Endoscopic retrograde cholangiopancreatography-guided transpapillary gallbladder drainage has emerged as an effective alternative for management of acute cholecystitis in nonoperable candidates. Delayed acute pancreatitis has not been previously described as an adverse event with this procedure. In this article, we describe 3 patients who developed acute pancreatitis between 2 and 6 weeks after stent insertion with no alternative inciting cause. Delayed acute pancreatitis may represent a rare and previously uncharacterized adverse event related to transpapillary gallbladder drainage.

Although ultrasound is the initial imaging modality of choice in patients with right upper quadrant pain or suspected biliary obstruction, a number of challenges in clinical practice limit its utility as a stand-alone imaging modality. This article presents a focused review of gallbladder and biliary ultrasound, highlighting current knowledge gaps, emerging applications, and directions for further study. The authors cover selected topics including acute cholecystitis, cystic artery velocity, gallbladder polyps, contrast-enhanced ultrasound, and incidental biliary duct dilatation.

Tuberculosis (TB) is one of the most common infectious diseases worldwide. The case of a 20-year old male refugee from Somalia, who initially presented with right-sided upper abdominal pain, vomiting, weight loss and jaundice with suspected cholecystitis is reported. In the course of further diagnostics, pyloric stenosis surprisingly appeared, which, like the cholestasis, was caused by compressing peripancreatic lymph nodes. Lymph node cytology finally showed evidence of caseating necrosis with evidence of TB pathogens.

Endoscopic ultrasound-guided gallbladder drainage (EUS-GBD) has been utilized as an alternative endoscopic technique for patients with acute cholecystitis. In addition to EUS-guided hepaticogastrostomy and EUS-guided cystogastrostomy, EUS-GBD has been reported as being useful for biliary drainage in cases with distal malignant biliary obstruction instead of conventional endoscopic retrograde cholangiopancreatography. We present a case of successful EUS-GBD for malignant obstruction of an aberrant hepatic duct draining directly into the cystic duct.

Gallbladder perforation is a relatively uncommon complication of acute cholecystitis and may occur with or without gallstones. Prophylactic cholecystectomy has been recommended for patients with very large stones (>3 cm) due to an increased risk of gallbladder cancer. We present the case of a 68-year-old woman who died of hemorrhagic shock following gallbladder perforation due to very large gallstones. This case provides additional support for consideration of prophylactic cholecystectomy in patients with very large gallstones.

Despite several reviews on the treatment of gallstones, there still seems to be unanswered questions regarding some topics. This review focuses on some of these: Why are more and more patients being cholecystectomised? Who needs further diagnostic work-up for common bile duct stones (CBDS) before laparoscopic cholecystectomy? Is elective cholecystectomy mandatory following endoscopic or conservative treatment of complicated gallstone disease (CBDS, acute pancreatitis, or acute cholecystitis)? Further research is still required in order to answer these questions.

Examined were 317 patients with brain concussion ranging in age from 16 to 75 years. Of them in 133 (41.91%), chronic diseases (I-II degree chronic insufficiency of cerebral circulation, arterial hypertension, chronic cholecystitis, gastric and duodenal ulcer disease, chronic bronchitis, bronchial asthma) were revealed. Peculiarities of the course of brain concussion in these patients are noted. The author stresses that it is necessary to take into account chronic diseases when observing and treating the sufferers with brain concussion.

Among 130 patients with fever of unknown origin (FUO) studied from 1981 to 1985, 34 were diagnosed as having hepatobiliary disorders: amoebic liver abscess (11), pyogenic liver abscess (4), hepatic hydatid cysts (2), hepatic fascioliasis (2), tuberculous hepatic granulomas (1), chronic calcular cholecystitis with recurrent cholangitis (2), chronic active hepatitis (2), hepatocellular carcinoma (3), lymphoma involving the liver (4) and hepatic metastasis in (3) cases. Hepatobiliary disorders were the cause in 27% of FUO seen during 4 years.

