A Pilot Study of Musculoskeletal Abnormalities in Patients in Recovery from a Unilateral Rupture-Repaired Achilles Tendon.

The purpose of this study was to compare the inter-limb joint kinematics, joint moments, muscle forces, and joint reaction forces in patients after an Achilles tendon rupture (ATR) via subject-specific musculoskeletal modeling. Six patients recovering from a surgically repaired unilateral ATR were included in this study. The bilateral Achilles tendon (AT) lengths were evaluated using ultrasound imaging. The three-dimensional marker trajectories, ground reaction forces, and surface electromyography (sEMG) were collected on both sides during self-selected speed during walking, jogging and runnin

Non-operative treatment of unicompartmental osteoarthritis of the knee: a prospective randomized trial with two different braces-ankle-foot orthosis versus knee unloader brace.

BACKGROUND: The use of an unloader brace is a non-surgical treatment option for patients with medial osteoarthritis (OA). However, many patients do not adhere to brace treatment, because of skin irritation due to the pads at the level of the joint space and bad fit. A new concept to unload the medial compartment of the knee is a foot ankle brace with a lever arm pressing the thigh in valgus. The aim of this prospective randomized trial was to examine the outcomes of patients with medial OA after treatment with a conventional knee unloader brace (Unloader One®) and the new foot ankle orthosis (

Arthroscopic versus open ankle arthrodesis.

BACKGROUND: It is thought that arthroscopic ankle fusion offers improved outcomes over open fusion in terms of functional outcomes, time to fusion, length of stay and fewer complications. However, there are doubts about whether correction of established severe deformity can be achieved using the arthroscopic approach. METHODS: A retrospective review of medical records and radiographs at our hospital identified consecutive tibio-talar ankle fusions between April 2009 and March 2014 with minimum 1 year follow up. Records were scrutinised for type of arthrodesis, demographics, length of stay (LO

Increased cytokine levels and histological changes in cartilage, synovial cells and synovial fluid after malleolar fractures.

BACKGROUND: Malleolar fractures are among the most common fractures in the human skeleton with a high risk of later development of post-traumatic osteoarthritis (OA). The acute ankle injury initiates a sequence of events potentially leading to progressive articular surface damage resulting from inflammatory changes in cartilage, synovial tissue and synovial fluid. We hypothesised that in the acute phase of ankle fracture, these changes occur at the same time in the different tissues. METHODS: Specimens of chondral tissue, synovial tissue and synovial fluid were collected from 16 patients with

Joint-level mechanics of the walk-to-run transition in humans.

Two commonly proposed mechanical explanations for the walk-to-run transition (WRT) include the prevention of muscular over-exertion (effort) and the minimization of peak musculoskeletal loads and thus injury risk. The purpose of this study was to address these hypotheses at a joint level by analysing the effect of speed on discrete lower-limb joint kinetic parameters in humans across a wide range of walking and running speeds including walking above and running below the WRT speed. Joint work, peak instantaneous joint power, and peak joint moments in the sagittal and frontal plane of the ankle

Longitudinal effects of maturation on lower extremity joint stiffness in adolescent athletes.

BACKGROUND: Yearly changes in active joint stiffness may help explain when neuromuscular sex differences emerge in adolescent athletes that may relate to increased anterior cruciate ligament injury risk in females. HYPOTHESIS: Pubertal males would demonstrate increases in knee stiffness while pubertal females would not. Second, postpubertal female athletes would have significantly lower knee joint stiffness than postpubertal male athletes. STUDY DESIGN: Cohort Study; Level of Evidence 2 and Cross-Sectional Study; Level of Evidence 3. METHODS: Two hundred sixty-five females and 50 males part

Reverse-flow island sural flap.

The reverse flow island sural flap is presented as an alternative to flaps currently used for reconstruction of small and medium substance losses in the distal third of the leg, ankle, and heel. This is a random type of flap, based on the reverse flow of the superficial sural artery, which mainly depends on the anatomy of the perforators of the peroneal artery system. The anatomic structures that constitute the pedicle are the superficial and deep fascias, the sural nerve, the short saphenous vein, and the superficial sural artery. The skin island and the subcutaneous cellular tissue complemen

[Distal pedicled sural island flap-plasty for defect coverage of the distal lower extremity].

