Drug delivery to the nail following topical application.

The absorption of drugs into the nail unit, following topical application to the nail plate, is highly desirable to treat nail disorders, such as onychomycosis (fungal infections of the nail). Nail permeability is however quite low and limits topical therapy to early/mild disease states. In this paper, the recent research into ungual drug delivery is reviewed. The nail unit and the two most common diseases affecting the nail--onychomycosis and nail psoriasis--are briefly described to set the scene and to give an overview of the nature and scope of the problem. The factors, which affect drug up

Laser therapy of stretch marks.

Striae distensae, better known as stretch marks, are a common disfiguring skin disorder of significant cosmetic concern. Many sources have reported the use of lasers to diminish the appearance of striae. Controlled clinical studies of the various treatment modalities available for striae are relatively uncommon, and much of the clinical data are anecdotal. The use of lasers alone or in combination with other therapeutic modalities can provide a safe and effective reduction in the appearance of both red and white striae distensae. Many of these therapies require special measures for darker skin

Intranasal corticosteroids for allergic rhinitis.

Intranasal corticosteroids are accepted as safe and effective first-line therapy for allergic rhinitis. Several intranasal corticosteroids are available: beclomethasone dipropionate, budesonide, flunisolide, fluticasone propionate, mometasone furoate, and triamcinolone acetonide. All are efficacious in treating seasonal allergic rhinitis and as prophylaxis for perennial allergic rhinitis. In general, they relieve nasal congestion and itching, rhinorrhea, and sneezing that occur in the early and late phases of allergic response, with studies showing almost complete prevention of late-phase symp

Endoscopic therapy with absolute ethanol for postoperative recurrent ulcers in intestinal Behçet's disease, and simple ulcers.

BACKGROUND: When examined macroscopically and histologically, there are no differences between "simple" intestinal ulcers and intestinal ulcers of Behçet's disease. The postoperative recurrence of intestinal Behçet's disease and simple ulcers was frequently. Medical treatment for postoperative recurrent ulcers is palliative, and the patient often requires surgery. We studied about an effect of absolute ethanol spraying via endoscopy for the ulcers. METHOD: The postoperative rate of recurrence was 56% (9/16). Commonly the ulcer recurred around the anastomotic region. We performed spraying for

[Therapeutic efficacy of a topical tolnaftate preparation in guinea pig model of tinea pedis].

The therapeutic efficacy of a topical antifungal ointment containing 2% tolnaftate was studied in a guinea pig model of tinea pedis using the following four topical antifungal preparations commercially available as reference drugs: variotin (3,000 U/g ointment); phenyl-11-iodo-10-undecynoate (0.5% ointment); siccanin (1% ointment); and clotrimazole (1% cream). After the infection fully developed, the infected animals were treated twice daily with the testing drug or reference drug for consecutive four weeks. Therapeutic efficacy was mainly evaluated on the basis of the extent of the yield of f

[Radiation-induced skin toxicities: prevention, treatment].

Acute and long term effects are frequent after radiotherapy. They may alter the general status and quality of life of the patients. Chronic radiodermatitis may result in ulceration and in transformation into a squamous cell carcinoma. There is a correlation of the frequency of acute dermatitis with the total dose. Chronic radiodermatitis may develop after repeated small doses of ionizing radiation for cardiac catheterization and coronary angioplasties. The other prognostic factors for the level of acute and late skin reactions are volume of tissue treated, total daily dose, fractionactions sch

Recurrent aphthous stomatitis.

Etiology and Epidemiology: The Greek term aphthai was initially used in relation to disorders of the mouth and is credited to Hippocrates (460-370 BC). Today, recurrent aphthous ulceration, or recurrent aphthous stomatitis (RAS), is recognized as the most common oral mucosal disease known to human beings. Considerable research attention has been devoted to elucidating the causes of RAS; local and systemic conditions, and genetic, immunologic, and infectious microbial factors all have been identified as potential etiopathogenic agents (Table 1). However, to date, no principal etiology has been

Topical use of papaya in chronic skin ulcer therapy in Jamaica.

The objective of the study was to determine the prevalence of the use of the fruit (papaya) of Carica papaya as topical ulcer dressings by registered nurses in the Spanish Town Hospital (STH), Kingston Public Hospital (KPH) and the University Hospital of the West Indies (UHWI), Jamaica. A ten-item pretested self-administered questionnaire was distributed to 285 randomly selected registered nurses at the UHWI, KPH and STH. There was a 72% response rate. The prevalence of topical papaya use among the respondents was 75%. Comments from the users of papaya suggested that topical application of the

[Nasal polyposis--of interest to several specialties. Improved diagnosis and new therapeutic possibilities].

Nasal polyposis, a disease seen mainly in humans and dominated in most cases by an eosinophilic inflammation of unknown etiology, has been an enigma for a few thousand years. Polyps are, however, overrepresented in patients with general airway inflammation as seen in asthma and idiopathic rhinitis, and in patients with infective nasal disease of chronic origin as seen in cystic fibrosis, immune deficiencies and dental sinusitis. Due to the eosinophilic nature of the inflammation, polyps are on the whole sensitive to local steroid treatment with spray or drops. Surgery is indicated when the pol

Pyrethroid-induced paresthesia--a central or local toxic effect?

BACKGROUND: Pyrethroid-induced paresthesia is frequently seen after dermal exposure to pyrethroids. Affected individuals experience a sensation of burning, tingling, itching, or numbness, most commonly in the face. This occurs 1-2 hours after the beginning of exposure and resolves spontaneously. MECHANISMS: Paresthesia occurs as a result of a direct effect on intracutaneous nerve endings at very low pyrethroid doses. It is related to potency of the pyrethroid with pyrethroids without an alpha-cyano group generally showing the weakest effect. CONCLUSION: Doses sufficient to cause paresthesia

Managing the transition to CFC-free inhalers.

Asthma inhalers which use CFC propellants will be replaced by CFC-free devices within three years. Research suggests that inflammation in the small airways is often inadequately treated. CFC-free steroid-based inhalers appear to increase total lung deposition and peripheral lung penetration.

Preservation of function and histologic appearance in the injured glottis with topical mitomycin-C.

OBJECTIVE: To evaluate the functional and histological effects of a single application of topical mitomycin-C after laser injury in the posterior canine glottis. STUDY DESIGN: A prospective, randomized study of 16 canines. METHODS: A supersaturated (1%) solution of topical mitomycin-C was applied to a unilateral, laser-induced injury near the cricoarytenoid joint in eight dogs. The mitomycin-soaked pledget was placed immediately after induction of the injury and was left in contact with exposed cartilage for 3 minutes. The opposite side was not injured to provide an internal control. In eigh

[Local corticosteroid therapy in dermatology].

UNLABELLED: FOUR LEVELS OF ACTION: The introduction of dermocorticosteroids (corticosteroids for topical application) has revolutionized management of many skin diseases. Depending on their antiinflammatory action, these products can be classed into four levels of action. Dermocorticosteroids have antiinflammatory, vasoconstrictor, antiproliferative, antisynthetic and immunosuppressive actions. SKIN PENETRATION: Penetration depends on the intrinsic characteristics of the drug, but also on many other factors including the nature of the excipient, additives, occlusion, localization, nature of th
