We report two cases of localized benign pleural mesothelioma with different clinical features. Neuropsychiatric symptoms, including coma, hemiplegia, seizures and misbehavior predominated in the first case, associated with hypoglycemia. The symptoms in the second case were essentially respiratory (cough, dyspnea, and chest pain). Treatment consisted in thoracotomy and complete surgical resection. Streptomyces species are most widely known for their production of antimicrobial substances and, apart from mycetoma, have rarely been reported as a cause of infection. We describe a patient with early human immunodeficiency virus infection who presented with fever, cough, and nodular pulmonary infiltrates. Open lung biopsy revealed necrotic tissue and a sulfur granule; aerobic bacterial cultures yielded Streptomyces species. The patient was treated successfully with clarithromycin for 6 months. We review the clinical presentation and treatment of invasive streptomyces infections.

Hypersensitivity pneumonitis is an allergic disease resulting from sensitization to inhaled organic dust, usually manifest with recurrent bouts of cough, dyspnea and fever. Most of the reported cases concern the adult population. The disease is rarely seen in the pediatric age group, as stated by most of the reviews on the subject. In this article, a case with hypersensitivity pneumonitis is described. Precipitating antibodies to avian protein were demonstrated in serum from a girl with recurrent bouts of pneumonitis which was otherwise unexplained.

Scrub typhus is a rickettsian disease which is seldom found in pregnancy. A 31-year-old, 34 weeks pregnant woman presented with fever, chill and cough for 6 weeks. Fetal jeopardy was found then a cesarean section was performed to deliver a 2,200 g male with hepatosplenomegaly. The mother's diagnosis was confirmed by positive Weil-Felix (OXK titer 1:320) and scrub typhus (titer 1:1600) tests. Vertical transmission was also demonstrated by a positive scrub typhus IgM in her child.

The decoction of Cynanchum glaucescens and the ethanol extract of C. stauntonii show significant antitussive and expectorant actions. The decoctions of C. stauntonii or C. glaucescens have antiasthmatic and anti-inflammatory effects. The decoction of C. atratum has some expectorant effect but no antitussive or antiasthmatic actions. On the other hand, the decoction of C. versicolor cannot relieve cough nor sputum. The results indicate that both C. atratum and C. versicolor cannot be used as C. glaucescens or C. stauntonii.

Arcanobacterium haemolyticum was found in 1.4% and beta-haemolytic streptococci in 23% of throat cultures from army conscripts with sore throat (n = 498). 38% of the beta-haemolytic streptococci were of group A. Patients culture-positive for A. haemolyticum or beta-haemolytic streptococci had pharyngeal exudate, cervical lymphadenopathy and ear ache significantly more often--but cough less often--than culture-negative patients. The pharyngeal colonization rate of healthy conscripts (n = 232) by A. haemolyticum was 0.4% and by beta-haemolytic streptococci, 6.5%.

Clinical and physical data about laryngeal mask use in eye surgery were collected in 57 patients. We observed 4 failures which required endotracheal intubation, 3 leaks during operative field installation, 4 salvations but no laryngeal spasm and no cough. There were no significative differences between body weights in laryngeal mask groups no 3 and no 4. Insufflation pressures of patients without curarisation were more important. The duration of ventilation was not correlated with salivation, leaks or increase of insufflation pressure.

Miliary tuberculosis presenting as fatal adult respiratory distress syndrome is reported in a 69-year-old man. Idiopathic thrombocytopenic purpura led to splenectomy 2 years before admission and was currently treated with corticosteroids. Moderate dyspnea, a dry cough and weight loss were presenting clinical features, preceding respiratory failure by only a few days. In these patients with atypical symptoms early diagnosis and prompt antituberculous chemotherapy are life-saving. The diagnosis of miliary tuberculosis should be systematically considered in ARDS of unknown origin.

2 cases of mycetomas are presented as exceptional manifestations of invasive aspergillosis. Both patients had diagnosis of leukemia and neutropenia, secondary to chemotherapy. The clinical features were subacute and mainly fever, cough, dyspnea and multiple infiltrates with cavitations and balls of hyphae within cysts shown in chest x-ray. The precipitin tests were positive for A. fumigatus and A. flavus, and were correlated with the specimens of the bronchial brushing. One of the patients improved with antimycotics and normalization of neutropenia.

The non-opioid antitussives dextromethorphan and carbetapentane, the active ingredients of several over-the-counter cough suppressants, provide a dose-related protection against maximal electroshock seizures in rats. Both drugs, which bind with high affinity to the same site in the brain, potentiated the effects of the prototypic antiepileptic drug diphenylhydantoin. We propose that these novel anticonvulsant drugs may represent potentially useful therapeutic agents for the treatment of some forms of epilepsy, either alone or in combination with existing antiepileptic drugs.

Eleven patients with exogenous lipoid pneumonia are reported. A history of usage of oil was obtained in all, but in five only retrospectively after thoracotomy. The clinical presentations ranged from occasional cough to severe respiratory difficulties. The radiologic manifestations varied from a solitary lipoid granuloma and larger consolidations simulating carcinoma to extensive subacute bronchopneumonia. The definitive diagnosis was made by history and radiographic findings in three, by history and analysis of sputum or bronchial washings in two, and after thoracotomy in six.