Soft tissue defects on the distal lower extremity often need extended reconstructive procedures, especially if tendinous, articular, or bony structures are exposed. The use of local flaps in the perimalleolar, pretibial and calcaneal area is limited, often free tissue transfer is required. The distally based sural island flap is a pedicled soft-tissue flap on the vascular axis of the sural nerve. It can be raised as a fasciocutaneous or a lipofascial flap. We report our experience of 16 cases, nine of these cases in the lipofascial technique. The aim of soft tissue coverage was reached in 15 c

Nursing assessment of injuries to the lower extremity.

When assessing injuries to the lower extremity, they can best be discussed if divided into two groups: (1) vehicular or crushing injuries; and (2) nonvehicular, torsion, or overuse injuries. Vehicular injuries require speedy, accurate assessment as they are often complex and frequently involve other systems. The patient is initially stabilized, then assessment of orthopedic problems is begun. The nurse's role is an assistive one. If the patient is admitted to the hospital, orthopedic care involves prevention of complications, reduction of pain, and assisting the patient in daily activities. No

Risk of injury to the sural nerve during posterolateral approach to the distal tibia: An ultrasound simulation study.

When surgeons operate on the foot and ankle, the most common complication that may arise is injury of the cutaneous nerves. The sural nerve (SN) is potentially at risk of being injured when treating fractures involving the distal tibia using the posterolateral approach. The aim of this study was to evaluate how differences in length and position of the surgical treatment of fractures involving the distal tibia can affect the risk of SN injury. The study involved 40 healthy volunteers (n = 80 lower limbs). Ultrasound simulation of each potential surgical incision site was used to locate the SN

Injuries in sedentary individuals enrolled in a 12-month, randomized, controlled, exercise trial.

BACKGROUND: The risk of musculoskeletal injury with the introduction of moderate-to-vigorous exercise in sedentary adults is not well established. The purpose of this report is to examine the effect of a 12-month exercise intervention on musculoskeletal injury and bodily pain in predominately overweight, sedentary men (n = 102) and women (n = 100), ages 40 to 75 years. METHODS: Participants were randomized to a moderate-to-vigorous aerobic exercise intervention (EX) (6 d/wk, 60 min/d, 60% to 85% max. heart rate) or usual lifestyle control (CON). Participants completed a self-report of musculo

[The inner flap of the thigh: an alternative about the cover of the ankle and the foot].

The authors relate a case of cutaneous cover of the ankle and the right foot on a leg with precarious vascularization by the inner flap of the thigh with a serious burnt person the left leg amputated. The flap, which has been autonomized first before being set apart, got a well development and allowed to cover the ankle and the foot. It is a flap which deserve to be used and can count among the cover means of the ankle and the foot and even to constitute an alternative to the use of classical flaps (sural and supramalleolar flaps) in case of vascular risk.

Missed lower extremity and spine fractures in athletes.

The majority of missed fractures in athletes are not overlooked because of difficulties recognizing them on radiographs; rather, these fractures occur less commonly and often exist without accompanying disabilities, which makes their diagnosis more of a challenge. To complicate matters, evaluation of an injured athlete is often completed in the setting of the playing field, where distractions can be abundant. The physician treating sports-related injuries must not only follow the general principles of fracture evaluation and management, but must also be aware of the commonly understated fractu

Severe vascular complications after derotational osteotomy of the tibia salvaged with free functional latissimus dorsi muscle transfer. A case report.

We present a case study of a 26-year-old male who sustained severe vascular and neurogenic injury during derotational osteotomy of the tibia. Directly postoperatively he complained of a drop foot, but 3 days later presented with an ischemic compartment syndrome of the anterior and lateral compartments. After debridement the osteotomy and metalware were exposed and the patient had a drop foot. Here we report how we salvaged his lower limb with a free functional latissimus dorsi muscle transfer that reconstructed soft tissues and restored ankle dorsiflexion.
