[Acute myocardial injury and repeated angina pectoris-like attacks in a young patient with Churg-Strauss syndrome].

A 23-year-old male with bronchial asthma developed eosinophilia (eosinophils greater than 2,000/mm3) and was observed at our hospital. After using a prescribed indomethacin suppository for fever at home, he experienced an attack of acute chest pain and severe dyspnea. He suffered cardiac arrest while being transferred to the ward. After resuscitation, he was diagnosed as having acute myocardial infarction on the basis of electrocardiographic and ultrasonic cardiographic findings, and marked elevation of serum concentrations of myocardial enzymes. Thereafter, he often complained of precordial p

The spectrum of animal related injuries managed at a major trauma centre in South Africa.

BACKGROUND: Humans come into contact and interact with an array of animals in a number of areas and environments. We set out to review our experience with animal-related injuries in Pietermaritzburg, KwaZulu-Natal, South Africa. METHOD: All patients who sustained an injury secondary to an interaction with an animal in the period December 2012-December 2017 were identified from the Hybrid Electronic Medical Registry (HEMR). RESULTS: There were 104 patients in the study sample. The mean age of patients in the study was 32.8 years, with a range from 1 to 76 years old. 75% (n = 78) were male and

Cause of a rare acute renal insufficiency: rupture aortocaval fistula.

INTRODUCTION: Abdominal aortic aneurysms can be complicated by some conditions and aortocaval fistula is one of them. Aortocaval fistula is an unusual entity. CASE REPORT: A male patient was admitted to the hospital for abdominal pain and leg oedema of three days duration. In addition, severe anuria was also noted. We determined a fistula from the right lateral wall of aneurysm to the distal of vena cava inferior. The aortocaval fistula was closed with pledged-reinforced 4/0 polypropylene suture in the aneurysmal wall. At the postoperative 10th day, he was discharged with normal renal functio

Severe celiac trunk stenosis following blunt trauma in a patient with diabetic ketoacidosis: a diagnostic dilemma of traumatic injury versus median arcuate ligament syndrome.

INTRODUCTION: Severe proximal celiac trunk stenosis or occlusion following blunt trauma is exceedingly rare, with potential etiologies including acute traumatic vascular injury, thrombosis, and underlying median arcuate ligament syndrome (MALS). In patients complicated by type 1 diabetic ketoacidosis (DKA), overlapping symptoms such as nausea and vomiting may arise from both metabolic derangement and visceral hypoperfusion, frequently obscuring the underlying vascular pathology and complicating diagnostic decision-making. METHODS: We report the case of a 33-year-old man with poorly controlled

Effect of local ozone treatment on rats with anterior rectal resection and the possible mechanisms.

BACKGROUND: Anterior resection syndrome (ARS) is characterized by the diverse and interchangeable evacuatory symptoms that may occur following distal colorectal resection. We aimed to investigate the effect and potential mechanisms of ozone perfusion on rats with anterior rectal resection (ARR). MATERIAL AND METHODS: After establishment of rat ARR model, 20, 40 and 80 ug/ml ozone was used to treat rats by enema administration. The pathological examination of intestinal tissue was detected using hematoxylin-eosin staining. The rate of loose stools, minimum threshold volume of abdominal withdra

TLR4-HMGB1-, MyD88- and TRIF-dependent signaling in mouse intestinal ischemia/reperfusion injury.

AIM: To characterize high-mobility group protein 1-toll-like receptor 4 (HMGB1-TLR4) and downstream signaling pathways in intestinal ischemia/reperfusion (I/R) injury. METHODS: Forty specific-pathogen-free male C57BL/6 mice were randomly divided into five groups (n = 8 per group): sham, control, anti-HMGB1, anti-myeloid differentiation gene 88 (MyD88), and anti-translocating-chain-associating membrane protein (TRIF) antibody groups. Vehicle with the control IgG antibody, anti-HMGB1, anti-MyD88, or anti-TRIF antibodies (all 1 mg/kg, 0.025%) were injected via the caudal vein 30 min prior to isc

Abdominal apoplexy complicating ulcerative colitis: a case report of venous mesenteric hemorrhage.

