years of age; of these, 22 suffered from acquired immune deficiency syndrome (AIDS) and all 19 patients with pulmonary tuberculosis belonged to this age group. Of the four patients with empyema thoracis in the age group of 0-5 years, two were suffering from AIDS. The

Uncommon presentation of gastrointestinal tuberculosis-A case series.

Tuberculosis (TB) is a common disease in underdeveloped countries. Of all tuberculosis cases worldwide, 28% were reported from India. The most prevalent site of infection is the lungs (pulmonary TB), whereas extrapulmonary tuberculosis (EPTB) is less common. The most common form of EPTB in India is Lymph Node TB, which accounts for around 35% of total EPTB cases. Splenic tuberculosis incidence is 8 % whereas tuberculosis of pancreas and gall bladder is extremely rare. Pancreatic and gallbladder tuberculosis (GT) is extremely rare and presents with non-specific features. The clinical manifestat

Isolated primary tuberculosis of spleen-A rare entity in the immuno-competent patient.

INTRODUCTION: Tuberculosis is a global public health concern, with 9.6 million affected individuals worldwide. Current screening and diagnostic regimes focus primarily on smear positivity, and hence, the rising numbers of Sputum negative and Extra-Pulmonary Tuberculosis has become a significant set-back to adequate diagnosis, disease notification and treatment, due to the large number of false negatives. PRESENTATION OF CASE: We hereby describe an intriguing presentation of tuberculosis - A 23 yr old lady with no comorbid illness, came to us with ten month history of on and off pyrexia, weakn

[Primary prostatic tuberculosis. Case report and bibliographic review].

OBJECTIVE: Primary prostatic tuberculosis is a very rare form of presentation of the tuberculous infection, which is generally caused by the M. tuberculosis, and which has shown an increase in incidence and prevalence, due to an increase of immunocompromised patients and the pandemic of the Syndrome of Acquired Immune Deficiency (AIDS).We describe a case of primary prostatic tuberculosis attended at the Hospital Universitario de Santander, Colombia, and to perform a discussion about this topic. METHODS/RESULTS: 65 year old man who consulted with symptoms of frequency, dysuria and hesitancy, a

Tuberculosis and diabetes in southern Mexico.

OBJECTIVE: To determine the impact of diabetes on the rates of tuberculosis in a region where both diseases are prevalent. RESEARCH DESIGN AND METHODS: Data from a population-based cohort of patients with pulmonary tuberculosis undergoing clinical and mycobacteriologic evaluation (isolation, identification, drug-susceptibility testing, and IS6110-based genotyping and spoligotyping) were linked to the 2000 National Health Survey (ENSA2000), a national probabilistic, polystage, stratified, cluster household survey of the civilian, noninstitutionalized population of Mexico. RESULTS: From March

[Choroidal granuloma revealing tuberculosis. A case report].

BACKGROUND: The incidence of tuberculosis is still high in developing countries and a steady increase in new cases has been observed in industrial countries within the past decade. Among other reasons, the growing number of immunodeficient patients and migration from developing to industrial countries are both contributing factors. OBSERVATION: A 70-year-old female patient presented with a marked decrease in visual acuity of the left eye. There was no history of tuberculosis. Funduscopy of the left eye revealed a choroidal tumor with multiple choroidal lesions, mostly located at the posterior

Is the BCG test of diagnostic value in tuberculosis?

SETTING: In developing countries including Turkey, tuberculosis is still a major problem. Rapid diagnosis and early medical intervention are the two most important considerations in preventing the spread of the disease. OBJECTIVE: This study was carried out to determine the diagnostic value of BCG test in childhood tuberculosis and compare it with tuberculin test in this regard. DESIGN: 50 patients and 20 healthy children without any evidence of previous BCG vaccination and aged 80 days-15 years were simultaneously tested with purified protein derivative (PPD) and BCG vaccine. RESULTS: In p

Pulmonary hydatic disease presenting as solid mass with haemoptysis in a child.

