[A case of brain metastasis from hepatocellular carcinoma].

Reported is the case of a patient who underwent surgical resection of a brain metastasis from a hepatocellular carcinoma. The 62-year-old male was admitted to hospital because of headaches and a left hemiparesis. Six years earlier he had undergone transcatheter arterial embolization for a hepatocellular carcinoma. Further, one year ago the lower lobe of his right lung had been resected because of a pulmonary metastasis from the same tumor. A neurological examination on admission revealed disorientation, dressing apraxia, and a left hemiparesis. A CT scan revealed two highly dense masses with p…

[Brain stem dysfunction in Arnold-Chiari II syndrome].

Among 76 patients suffering from myelomeningocele treated during 1978 to 1987 there were 12 children with brain stem signs as a sequel to Arnold-Chiari II syndrome. In 2 of these patients only stridor was seen, in 4 stridor with attacks of apnoea, in 2 attacks of apnoea with dysphagia, and in 4 children stridor, attacks of apnoea and dysphagia. Hence, it will be necessary to modify the classification given by Charney et al (4) in respect of brain stem patterns of signs according to three grades, since the signs of stridor, apnoea and dysphagia can be combined with each other in different ways.…

[Metastatic brain lesion in small-cell lung cancer].

Clinico-diagnostic correlations were studied in 74 patients suffering small cell lung cancer. Metastases to the brain were mostly multiple (66.22%) affecting the posterior cranial fossa (75%), right hemisphere and subtentorial area (34.6%). Diagnosis made by neuro-oncological examination was confirmed in 90.54%, brain scan--77.78, computed tomography--84.91, ophthalmological examination--28.81, EEG--58.18 and emission tomography--80%.

Technical and clinical aspects of topographic brain mapping.

TBM provides physiological and diagnostic information which is quite complementary to EEG, but does not replace the need for standard EEG recordings. For example, it does not show subtle morphologic abnormalities such as FIRDA, PLEDs or epileptiform discharges. Simultaneous EEG recording as well as impeccable technique are essential in proper recording of TBM, particularly as it can generate new forms of artifact. By prolonging the averaging time to over 180 seconds, and by application of flexible montages, a more accurate and cleaner record with less artifact can be obtained. Certain phenomen

Intracranial lymphoblastic sarcoma mimicking a primary brain tumor.

A lymphoblastic sarcoma mimicked a medulloblastoma in a 15-year old girl with a history of acute lymphoblastic leukemia diagnosed at age of 2 years. There was no evidence of leukemic infiltration in the brain during the 13-year latency period after the original diagnosis. She had received both prophylactic intrathecal methotrexate and central nervous system irradiation. This case emphasizes the need to consider leukemia as the cause of a 'brain tumor' in any patient that has a prior history of leukemia, regardless of the tumor manifestations or the remission duration of the leukemia.

[Brain abscess following cerebral infarction: a case report].

The authors report a case of brain abscess following cerebral infarction. A 73-year-old man was admitted to our clinic with symptoms of right hemiparesis and total aphasia. CT scan revealed abnormal low density area in the left fronto-temporo-parietal region. Cerebral angiography demonstrated occlusion of the left middle cerebral artery at the M1 portion. On the 16th hospital day, an episode of generalized seizure with high fever appeared, and intermittent high fever persisted thereafter. Two months after admission, CT scan revealed several cystic lesions with marked ring enhancement at the si…

Brain potentials reveal covert facial recognition in prosopagnosia.

Brain potentials were recorded in a prosopagnosic patient, in response to familiar and unfamiliar faces he was asked to recognize. The amplitude of the P300 component was found to be an inverse function of probability for each category of faces despite the patient's inability to consciously recognize the familiarity of these faces. In addition, P300 latency varied from 700 to 800 msec according to the familiarity of the faces, and P300 scalp location was different as a function of faces probability and overt recognition. The results imply that covert facial recognition may be evidenced in usin…

Neonatal brain abscess with intracystic hemorrhage--case report.

The authors describe the case of an 11-day-old boy with high fever and signs of meningeal irritation in whom computed tomography demonstrated a large brain abscess with intracystic hemorrhage in the right temporal lobe. Cerebrospinal fluid analysis indicated purulent meningitis caused by Escherichia coli. After aspiration of the abscess contents, the entire ventricular system gradually enlarged. Despite repeated ventricular drainage and ventriculoperitoneal shunting, the lateral horn of the left lateral ventricle remained dilated. The isolation of the lateral ventricle may have resulted from s…

Tumor necrosis factor identified in multiple sclerosis brain.

