Systemic Psoriasis: From Molecular Mechanisms to Global Management Strategies.

Psoriasis is a chronic, immune-mediated inflammatory disorder marked by a complex interplay between genetic predisposition, cytokine dysregulation, and environmental triggers. Originally perceived as a superficial dermatological condition, it is now recognized as a systemic disease with far-reaching health implications. Advances in molecular genetics have uncovered over 80 susceptibility loci, with key variants such as HLA-C*06:02, IL23R, and CARD14 contributing to the multifactorial nature of the disorder. Central to its pathogenesis is the aberrant activation of the IL-23/Th17 axis, resultin

Diaphragm muscle: a pump that can not fail.

In mammals, breathing requires an inspiratory pump to generate a negative intrathoracic pressure and thus pull air into the lungs for gas exchange. Exclusively in mammals, the diaphragm muscle (DIAm) is the major inspiratory pump, which separates the thoracic and abdominal cavities. With DIAm contraction, a negative intrathoracic and positive abdominal pressure are generated [i.e., transdiaphragmatic pressure (Pdi)]. During breathing, the DIAm is very active, with a duty cycle (time active vs. inactive) similar to that of the heart. Like the heart, this is a pump that cannot fail! Thus, in con

Tailorable Ultrathin Copper Oxysulfide for Room-Temperature, Reversible, and Selective Hydrogen Sulfide Sensing.

Accurate, low-cost, and energy-efficient detection of hydrogen sulfide (H2S) is vital for industries such as petroleum, natural gas, and wastewater treatment. While chemiresistive sensors are well suited for this purpose, traditional metal oxides typically require high operating temperatures (>100 °C) or external stimuli (e.g., UV light) for activation. In this work, we introduce two-dimensional (2D) copper oxysulfide nanoflakes (∼10 nm thick) as a novel material for room-temperature, reversible, and selective H2S sensing. These 2D copper oxysulfides, synthesized via the calcination of copper

Amine-functionalized polysiloxane-engineered cellulose foam with Ag/MnO2 schottky interfaces for synergistic adsorption-photocatalytic formaldehyde elimination.

Existing formaldehyde treatment systems are generally limited by the "adsorption-catalysis" interface kinetic barriers when coupling the dual functions of adsorption and photocatalysis. Pollutants captured at the adsorption sites tend to accumulate on the material surface due to the absence of a directional migration pathway, leading to the passivation of active sites. Simultaneously, catalytic sites suffer from low free radical utilization efficiency due to an insufficient supply of reactants, ultimately resulting in a vicious cycle of "adsorption saturation-catalysis stagnation". This study

Solution Blow Spun Mats with Beaded-Fiber Morphologies as a Drug Delivery System with Potential Use for Skin Wound Dressing.

The regeneration of skin injuries can be aided by tissue engineering strategies, which enable the recovery of the structural and functional integrity of the damaged tissue. The Solution Blow Spinning (SBS) technique has attracted the attention of researchers due to the production of nanofiber mats in a continuous process, which exhibit high porosity and the ability to deliver drugs locally. The objective of this work was to produce and encapsulate ibuprofen in mats of PCL/PEG as a fast-acting analgesic drug delivery system. Initially, beaded nanofiber structures were produced from PCL solution

Chemical injury history increases pulmonary susceptibility to PM2.5 inhalation: Toxicological evidence for risk assessment in a neglected vulnerable population.

Environmental health risk assessments typically prioritize vulnerable populations such as the elderly, children, and individuals with respiratory diseases. However, individuals with a history of chemical-induced lung injury are rarely considered, despite potentially heightened sensitivity to air pollutants. Therefore, this study investigated the pulmonary health risks of particulate matter (PM) 2.5 exposure in a chemical lung injury model, reflecting realistic exposure conditions. Mice were intratracheally administered polyhexamethylene guanidine phosphate (PHMG-P; 0.3 mg/kg), followed by nose

Beyond deterministic air quality modeling: a probabilistic screening approach for emission inputs in AERMOD.

Traditional air dispersion modeling usually relies on deterministic frameworks that use multiple conservative assumptions as inputs. For example, atmospheric pollutant concentrations are often overestimated by using maximum emission rates for point sources, an approach recommended by regulatory agencies that may not reflect typical operating conditions, especially for sources with variable emissions. To better understand how emission variability affects modeled pollutant concentrations, this study presents a novel probabilistic modeling framework designed to estimate pollutant concentrations f

Synergistic Effect of Metal-Organic Frameworks and Metal Oxides for Ultrasensitive and ppb-Level NO2 Detection at High Humidity.

Efficient NO2 gas sensors are critical for environmental monitoring and industrial safety. However, the design of reliable NO2 sensing materials with high activity and moisture resistance remains a challenge, primarily due to limited active sites and interference from high air humidity. In this paper, SnO2/ZIF-8 nanocomposites were prepared by leveraging the inherent moisture resistance and abundant active sites of ZIF-8 nanoparticles, as well as the good electronic migration capabilities of SnO2 nanospheres. The synergistic effect between metal-organic frameworks and metal oxides enables the

Nutritional Support of Chronic Obstructive Pulmonary Disease.

Background: COPD is a heterogenous disease of the respiratory tract caused by diverse genetic factors along with environmental and lifestyle-related effects such as industrial dust inhalation and, most frequently, cigarette smoking. These factors lead to airflow obstruction and chronic respiratory symptoms. Additionally, the increased risk of infections exacerbates airway inflammation in COPD patients. As a consequence of the complex pathomechanisms and difficulty in treatment, COPD is among the leading causes of mortality both in the western countries and in the developing world. Results: The

Successful management of left-sided thoracic impalement in a resource-limited setting: A case report.

