Association of muscle performance and heart rate variability in middle-aged type 2 diabetes mellitus.

BACKGROUND: Type 2 diabetes mellitus (T2DM) is characterized by insulin resistance, which impairs the metabolism of carbohydrates, lipids, and proteins. Recent findings indicate that skeletal muscle atrophy, decreased muscle strength, and reduced muscle performance have become prevalent complications of diabetes. Additionally, dysfunction of cardiac autonomic nerves also leads to skeletal muscle impairment. These issues further exacerbate diabetes, thereby leading to a vicious cycle. This study investigates the association between muscle performance and cardiac autonomic neuropathy in middle-a

Ultrasound-Stimulated BMSCs Promote Regenerative Healing in Refractory Foot Ulcer by Paracrine Effect.

OBJECTIVE: Diabetic foot ulcer (DFU) is a common and serious complication of diabetes, often leading to infection, amputation and poor quality of life. Bone marrow mesenchymal stem cells (BMSCs) have shown promise in treating chronic wounds, but their therapeutic efficacy is limited due to poor survival and low regenerative activity. Low-intensity pulsed ultrasound (LIUS), a non-invasive physical modality, has been shown to enhance the biological behavior of BMSCs. LIUS in combination with BMSCs shows promising potential to promote healing of refractory foot ulcer wounds. METHODS: Several met

Gender Differences in the Clinical Profile of Sodium-Glucose Cotransporter-2 Inhibitor-Related Urinary Tract Infections.

Background Sodium-glucose cotransporter-2 inhibitors (SGLT2Is) are a novel class of oral antidiabetic agents with proven cardiovascular and mortality benefits. By promoting glucosuria, SGLT2Is increase the risk of genital and urinary tract infections (UTIs), which remain uncomplicated in most cases. Comparative studies detailing the gender differences in the clinical profile of SGLT2I-related UTIs (SUTIs) are lacking. Hence, this study was designed to investigate the gender-related differences in the clinical profile of SUTIs. Methodology This prospective study enrolled 100 consecutive diabete

De Novo Fibrinogen A Alpha Chain Amyloidosis in a Kidney Transplant Patient: Case Report and Literature Review.

RATIONALE: De Novo transplant amyloidosis denotes the condition when a patient develops amyloidosis after transplantation but had not been diagnosed with the disease prior to transplantation. The incidence of de novo amyloidosis in kidney transplants is rare, but few published case reports have described the occurrence of de novo Amyloid A protein (AA) and Light Chain (AL) amyloidosis. However, de novo hereditary fibrinogen A alpha chain (AFib) has not been previously reported. PATIENT PRESENTATION: We present a 72-year-old man, a kidney transplant recipient, who developed progressive rise in

Utilization of the common functional groups in bioactive molecules: Exploring dual inhibitory potential and computational analysis of keto esters against α-glucosidase and carbonic anhydrase-II enzymes.

Diabetes mellitus, a progressive chronic disease, characterized by the abnormal carbohydrate metabolism is associated with severe health complications including long term dysfunction or failure of several organs, cardiovascular and micro-angiopathic problems (neuropathy, nephropathy, retinopathy). Despite the existence of diverse chemical structural libraries of α-glucosidase inhibitors, the limited diabetic treatment due to the adverse side effects such as abdominal distention, flatulence, diarrhoea, and liver damage associated with these inhibitors encourage the medicinal research community

Brain-derived neurotrophic factor, insulin like growth factor-1 and inflammatory cytokine responses to continuous and intermittent exercise in patients with type 1 diabetes.

AIMS: Type 1 diabetes mellitus (T1DM) is an important risk factor for cognitive decline and motor dysfunction due to progressive muscular atrophy. Chronic hyperglycemia may be responsible for impaired vascular function, loss of muscle mass, and morphological abnormalities in the myocytes. For the proper functioning of the neuromuscular system, two crucial growth factors are necessary: brain-derived neurotrophic factor (BDNF) and insulin-like growth factor-1 (IGF-1), whose reduced expressions have been implicated in progressive neuropathy and muscle atrophy in patients with T1DM. The aim of the

Prognostic value of cardiac I-123 metaiodobenzylguanidine imaging in patients with non-insulin-dependent diabetes mellitus.

BACKGROUND: 123I-metaiodobenzylguanidine (MIBG) myocardial scintigraphy is a useful tool for diagnosing cardiac autonomic neuropathy (CAN) in non-insulin-dependent diabetes mellitus (NIDDM). However, the long-term prognostic value for cardiac events or mortality in NIDDM has not been clarified. METHODS AND RESULTS: We investigated the long-term prognostic value of cardiac MIBG imaging for both cardiac event and mortality owing to a variety of causes of death in NIDDM. 144 NIDDM patients were analyzed retrospectively with the end-points of a cardiac event and various causes of mortality. Ilizarov external frame technique for pirogoff amputations with ankle disarticulation and tibiocalcaneal fusion.

BACKGROUND: The high rates of wound failure, persisting infection, and nonunion of the tibiocalcaneal arthrodesis are the main reasons why the Pirogoff ankle disarticulation is rarely used for limb salvage. Use of the Ilizarov external frame has increased our fusion rate. The purpose of this study was to review our experience with the use of the Ilizarov external frame as a technique for Pirogoff amputations with ankle disarticulation and tibiocalcaneal fusion. METHODS: Twenty-four patients (median age, 57.4 years; range, 29-76 years) underwent a Pirogoff amputation with Ilizarov external fra

Duloxetine treatment and glycemic controls in patients with diagnoses other than diabetic peripheral neuropathic pain: a meta-analysis.

OBJECTIVE: Mood disorders are often associated with poor glycemic control, and antidepressant treatments for mood and pain disorders can alter plasma glucose levels in patients with diabetes. A previous meta-analysis from three studies showed that duloxetine modestly increased fasting plasma glucose (FPG) and HbA(1c) levels in patients with diabetic peripheral neuropathic pain (DPNP). This meta-analysis examined whether there were any short- and long-term effects of duloxetine (20-120 mg/day) on glycemic control in patients with diagnoses other than DPNP. RESEARCH DESIGN AND METHODS: Short-te

[External fixator for reconstruction of foot statics in neurogenic osteoarthropathies].

The localization of neurogenic osteopathy in the hindfoot often results in deformities which cannot be corrected by conservative methods. Indications for operation are recurring ulcers, deep infection, and reduced stability with progressive deformity. The aim of this study was to ascertain whether external fixation enables reestablishment of foot stability even when the osteoarthropathic processes have not entirely ceased. A bilaterally mounted Hoffman 2 fixator was used for open repositioning and restabilization on 14 patients with osteoarthropathy of the hindfoot: 12 had diabetes mellitus an

Changes in motor and sensory nerve conduction parameters with temperature in normal and diseased nerve.

The effects of temperature on conventional motor and sensory nerve conduction parameters were studied in normals and in some pathological conditions. Surface stimulating and recording electrodes were used to examine the function of the median nerve. The motor and sensory conduction velocities, the parameters of compound muscle action and sensory nerve potentials were correlated with skin temperature. In the control subjects all nerve conduction parameters changed with temperature. These findings were similar to those published previously, but the mean slope for MCV was lower than that reported

Management of wounds in anesthetic tissue.

The management of trophic ulcers, the most common wound in hypesthetic or anesthetic tissue, is reviewed. Pressure from an extrinsic source is the most important factor leading to development of the lesion, but the threshold for ulceration is markedly lowered by impairment of protective vasomotor reflexes and metabolic responses. Prevention of severe or sustained compression of such tissue is the hallmark of proper management. The most useful surgical procedures are also discussed.