The authors present an analysis of the results of complex treatment in 4318 patients operated upon for acute peritonitis, caused by acute appendicitis, perforating gastric and duodenal ulcers, acute cholecystitis, ruptures and perforations of the intestine and other surgical and gynecological diseases. Patients with diffuse purulent peritonitis showed marked disorders in protein-aminoacid, nitrogen, and water electrolyte metabolism, acid-base balance, a reduced nonspecific immune responsiveness of the organism. Therpeutic tactics was delineated taking into account the revealed changes.

The value of an ultrasound prior to planned interventions is undisputed. Clinically relevant incidental findings are also among the reasons why abdominal ultrasound examinations should be considered part of the diagnostic evaluation during inpatient admission. We report here an interesting case of an unusual "three-headed" polypoid lesion on ultrasound, which histologically and pathologically revealed a saprophytic manifestation of local aspergillosis in the gallbladder without clinical signs of cholecystitis.

Abnormalities in the position of the gallbladder are not encountered commonly such as the ectopic location. We present a case of laparoscopic cholecystectomy for gallbladder that was found in an ectopic position. The surgical procedure can be difficult in some cases of acute cholecystitis and ectopic position of the gallbladder may add to complexities of the procedure due to abnormal location or anatomical variants of the biliary tree. Preoperative identification of ectopic gallbladder may aid in planning and performing a safe surgical procedure.

Mesenteric panniculitis is a benign disease characterized chronic nonspecific inflammation of the mesentery adipose tissue of the small intestine and colon. The specific etiology of the disease is unknown. The diagnosis is suggested by computed tomography (CT) and is usually confirmed by surgical biopsies. In recent years, abdominal CT is used as an effective method for diagnostic evaluation.We reported a case of the mesenteric panniculitis diagnosed with CT and operated for acute pancreatitis and cholecystitis five years ago.

ORIGINAL POSITION: The number of patients with biliary tract pathology is constantly growing up. Participation of psychovegetative state in the development of gallbladder dysfunction was confirmed.
CONCLUSION: Revealing and treatment of gallbladder dysfunction, treatment of disorders of psychovegetative disorders system in patients with dysfunction of gallbladder is the necessity for successful prophylaxis of chronic cholecystitis, cholelithiasis, pancreatitis.

Gallbladder volvulus is a rare condition which can mimic an acute cholecystitis. This condition is characterized from a rotation of the gallbladder on its mesentery along the axis of the cystic duct and cystic artery. Preoperative diagnosis is difficult. This is an acute surgical emergency that must be treated with immediate detorsion and cholecystectomy. We report a case of acute gallbladder torsion in an elderly man and review the clinical aspects of the disease in the context of the available literature.

Rupture of a right hepatic artery pseudoaneurysm into the gallbladder is very rare. We demonstrated a 20-mm dumbbell-shaped pseudoaneurysm in the gallbladder lumen by using contrast-enhanced magnetic resonance angiography in a 73-year-old man with acute right upper abdominal pain. Inflammation of the gallbladder caused by calculous cholecystitis, which leads to biliary leakage and erodes the right hepatic artery, could have been the cause.

Gallblader tuberculosis is uncommon and may mimic a neoplasia. We report a 55-year-old man who presented with fever and abdominal pain compatible with cholecystitis. Abdominal ultrasound and computed tomographic scan showed a tumoral aspect of the bottom of the gallbladder and invasion of adjacent liver. Diagnosis of pseudo-tumoral gallbladder tuberculosis was obtained with the histologic examination of surgical sampling. Disease course was uneventful with appropriate antituberculous therapy.

Torsion of gallbladder is simply defined as a rotation of the gallbladder on its mesentery along the axis of the cystic duct and the cystic artery. It is rare disease which etiology it's still unknown. However we can point some factors postulated as playing causative roles. This disease symptoms mimic acute cholecystitis. In spite of advanced radiological imaging technics there are still problems to set up correct preoperative diagnosis of gallbladder torsion.
