BACKGROUND: Exposure and response prevention, a type of cognitive-behavioral therapy, is an effective first-line treatment for obsessive-compulsive disorder (OCD). Despite extensive evidence of the efficacy of exposure and response prevention (ERP) from clinical studies and in real-world samples, it is still underused as a treatment. This is likely due to the limits to access to care that include the availability of adequately trained therapists, as well as geographical location, time, and cost barriers. To address these, NOCD created a digital behavioral health treatment for OCD using ERP del

BACKGROUND: Despite long-standing calls to disseminate evidence-based treatments for generalized anxiety (GAD), modest progress has been made in the study of how such treatments should be implemented. The primary objective of this study was to test three competing strategies on how to implement a cognitive behavioral treatment (CBT) for out-patients with GAD (i.e., comparison of one compensation vs. two capitalization models).
METHODS: For our three-arm, single-blinded, randomized controlled trial (implementation of CBT for GAD [IMPLEMENT]), we recruited adults with GAD using advertisements i

Anti-N-methyl-D-aspartate receptor (anti-NMDAR) encephalitis frequently presents with prominent psychiatric symptoms, often mimicking primary psychiatric disorders and posing a significant diagnostic challenge. We report the case of a 33-year-old woman who presented with a 10-day history of acute behavioral and psychiatric symptoms initially suggestive of a primary psychiatric disorder. Although initial management was directed toward psychiatric illness, the clinical course evolved with fluctuating consciousness and abnormal involuntary movements, raising suspicion of an organic neurological p

Catatonia is a neuropsychiatric syndrome with motor and behavioral symptoms, including mutism, negativism, posturing, stereotypy, and hypoactivity. Although previously linked primarily to psychiatric illness, a variety of medical conditions and substance intoxication or withdrawal have also been implicated. It is important to identify all potential underlying drivers of catatonia and treat them along with the catatonia itself for the best outcome. We present the case of a 48-year-old female with a psychiatric history of bipolar disorder and past medical history of sickle cell disease who has f

The temporal pole represents a key node within temporolimbic networks and may contribute to seizure generation in temporal lobe epilepsy. Direct cortical stimulation (DCS), performed during intracranial presurgical evaluation, is a valuable method for delineating the seizure onset zone by reproducing electro-clinical features of a patient's habitual seizures. However, data specifically addressing temporopolar stimulation remain limited and dispersed. We conducted a systematic review in accordance with PRISMA criteria to summarize published evidence on the semiology of DCS-induced signs and sym

Creutzfeldt-Jakob disease (CJD) is a rare and rapidly progressive neurodegenerative disorder caused by the accumulation of misfolded prion proteins, resulting in neuronal damage. It can present with symptoms that mimic other neurological conditions, which can complicate early diagnosis. We report a case of a 58-year-old male who presented with cognitive decline, motor dysfunction, and behavioral changes following alcohol cessation, initially resembling Wernicke's encephalopathy. As his condition progressed, he developed nonconvulsive status epilepticus, and subsequent diagnostic workup demonst

BACKGROUND: Cytomegalovirus (CMV) encephalitis is a rare but serious complication in immunocompromised patients, particularly transplant recipients as they are heavily immunosuppressed.
CASE SUMMARY: We report the first documented case to our knowledge of CMV encephalitis in a 67-year-old male heart transplant recipient, confirmed by cerebrospinal fluid polymerase chain reaction. The patient presented with confusion and distinct behavioral changes, yet brain magnetic resonance imaging revealed no acute abnormalities.
DISCUSSION: This case highlights the potential for CMV encephalitis to pres

PURPOSE OF REVIEW: Non-relaxing pelvic floor dysfunction (NR-PFD) is a poorly understood and underdiagnosed cause of voiding dysfunction in patients without clear anatomic or neurologic obstruction. Symptoms may include pelvic pain, urinary complaints, defecatory dysfunction, and sexual issues, but their variability makes NR-PFD challenging to recognize and manage. This review focuses on the urologic manifestations of NR-PFD and outlines current diagnostic and treatment strategies.
RECENT FINDINGS: Video urodynamics and surface EMG, alongside focused physical examination, are key tools for di