Frozen brain specimens from patients with multiple sclerosis (MS) and other neurologic diseases were analyzed using immunocytochemical techniques for the presence of TNF. In brain lesions in MS, and subacute sclerosing panencephalitis, TNF+ cells were demonstrated. At the lesion site in MS, TNF+ staining is associated with both astrocytes and macrophages. These observations were not made in Alzheimer's disease or normal brain tissue. The presence of TNF in MS lesions suggests a significant role for cytokines and the immune response in disease progression.

Brain lesions in patients with acquired immunodeficiency syndrome.

Infections involving the brain have become a major complication in patients with acquired immunodeficiency syndrome. We have reviewed 48 cases of central nervous system lesions in patients with acquired immunodeficiency syndrome. and its related complex. All patients had computed tomographic scans with contrast performed; 31 of 48 were intravenous drug abusers. Computed tomographic abnormalities found included 21 patients with multiple ring-enhancing lesions, 13 with single ring-enhancing lesions, 11 with single hypodense lesions, and three with multiple hypodense lesions. Twenty-five patients…

Treatment of brain trauma with liposomal superoxide dismutase.

Brain trauma was induced in rats by impact of a steel bar on the head with a force such that damage (as measured by neurological scoring) was reversible in fourteen days. Systemic treatment (intraperitoneal injections) with free bovine copper superoxide dismutase or a liposomal form of the enzyme considerably shortened recovery time to less than half. Tests included cranial nerves--cornean and aural reflexes, and sensorial motricity functions--gripping reflexes, displacement reactions, recovery and flexion reflexes, equilibrium tests and spontaneous mobility. Normalisation of EEG recordings wa…

Tumor of the tongue containing heterotopic brain tissue.

A polypoid tumor containing astrocytic glia and ependyma was excised from the base of the tongue of a 3-month-old infant. Neuroid elements were identified immunocytochemically and by means of electron microscopy. We attribute this rare anomaly to displacement of brain tissue in early embryogenesis before closure of the palate.

[Metastatic brain tumor originating from the cardiac sarcoma].

A 39-year-old woman had suddenly onset of headache, aphasia and right hemiparesis. CT scan revealed a mass lesion in the left parieto-temporal lobe. The brain tumor was resected twice. But, heart tumor could not be resected because congestive heart failure progressed rapidly. At last, she succumbed to systemic metastasis at 4 months after the onset of symptoms. At autopsy, tumors were found to be localized in left atrium of the heart, left parietal lobe, bilateral occipital lobe of the brain, pancreas, liver and stomach. Histological examination was diagnosed as a malignant-fibrous-histiocytom…

Differentiation between brain lesions in experimental thiamine deficiency.

Dietary deprivation of thiamine combined with pyrithiamine administration in rats was used for pathophysiological and morphological investigations. The animals passed through three different symptomatic stages, ranging from slight neurological abnormalities to generalized seizures from day 8 up to day 11. Hypothermia was a consistent finding during the second week. Histological examination revealed two types of neuropathological lesions in the rats. Those in the colliculi inferiores and the vestibular nuclei were characterized by a bullous spongiform appearance of the neuropil with severely da

Clinical brain death with preserved cerebral arterial circulation.

We present a case of a 44-year old male suffering from severe subarachnoid haemorrhage secondary to a ruptured basilar artery aneurysm. Clinical examination on 3 consecutive days fulfilled the criteria for the diagnosis of clinical brain death. Nevertheless, cerebral angiograms showed intracranial filling until the third day after the bleeding. Autopsy showed acute infarction of the brain stem and the cerebellum.

Age-specific characteristics of brain death in children.

Clinical and neuropathologic characteristics of 45 children who met criteria for brain death were analyzed. Children between 2 months and 1 year of age were compared with children older than 1 year and children older than 5 years. The observation period to fulfill brain death criteria was not different between the age groups. Deep tendon and spinal reflexes were preserved significantly less frequently in children younger than 1 year old. Diabetes insipidus and the necessity of inotropic support were significantly more frequent in children older than 5 years. Fifty-eight percent (26/45) of pati…
