INTRODUCTION: Radiation exposure from intra-operative fluoroscopy in orthopaedic trauma surgery is a common occupational hazard. References for fluoroscopy use in the operating room for commonly performed operations have not been reported adequately. This study aimed to report appropriate intra-operative fluoroscopy use in orthopaedic trauma and compare the effect of surgery type and surgeon grade on radiation exposure.
METHODS: Data on 849 cases over an 18-month period were analysed retrospectively. Median and 75th centile values for dose area product (DAP), screening time (ST), and number o

Resorbable plating systems are used extensively for craniofacial reconstruction, particularly in children. The systems provide rigid fixation while potentially minimizing the long-term drawbacks of permanent plating systems such as plate "migration," bone growth restriction, and imaging artifact. However, the need for active plate resorption represents both the strength and weakness of these fixation systems. We present four patients who underwent cranial vault remodeling using a resorbable plating system, following which the plates were palpable and observable for at least 2 years postoperati

BACKGROUND CONTEXT: Perforation of the esophagus after anterior cervical spine surgery is a rare, but well-recognized complication. The management of esophageal perforation is controversial, and either nonoperative or operative treatment can be selected.
PURPOSE: Several reports have described the use of a sternocleidomastoid muscle flap for esophageal repair. In this case report, we describe a longus colli muscle flap as a substitute for a sternocleidomastoid flap in a patient with an esophageal perforation.
STUDY DESIGN: Case report.
PATIENT SAMPLE: A 20-year-old man sustained cervical sp

BACKGROUND CONTEXT: After cervical corpectomy, the use of tricortical autologous bone to fill the large defect is biomechanically and structurally inadequate and may lead to excessive donor site pain and morbidity. The major alternative, fibular strut allograft, has inherent problems that lead to lower rates of solid arthrodesis and graft migration. Majd et al. reported on 34 cases with a 97% solid fusion rate using titanium mesh cages and local bone graft to fill the cervical corpectomy defect.
PURPOSE: With long-term results, to confirm the results previously reported by Majd et al.

Fractures of the distal metaphysis of the tibia often include an extension into the ankle. Intramedullary nailing combined with covered screw osteosynthesis should reduce the high incidence of soft tissue and ankle problems and should be an alternative to open plate fixation, with good ultimate functional outcome. Between January 1993 and December 1995, a prospective study on 49 patients with distal metaphyseal tibia fracture and involvement of the ankle was performed. All the fractures were treated with intramedullary nailing combined with covered screw osteosynthesis, and plate fixation in c

Presentation: A 65-year-old female with a prior right-sided cephalomedullary nail fixation for a pertrochanteric neck of femur fracture fell from a gate onto her right hip. She was unable to weight-bear on her right leg and presented to the emergency department. Diagnosis: X-ray imaging in the emergency department diagnosed a peri-implant subcapital neck of femur fracture at the base of the lag screw threads. Treatment: After evaluation of multiple treatment options, the patient was managed conservatively with physiotherapy, progressive weight-bearing and bone protection interventions. Discuss

» Osteogenesis imperfecta (OI) produces lifelong skeletal fragility from collagen synthesis defects, with fracture risk 13-fold higher than the general population, despite bone mineral density-increasing drugs lacking fracture reduction evidence. » Telescoping intramedullary rods remain the gold standard for pediatric OI fractures and deformity correction, pioneered by Bailey-Dubow and refined by Fassier-Duval to accommodate growth. » Surgical fixation faces unique challenges including thin cortices and poor screw purchase. Adjunctive plating reduces complications from 45% (rods alone) to 16%

Radiolunate (RL) fusion is a selective salvage procedure for focal post-traumatic radiolunate arthritis, designed to address an isolated radiolunate lesion while preserving the uninvolved wrist compartments. Compared with radioscapholunate fusion, isolated radiolunate fusion preserves the radioscaphoid articulation and may theoretically favor preservation of the dart-throwing motion. Technical descriptions of RL fusion remain limited, particularly for the arthroscopic approach. This article describes a stepwise arthroscopic technique based on precise identification of the radial scapholunate c