INTRODUCTION: Thoracic impalement injuries are rare and life-threatening. Managing them in resource-limited settings poses significant challenges. CASE PRESENTATION: A 63-year-old male fell 3 m from a tree, sustaining a left-sided chest impalement at the 7th intercostal space along the mid-axillary line. On arrival, his vitals were BP 100/70 mmHg, pulse 104 bpm, RR 32/min, and SpO₂ 78 % (improving to 90 % with oxygen). Examination revealed absent air entry on the left, a sucking chest wound, and an impaled wooden fragment. Initial management included resuscitation, antibiotics, and tetanus pr

Airflow Determines Natural Airborne Transmission of Tuberculosis in a Guinea Pig Model.

Tuberculosis (TB) spreads through the air when Mycobacterium tuberculosis (Mtb) passes from infected to susceptible hosts, yet the environmental, biophysical and microbial factors governing this process remain poorly understood. In the early twentieth century, Perla and Lurie used guinea pigs to demonstrate natural airborne transmission of Mtb, but such studies have not been revisited in the modern biosafety era. Here, we developed an aerodynamically-optimized guinea pig housing system that models natural, airborne, animal-to-animal Mtb transmission under biosafety level 3 (BSL-3) containment.

Extensive Surgical Emphysema and Pneumomediastinum Following Chest Drain Insertion in a Patient With Bullous Chronic Obstructive Pulmonary Disease (COPD): A Case Report.

Subcutaneous (surgical) emphysema and pneumomediastinum are recognized complications of thoracic procedures. They are often self-limiting but may occasionally indicate serious underlying pathology or precipitate life-threatening deterioration, particularly in patients with chronic obstructive pulmonary disease (COPD) and bullous lung disease. We present the case of an 87-year-old male patient with known COPD who was admitted with worsening dyspnoea and a productive cough. Imaging revealed a large left-sided pneumothorax, and emergency needle decompression followed by intercostal chest drain in

Tension pneumothorax from large bowel herniation and perforation as a late presentation of traumatic diaphragmatic hernia during pregnancy: a case report.

BACKGROUND: Diaphragmatic hernias can be congenital or acquired, with trauma being the primary cause of the latter. Both types may have delayed presentations, with abdominal organs protruding into the thoracic cavity, causing symptoms of varying severity. Pregnancy can sometimes precipitate the condition. Tension pneumothorax resulting from bowel perforation into the thorax is exceptionally rare, with only a few cases reported. To the best of the authors knowledge, this is the third documented case of a late-presenting trauma-related diaphragmatic hernia during pregnancy, complicated by tensio

Bezold's Abscess Secondary to a Subclinical Mastoid Infection: Diagnostic Challenges and Literature Review.

Bezold's abscess is a rare but serious extracranial complication of otitis media, typically resulting from erosion of the mastoid tip. We report the case of a 34-year-old male with a two-month history of recurrent right-sided otalgia, occurring two to three times and each treated with short courses of oral antibiotics, without any ear discharge, who presented with a progressively enlarging, painful, right cervical swelling and low-grade fever. The recurrent mild otologic symptoms, likely the result of repeated antibiotic exposure, delayed recognition of the more serious underlying mastoiditis.

Lassa Virus Infection of Primary Human Airway Epithelial Cells.

Lassa mammarenavirus (LASV), a member of the family Arenaviridae, is a highly pathogenic virus capable of causing severe systemic infections in humans. The primary host reservoir is the Natal multimammate mouse (Mastomys natalensis), with human infections typically occurring through mucosal exposure to virus-containing aerosols from rodent excretions. To better understand the molecular mechanisms underlying LASV replication in the respiratory tract, we utilized differentiated primary human airway epithelial cells (HAECs) grown under air-liquid interface conditions, closely mimicking the bronch

Assessment of In Vitro Models of the Human Buccal Mucosa for Vaccine and Adjuvant Development.

To understand requirements for immunization via the oral mucosa, an in vitro model that recapitulates the physical barrier of the mouth, allows for quantification of antigen uptake and permeability and mounts an inflammatory response to antigen and adjuvant is needed. The physical structure of 4 models of the human oral mucosa was determined by histochemical staining and transepithelial electrical resistance (TEER) measurements. A TR146 based air-liquid interface (ALI) model most closely mimicked in vivo conditions. This was confirmed by validation studies using dextran and caffeine as diffusa

Skin irritation testing using human iPSCs derived 3D skin equivalent model.

Artificial skin models have emerged as valuable tools for evaluating cosmetic ingredients and developing treatments for skin regeneration. Among them, 3D skin equivalent models (SKEs) using human primary skin cells are widely utilized and supported by standardized testing guidelines. However, primary cells face limitations such as restricted donor availability and challenges in conducting genotype-specific studies. To overcome these issues, recent approaches have focused on differentiating skin cells from human-induced pluripotent stem cells (hiPSCs). In this study, we developed a protocol to

The two-stage detection-after-segmentation model improves the accuracy of identifying subdiaphragmatic lesions.

Chest X-rays (CXRs) are primarily used to detect lung lesions. While the abdominal portion of CXRs can sometimes reveal critical conditions, research in this area is limited. To address this, we introduce a two-stage architecture that separates the abdominal region from the CXR and detects abdominal lesions using a specialized dataset. We compared the performance of our method on whole CXRs versus isolated abdominal regions. First, we created masks for the right upper quadrant (RUQ), left upper quadrant (LUQ), and upper abdomen (ABD) regions and trained corresponding segmentation models for ea