INTRODUCTION: Abdominal apoplexy, or idiopathic spontaneous intraperitoneal hemorrhage, is a rare and potentially fatal vascular emergency defined by spontaneous hemoperitoneum in the absence of trauma, iatrogenic injury, or identifiable intra-abdominal pathology. Fewer than 200 cases have been reported in the literature, and venous-predominant bleeding without an identifiable arterial source represents a particularly uncommon and diagnostically challenging subset. CASE PRESENTATION: A 52-year-old male with a background of left-sided ulcerative colitis (UC) presented with sudden-onset general

Peri-Operative Fall in Serum Albumin Levels Correlate Well With Outcomes in Children Undergoing Emergency Abdominal Surgery: A Prospective Study From a Resource-Limited Setting.

Background Albumin is a negative acute-phase protein as its levels fall after injury, sepsis, and surgical stress. A review of the literature suggests that serum albumin level drops rapidly after surgery in adults and correlates well with the outcomes. However, there is limited data on the use of peri-operative fall in serum albumin levels as an outcome predictor in children undergoing emergency abdominal surgeries. We aim to investigate the correlation between the degree of peri-operative fall in serum albumin levels and the outcomes in children undergoing emergent abdominal surgeries. Materi

Gastric fistula after 20 months of postoperative drain retention: a case report.

BACKGROUND: In trauma or emergency surgery, postoperative drainage may be required for a prolonged period in selected patients because of complex injuries, contamination, or postoperative complications. However, visceral erosion, drain migration, and fistula formation related to long-term drain retention are rare but potentially serious. CASE PRESENTATION: In April 2024, a 32-year-old man underwent emergency abdominal surgery after a traffic accident, including laparoscopic cholecystectomy and conversion to open splenectomy because of intraoperative bleeding, and a drainage tube was placed in

[The preclinical care of polytraumatized patients].

In industrially developed countries, trauma is the major mortality factor for people younger than 40 years. The preclinical management of polytraumatized patients influences the prognosis of mortality and morbidity. In this period, a number of decisions have to be made under unfavourable conditions and with limited time. This situation represents a great challenge for the whole rescue team. Diagnostic overview, protection of the vital functions under the special situation of shock, immobilization of the spine and the treatment of the isolated injuries are part of the preclinical management eff

Both primary and secondary abdominal compartment syndrome can be predicted early and are harbingers of multiple organ failure.

BACKGROUND: Primary abdominal compartment syndrome (ACS) is a known complication of damage control. Recently secondary ACS has been reported in patients without abdominal injury who require aggressive resuscitation. The purpose of this study was to compare the epidemiology of primary and secondary ACS and develop early prediction models in a high-risk cohort who were treated in a similar fashion. METHODS: Major torso trauma patients underwent standardized resuscitation and had prospective data collected including occurrence of ACS, demographics, ISS, urinary bladder pressure, gastric tonometr

Safety and feasibility of open abdomen with negative pressure therapy in major liver trauma: a retrospective bicenter study.

BACKGROUND: Abdominal trauma is the third leading cause of death in the young population, and liver trauma is among the most common. For major liver injury, perihepatic packing (PHP) is a life-saving procedure that allows rapid control of the hemorrhage. The use of the open abdomen (OA) represents a strategy for the management of major abdominal trauma. However, the effects of combined use with Negative Pressure Therapy (NPT) in patients with liver trauma are not yet clear. The aim of our study was to evaluate the safety and feasibility of OA with NPT. MATERIALS AND METHODS: This retrospectiv

Strategy for Totally Percutaneous Management of Vascular Injury in Combined Transfemoral Transcatheter Aortic Valve Replacement and Endovascular Aortic Aneurysm Repair Procedures.

Minimally invasive treatment of severe aortic stenosis by transcatheter aortic valve replacement (TAVR) and infrarenal abdominal aortic aneurysm by endovascular aortic aneurysm repair (EVAR) requires large-bore vascular access. These percutaneous transfemoral interventions may be performed as a combined procedure, however, vascular injury may necessitate surgical vascular repair. We implemented a strategy designed to enable percutaneous vascular repair, with stent-graft implantation, if necessary, after these combined procedures. We identified all combined percutaneous TAVR and EVAR procedures
