Probabilistic independent component analysis of dynamic susceptibility contrast perfusion MRI in metastatic brain tumors.

PURPOSE: To identify clinically relevant magnetic resonance imaging (MRI) features of different types of metastatic brain lesions, including standard anatomical, diffusion weighted imaging (DWI) and dynamic susceptibility contrast (DSC) perfusion MRI. METHODS: MRI imaging was retrospectively assessed on one hundred and fourteen (N = 114) brain metastases including breast (n = 27), non-small cell lung cancer (NSCLC, n = 43) and 'other' primary tumors (n = 44). Based on 114 patient's MRI scans, a total of 346 individual contrast enhancing tumors were manually segmented. In addition to tumor vol

MRI and spectroscopy in (near) term neonates with perinatal asphyxia and therapeutic hypothermia.

BACKGROUND: Previous studies have demonstrated the association of abnormalities on diffusion-weighted MRI (DW-MRI) and proton magnetic resonance spectroscopy (1H-MRS) in infants with perinatal asphyxia. The use of therapeutic hypothermia might change this association. AIM: To study the association between DW-MRI and 1H-MRS and outcome after perinatal asphyxia and therapeutic hypothermia in infants with a gestational age of ≥36 weeks. PATIENTS AND METHODS: Infants with perinatal asphyxia and therapeutic hypothermia (n=88) were included when an MR examination was performed within 7 days after

Vertebral Artery Dissection Leading to Fornix Infarction: A Case Report.

BACKGROUND: The subcallosal artery is a proximal branch of the anterior communicating artery and has been recognized as the vessel responsible for fornix infarction. Fornix infarction caused by vascular damage to the posterior circulation has not been reported previously. RESULTS: A 26-year-old woman suffered from fornix infarction due to artery-to-artery embolism after vertebral artery dissection. Cerebral infarctions were also found in the left thalamus, body of the left caudate nucleus, and the left occipital lobe other than the fornix. CONCLUSIONS: Occlusion of the subcallosal artery res

Extensive vasogenic edema of anti-aquaporin-4 antibody-related brain lesions.

OBJECTIVE: Using neuroimaging, we analyzed the nature of extensive brain lesions in five anti-aquaporin-4 (AQP4) antibody-positive patients with neuromyelitis optica spectrum disorders. RESULTS: Extensive brain lesions involved white matter in three, and basal ganglia and corpus callosum in one each. Four patients showed high diffusivity on apparent diffusion coefficient maps and three demonstrated increased choline/creatine ratios and decreased N-acetyl-aspartate/creatine ratios on (1)H-magnetic resonance spectroscopy. These findings suggested that the lesions were vasogenic edema associated

[Plaque characterization using carotid MRI in acute ischemic cerebrovascular syndromes (three case reports)--can carotid MRI be useful as an adjunct to the diagnosis of TIA in which the ischemic lesions were recognized at a later date?].

Recent diffusion-weighted imaging (DWI) studies describe a relevant ischemic lesion in almost half of all patients with transient ischemic attack (TIA). We report 3 cases of TIA in which the ischemic lesions were recognized on DWI at a later date ("late-imaged TIA"). Three men (average age, 72.7 years) were admitted to our hospital on complains of monoparesis or sensory disturbance in their right arms. On admission, no apparent ischemic lesion was detected by DWI; however, spotty lesions were apparent in the left precentral or postcentral gyrus on hospital days 2-5. The neurological symptoms r

Chronic progressive external ophthalmoplegia: MR spectroscopy and MR diffusion studies in the brain.

OBJECTIVE: The purpose of our study was to show how, despite pathognomonic signs of cerebral involvement in chronic progressive external ophthalmoplegia (CPEO), mitochondrial respiratory chain insufficiency is associated with increased lactate and reduced N-acetylaspartate. CPEO and mitochondrial myopathy are caused by mitochondrial DNA mutations leading to impaired oxidative phosphorylation. Cortical and subcortical metabolites, cerebral diffusivity, and structural MRI were assessed to characterize possible subclinical cerebral pathology in CPEO. SUBJECTS AND METHODS: Ten patients with CPEO

MR diffusion tensor imaging, fiber tracking, and single-voxel spectroscopy findings in an unusual MELAS case.

A 23-year-old man was admitted to the intensive care unit for respiratory failure, global lower and upper limb palsy, and higher cognitive function deterioration. Imaging, performed with a combination of the MR diffusion tensor imaging, fiber tracking, and MR spectroscopy, suggested the diagnosis of an acute severe unusual mitochondrial encephalopathy, lactic acidosis, and strokelike event, which was confirmed by muscle biopsy, but fiber tracking showed unexpected unaltered white matter tracts.

Perfusion and molecular diffusion-weighted MR imaging of the brain: in vivo assessment of tissue alteration in cerebral ischemia.

The combined use of perfusion imaging (PI) and diffusion-weighted imaging (DWI) is opening a new window into the processes that occur during the first hours of ischemia. DWI detects changes in molecular diffusion associated with cytotoxic edema. PI characterizes the degree of regional hypoperfusion. Regions showing mismatches between DWI and PI, i.e. hypoperfused areas with normal diffusion behavior are considered potentially salvageable. We present results of 11 patients with an occlusion of the middle cerebral artery stem and spontaneous stroke evolution. Whereas the infarct was clearly visi

Investigating the Association of Concussion and Contact Sport Exposure History With Brain Microstructure Using Quantitative Susceptibility Mapping.