A middle childhood HIV-negative female patient presented with three episodes of haemoptysis. The chest X-ray demonstrated an oval-shaped, well-circumscribed left upper lobe homogenous opacification. She did not respond to tuberculosis treatment. A left upper lobectomy was performed for a solid mass in the lung, and hydatic disease was histologically confirmed. Calcification was found in the pulmonary lesion. Pulmonary hydatic cyst rarely presents as a solid lesion with calcifications and haemoptysis.

[Neurosarcoidosis as the first manifestation of sarcoidosis: diagnosis difficulty. A case report].

Clinically apparent involvement of the nervous system occurs in a relatively small number of patients with sarcoidosis. The diagnosis of neurosarcoidosis is often difficult and particularly in patients who lack either pulmonary or systemic manifestations of sarcoidosis. Furthermore clinical and imaging features of neurosarcoidosis are extremely variable. We report a case of neurosarcoidosis which had been considered and treated at first as tuberculosis. Only the occurrence, two years later, of pulmonary manifestations of sarcoidosis enabled the diagnosis.

Disseminated Tuberculosis in a Nigerian Adolescent with Linear IgA Bullous Dermatosis: A Case Report and Review of Literature.

Linear IgA bullous dermatosis (LABD) is an auto-immune disease affecting young children and adults, characterized by the linear deposition of IgA at the basement membrane zone with resultant complement activation and a cascade of immune reactions. There is a loss of adhesion at the dermo-epidermal junction and subsequent blister formation. It is a rare disease that has a good prognosis with adequate therapy. However, the underlying depressed immunity associated with the disease may expose them to such infections as tuberculosis. We report the case of an 11-years-old Nigerian female adolescent

Peritoneal tuberculosis.

The peritoneum is one of the locations outside the most common pulmonary tuberculosis. Peritoneal tuberculosis poses a public health problem in endemic regions of the world. The phenomenon of migration, the increased use of immunosuppressive therapy and the epidemic of AIDS have contributed to a resurgence of this disease in regions where it was previously controlled. The aim of this review is to expose the clinical, biologic end radiologic futures of the peritoneal tuberculosis and to present the methods of diagnosis and treatment. The diagnosis of this disease is difficult and still remains

Silicotuberculosis in the elderly: report of two cases.

Silicotuberculosis is observed rarely in the current clinical practice. We present two patients (a 72-year-old man and a 84-year-old woman) who developed silicosis after having worked for several decades in the ceramics industry. In both, pulmonary tuberculosis complicated the clinical picture several years after retirement. The first subject presented a multicavitary lesion in the apex of the right lung, which subsequently evolved with fibrosis. The other developed bilateral tubercular bronchopneumonia and right tubercular pleurisy, that improved after prolonged antimycobacterial polychemothe

Silicosis and tuberculosis: Part 2--a radiographic presentation of nodular tuberculosis and silicosis.

Indolent nodular pulmonary tuberculosis (TB) in workers exposed to silica dust may go undetected clinically and radiographically, especially in the absence of identification of tubercle bacilli in sputum. Illustrative cases demonstrating the radiographic manifestations of coexistent pulmonary silicosis and the indolent form of nodular TB are presented. Alterations in the usual chronologic progress, a rapid advance in nodular profusion or size outside the expected time frame, and distinct pattern alterations are features indicating the presence of TB associated with silicosis.

Sarcoidosis presenting with severe haemoptysis.

Major haemoptysis is a very uncommon presentation of sarcoidosis inspite of the high rate and extent of pulmonary involvement in this disease. We report a 37-year-old Nigerian who presented with cough and weight loss and in whom haemoptysis was prominent. In developing tropical countries this mode of presentation is almost invariably due to pulmonary tuberculosis. In this case the haemoptysis was found to be due to ulcerations of nasopharyngeal granulomata due to sarcoidosis. Involvement of the upper respiratory tract in itself is an uncommon finding in sarcoidosis.