Brachial plexus injuries (BPI) can result in irreversible functional loss of shoulder girdle musculature leading to humeral head subluxation and instability. The resultant mechanical pain can be debilitating and unresponsive to neuropathic pain medications. Surgical intervention, in the form of shoulder arthrodesis (SA), stabilizes the joint and can help to relieve this mechanical pain. Outcomes after SA have been variable in the general population, with high reported rates of nonunion. This is of particular concern in patients with BPI as they often have disuse osteopenia. Here, we describe t

CASE: A 14-year-old national-level badminton player presented with bilateral L5 pars interarticularis defects with pseudoarthrosis, unresponsive to conservative management. Computed tomography confirmed bilateral pars defect with sclerosed margins without spondylolisthesis or canal compromise. The patient underwent robotic-assisted minimally invasive pars repair augmented with recombinant human bone morphogenetic protein. Postoperatively, he reported minimal pain, resumed full athletic training by 12 weeks, and remained symptom-free at 1-year follow-up.
CONCLUSION: Robotic-assisted direct par

The elbow is one of the most commonly dislocated joints. Although simple dislocations of the elbow usually resolve with conservative management, certain patients can experience residual chronic instability. Posterolateral rotational instability accounts for approximately 80% of elbow chronic instability cases. We propose an all-arthroscopic reconstruction of the posterior, or ulnar, fascicle of the lateral ligament complex using an autograft or allograft, performed with a 5-mm-thick and 8-cm-long graft. The graft is first inserted distally into the supinator crest with an Arthrex 4.75-mm Swive

The socket-shield technique is considered one of the best treatment modalities that reduce the amount of facial bone loss and gingival recession following tooth extraction. However, the difficulty in preparing the shield by partial tooth extraction makes it technique-sensitive and limits its use. This case report presents a 29-year-old medically fit male patient with a destructed non-vital lateral incisor. It was planned for immediate implant placement in conjunction with the socket-shield technique. The shield was prepared in a new technique using a nickel-titanium endodontic file, which was

This is a rare case report of medial femoral condyle fracture with irreducible incarcerated patella dislocation. Following the literature search, only a few cases have been reported in the literature. In this case, the patient had undergone open reduction and screw fixation of the femoral condyle, augmented with a distal femoral locking plate (LCP). Postoperatively patient was immobilised with an above knee backslab for a month. After removing the backslab, physiotherapy was commenced to improve the range of motion and strengthening the quadriceps muscle. After 18 months of follow-up, the pati

This paper describes two different techniques for removal of broken Precice Stryde intramedullary bone lengthening nails, which unlike trauma nails are solid containing mechanical components. Consequently, surgeons face unique challenges when these implants brake within medullary canal. Here, we present our surgical approach for removal of three broken implants. In one patient (46kg) both Ø10mm femoral Stryde implants (max. weight allowance 68kg) broke through the proximal locking screw hole preoperatively on the right side and intraoperatively on the left side (413 and 504 days after index su

If left uncorrected, valgus ankle deformity in multiple hereditary exostoses can cause significant disability in skeletally immature children and in adults. Various management methods have been described, including hemiepiphyseal stapling, transphyseal screw placement, fibular-Achilles tenodesis, distal tibial osteotomy, and ablative epiphyseodesis. In this article, we report the cases of 3 skeletally immature children who had undergone hemiepiphyseal stapling of the medial distal tibial epiphysis for correction of valgus ankle deformity in multiple hereditary exostoses. A case report of a 6-year-old child with autosomal dominant osteopetrosis who sustained a closed femoral neck fracture initially treated with a spica cast with subsequent fracture displacement is presented. Further treatment included closed reduction and operative fixation, first, with a guide pin placement for a cannulated screw insertion that was abandoned because of the hardness of the bone. Three Kirschner wires were then inserted to maintain the closed reduction, followed by immobilization in a spica cast. At the 2-year follow-up, the child was completely free of any complications resulti

A 15-year-old girl was admitted to our hospital with severe low back pain. She had scoliosis dextra and tight hamstrings. A plain radiograph showed high-grade L5 spondylolisthesis with vertebral scalloping from the fourth lumbar to the first sacral vertebra. L5 wide laminectomy and posterior lumbar interbody fusion by iliac bone graft was performed using the Galveston method of sacral fixation and a pedicle screw system. The rod and hook system was used from T9 to the bilateral iliac wing. We added posterolateral fusion from T10 to S1 by autograft and allograft. The patient became pain free an