A growing body of evidence suggests that repeated concussions and exposure to repetitive head impacts may be associated with subtle abnormalities in neurological health. Prior studies have demonstrated associations of prior concussion and repetitive head impacts with the brain's microstructure, typically using diffusion magnetic resonance imaging, though the direction of these relationships has varied across groups. Quantitative susceptibility mapping is a quantitative extension of susceptibility weighted imaging that is sensitive to pathophysiology associated with neurotrauma and thus represe…

Ultrasound scanning of the pelvis and abdomen for staging of gynecological tumors: a review.

This Review documents examination techniques, sonographic features and clinical considerations in ultrasound assessment of gynecological tumors. The methodology of gynecological cancer staging, including assessment of local tumor extent, lymph nodes and distant metastases, is described. With increased technical quality, sonography has become an accurate staging method for early and advanced gynecological tumors. Other complementary imaging techniques, such as computed tomography and magnetic resonance imaging, can be used as an adjunct to ultrasound in specific cases, but are not essential to

Paediatric magnetic resonance enterography in inflammatory bowel disease.

Paediatric magnetic resonance enterography (MRE) has been increasingly employed in the evaluation of inflammatory bowel disease (IBD) in the past decade. This is in parallel with a global rise in paediatric-onset IBD, greater recognition of the hazards of ionizing radiation, especially in children, and improved treatment options. MRE is now the reference standard for assessing the small bowel in suspected or proven paediatric IBD, and is central to the diagnosis of Crohn's disease (CD), helping distinguish it from ulcerative colitis. This review will discuss the standard and advanced technique

Minor neurological and perceptuo-motor deficits in children with congenital muscular dystrophy: correlation with brain MRI changes.

Diffuse white matter changes on magnetic resonance imaging (MRI) have been a consistent feature in some children with the "pure" form of congenital muscular dystrophy (CMD) in which there are no structural changes in the brain or severe mental retardation. The aim of this study was to assess fine motor and perceptuo-motor abilities in children with CMD with and without MRI changes. Twenty-two children with "pure" CMD were investigated with a standard neurological examination and a battery of tests (Manual dexterity from the Movement ABC, test of visual-motor integration, Zurich Neuromotor test

[Use of Gadolinium in Follow-Up MRI of Multiple Sclerosis Patients: Current Recommendations].

Multiple sclerosis is the most frequent demyelinating disease of the central nervous system and is characterized by early onset and progressive disability. Magnetic resonance imaging, due to its high sensitivity and specificity in the detection of demyelinating lesions, is the most useful diagnostic test for this disease, with the administration of gadolinium-based contrast agents being an important contribution to imaging interpretation. Although contrast is essential for diagnostic purposes, its routine use in monitoring disease activity, response to treatment, and related complications is c

Residual lesions after Kasabach-Merritt phenomenon in 41 patients.

BACKGROUND: Kasabach-Merritt phenomenon (KMP) is the association of a vascular tumor and thrombocytopenic coagulopathy. Vascular tumors are either kaposiform hemangioendothelioma or tufted angioma but not "true" common hemangioma of infancy. There is a conspicuous absence in the literature regarding the late outcome and possible residual lesions after apparent clinical cure of KMP. OBJECTIVE: The purpose of the study was to analyze these residua in a large number of patients. METHODS: Clinical data on 41 patients who had KMP were accrued in an international cooperative study. The emphasis wa

Magnetic resonance imaging findings of non-mass enhancements in the breast and their association with malignancy.

Our aim is to investigate the relationship between the MRI (Magnetic Resonance Imaging) features of non-mass enhancement (NME) lesions and malignancy. MRI findings of 200 and 2 NME lesions were retrospectively evaluated. Age, family history of breast cancer, lesion pathology results, and follow-up durations of the patients were obtained from medical records. Based on follow-up findings and pathology results, the lesions were classified into 3 groups as benign, high-risk, and malignant. The association between these groups and MRI features was analyzed statistically. Segmental (48.4%) and diffu

Seizure syndrome as a first manifestation of solitary tumor-like mass lesion of PACNS: Two case reports.

RATIONALE: Primary angiitis of the central nervous system (PACNS) is an inflammatory disease involving cerebrovascular and parenchymal, and solitary tumor-like mass lesion of PACNS (TLML-PACNS) is frequently misdiagnosed as neoplastic or other inflammatory diseases. However, seizure syndrome as a first manifestation of TLML-PACNS has rarely reported before. PATIENT CONCERNS: Here, we report 2 cases of seizure syndrome, which was the first sign that presented prior to the diagnosis of TLML-PACNS by brain biopsy. DIAGNOSES: A mass lesion in the white and gray matters was detected by magnetic r

Vertebral artery hypoplasia, posterior circulation infarction and relative hypoperfusion detected by perfusion magnetic resonance imaging semiquantitatively.

Vertebral artery hypoplasia (VAH) has been considered a risk factor of posterior circulation infarction (PCI), especially in the territory of the posterior inferior cerebellar artery (PICA). But whether VAH is an independent risk factor for PCI remains uncertain and how VAH participates in the evolvement of PCI is still not clear either. Therefore, this study aims to examine whether VAH is an independent risk factor for PCI and evaluate the effect of VAH on the cerebral perfusion in the territory of the PICA detected by perfusion magnetic resonance imaging (MRI) semiquantitatively.