Parkinson's disease (PD) is characterized by degeneration of the nigrostriatal dopamine system, resulting in progressive motor and nonmotor symptoms. Although most studies have focused on the basal ganglia network, recent evidence suggests that the zona incerta (ZI), a subthalamic structure composed of 4 neurochemically defined regions, is emerging as a therapeutic target in PD. This review summarizes the clinical and animal studies that indicate the importance of ZI in PD. Human clinical studies have shown that subthalamotomy or deep brain stimulation (DBS) of the ZI alleviates muscle rigidit

Depression (Dep) is one of the most common concomitant symptoms of Parkinson's disease (PD), but there is a lack of detailed pathologic evidence for the occurrence of PD-Dep. Currently, the management of symptoms from both conditions using conventional pharmacological interventions remains a formidable task. In this study, we found impaired activation of extracellular signal-related kinase (ERK), reduced levels of transcription and translation, and decreased expression of brain-derived neurotrophic factor (BDNF) in the medial prefrontal cortex (mPFC) of PD-Dep rats. We demonstrated that the ab

OBJECTIVES: Anabolic androgenic steroids (AAS) are derivatives of testosterone, used to treat gonadal disturbances, performance enhancement, and aesthetic purposes. AAS abuse can lead to side effects, including androgenic, cardiovascular, and liver disturbances, effects on libido, gynecomastia, and behavioral effects. There is a hypothesis that some joint tissues may be targets for sex hormones, and the use of AAS without medical follow-up may exacerbate temporomandibular joint problems in patients seeking performance and aesthetics.
METHODS: In this study, a cross-sectional survey was conduc

INTRODUCTION: Approaching patients with fibromyalgia (FM) is challenging due to the limited availability of scientifically proven effective therapies.
OBJECTIVE: Review the treatments in use for FM and present new knowledge that could benefit these patients. Non-pharmacological interventions are recommended as the first line of treatment: aerobic exercise, cognitive behavioral therapy and patient education, all aimed at improving pain and other symptoms. Additional approaches have been studied, such as, digital health interventions, combined treatments, noninvasive neuromodulation, and others

In this observational study, we assessed the extent to which a community-created pilot intervention, providing trauma-informed care for persons with HIV (PWH), affected HIV care retention and viral suppression among PWH attending an HIV Services Organization in the Southern US. PWH with trauma exposure and/or trauma symptoms (N = 166) were offered a screening and referral to treatment (SBIRT) session. Per self-selection, 30 opted-out, 29 received SBIRT-Only, 25 received SBIRT-only but reported receiving other behavioral health care elsewhere, and 82 participated in the Safety and Stabilization

More than 10 million people worldwide have Alzheimer's disease (AD), a degenerative neurological illness and the most prevalent form of dementia. AD's progression in memory loss, cognitive deterioration, and behavioral changes are all symptoms. Amyloid-beta 42 (Aβ42), the hyperphosphorylated forms of microtubule-associated tau protein, and other cellular and systemic alterations are all factors that contribute to cognitive decline in AD. Rather than delivering a possible cure, present therapy strategies focus on reducing disease symptoms. It has long been suggested that various naturally occur

Interstitial cystitis/bladder pain syndrome (IC/BPS) is characterized by chronic pelvic, perineal, or bladder pain in addition to lower urinary tract symptoms. The etiology of this condition is not fully understood, which presents a challenge for effective therapeutic intervention. Current treatment guidelines recommend the use of multimodal pain management strategies including behavioral/non-pharmacologic, oral medications, bladder instillations, procedures, and major surgery. However, the safety and efficacy of these modalities vary, and there is currently no optimal treatment for the manage

Fibromyalgia (FM) is a chronic disorder characterized by widespread musculoskeletal pain, fatigue, and cognitive impairment. Despite the availability of various treatment options, FM remains a challenging condition to manage. In the present study, we investigated the efficacy of formulated nanodispersions of lutein and beta-carotene in treating FM-related symptoms induced by reserpine in female Wistar rats. Several techniques have been implemented to assess this efficacy at various levels, including biochemical, bioelectrical, and behavioral. Namely, oxidative stress markers, monoamine levels,
